Employee was using an utility knife to cut a hose off the radiator of a portable water pump. EE was cutting with right hand, they allowed their left hand to get in the way. The blade passed through the hose and made contact with left thumb. Employee was taken to clinic where they received 5 stitches. They returned to work after leaving the clinic. No restrictions, no lost time.

Miner was using a ratchet strap to bead tire while airing it up. When strap was released, it jumped making a small laceration on miner's thumb.
While using a sledge to remove a hammer a piece of metal broke off and entered left leg.
Miner was using a pry bar trying to dislodge rock stuck in the tail pulley. The bar came loose striking the miner in the mouth damaging a tooth.
Miner was using a long bar to move a motor over to mount it. EE felt like EE pulled something in EE's back. EE finished putting bolts on as long as EE could. Our miner went to the chiropractor after work thinking EE's back and hip went out. EE felt better after EE's adjustment. EE's pain came back but now it's EE's front right side around EE's waist.
While driving a brass rod out of a dredge chain link, EE used an 8 pound maul with two hands. EE slipped with right hand. As the momentum of EE's swing continued, EE's right index finger landed between the maul head and pin as they collided.
EE was using a scrapper to remove material from the side of the surge bin. EE's right hand came into contact with the roller guard during this process resulting in a broken right pinkie finger.
WAS LIFTING UP ANCHORS WITH A HAND WINCH AND EE THOUGHT THE WINCH WAS LOCKING AS IT WAS RAISED. WHEN I RELEASED THE HANDLE IT SPUN AROUND AND HIT EE'S RIGHT THUMB. UPON XRAY IT WAS DISCOVERED THE TIP OF EE'S RIGHT THUMB WAS BROKEN. A SPLINT WAS APPLIED.
REPLACING IMPACT HAMMERS. NUT WAS STUCK IN SOCKET. MINER PUT BOLT BACK IN THE NUT AND WAS HITTING THE NUT WHEN THE SOCKET SLIPPED AND HIT MINER'S LEFT INDEX FINGER.
CLEANING SHOP FLOOR WITH A SHOVEL WHEN THE SHOVEL HANDLE BROKE. A PIECE OF WOOD PIERCED THE HAND AND WASN'T ABLE TO BE REMOVED WITHOUT A MEDICAL PROCEDURE.

REMOVING A BELT CLAMP FROM A 54" CONVEYOR BELT. HE THOUGHT HE HAD HIS HAND UNDER THE ENTIRE CLAMP AND STARTED PULLING IT OFF THE END OF THE BELT. HE HAD HIS HAND BETWEEN THE CLAMP HALVES AND THE TOP CAME DOWN SMASHING THE MIDDLE THREE FINGERS ON HIS RIGHT HAND.
Employee was changing a screen on a screen plant and the wrench he was using slipped off the nut causing him to bump his elbow.
Cutting backwall support off of frame, he used pry bar to see if it was cut all the way. Pry bar slipped, pinching his thumb between support and pry bar rusulting in needing four stitches to repair slice in thumb.

Employee was prying on a smooth surface to install a wear plate in synton feeder. bar lost grip and flew back and struck ee on right side of face next to nose resulting in three stitches.
