Employee was using an utility knife to cut a hose off the radiator of a portable water pump. EE was cutting with right hand, they allowed their left hand to get in the way. The blade passed through the hose and made contact with left thumb. Employee was taken to clinic where they received 5 stitches. They returned to work after leaving the clinic. No restrictions, no lost time.

Employee was stemming a blast hole. When they were finished they were using their foot to level it off when their knee gave out.

Employee was trying to move screen with a pry bar and pulling towards themselves. Pry bar slipped and they struck themselves in the eyebrow with fist and prybar, causing a small cut on left eyebrow

While operating hand valve on dewatering equipment, employee slipped on muddy ground and struck knee against the pipe flange causing two small (approximately 1") lacerations. Employee went to ER and they closed the wound with medical adhesive, gave EE a prescription antibiotic and returned EE to full duty immediately.

The affected building is a 9-bay retail rental storage unit with 7 private renters. 2 of the bays are used by the operator- one as a wash bay and the 2nd one is primarily for storage of supplies unrelated to mining. It was Sunday, no mining activities were being conducted. The only EE on-site was in their office. No injuries. Report is being submitted to have citation terminated.

When exiting the machine employee stepped on a piece of ice causing them to roll their ankle.

Miner pick up a 1/2" chain and their left shoulder popped.

Miner was coming down the surge bin ladder and fell landing on the ground.

Miner was using a ratchet strap to bead tire while airing it up. When strap was released, it jumped making a small laceration on miner's thumb.

Miner stepped back while holding a machine shield and left knee popped.

The pipe rolled off the clean out plate striking the miner in the lower left leg causing a bruise.

Miner was standing on a 6 foot step ladder when the miner tried to step down the miner's foot got caught and the miner fell head first with head and shoulder hitting the ground first. ***Miner had personal neck surgery previously. No medical would see them. We took next available with previous surgeon. Miner was immediately released to full duty.***

In the process of changing hammers, EE was unscrewing it and lost control of the hammer injuring left index finger. (Cut & bruised.)

While using a sledge to remove a hammer a piece of metal broke off and entered left leg.

Miner was using a pry bar trying to dislodge rock stuck in the tail pulley. The bar came loose striking the miner in the mouth damaging a tooth.

Miner was using a long bar to move a motor over to mount it. EE felt like EE pulled something in EE's back. EE finished putting bolts on as long as EE could. Our miner went to the chiropractor after work thinking EE's back and hip went out. EE felt better after EE's adjustment. EE's pain came back but now it's EE's front right side around EE's waist.

While driving a brass rod out of a dredge chain link, EE used an 8 pound maul with two hands. EE slipped with right hand. As the momentum of EE's swing continued, EE's right index finger landed between the maul head and pin as they collided.

Miner was walking through our maintenance shop and tripped falling to the concrete and hitting the left side of face.

EE was using a scrapper to remove material from the side of the surge bin. EE's right hand came into contact with the roller guard during this process resulting in a broken right pinkie finger.

Miner was lifting the tongue of a load out bin to hook up to a truck and strained back.

EE was pulling into position to back up and dump stockpile truck #72308400. EE was stopped and waiting. Our Loader operator backed their Loader into the driver's door of the stockpile truck. EE thought EE was fine the day of the incident, but the next morning EE was very stiff and sore.

This incident became reportable when the miner finally agreed to go to the doctor and an MRI revealed a torn meniscus on 7-18-22. On the date of the incident the miner said miner was fine and no medical care was needed. On May 12th the miner slipped on the bumper of miner's stockpile truck when shutting the hood during pre-operation inspection.

HOOKING WATER PIPE UP TO SAND SCREW. BOLT BROKE LOOSE AND PIPE FELL AND HIT MY SHIN AND LANDED ON MY RIGHT FOOT.

WAS LIFTING UP ANCHORS WITH A HAND WINCH AND EE THOUGHT THE WINCH WAS LOCKING AS IT WAS RAISED. WHEN I RELEASED THE HANDLE IT SPUN AROUND AND HIT EE'S RIGHT THUMB. UPON XRAY IT WAS DISCOVERED THE TIP OF EE'S RIGHT THUMB WAS BROKEN. A SPLINT WAS APPLIED.

PULLING CORD ON OUTBOARD MOTOR TO START IT.

Was changing oil in drill and went to crawl inside to change hydro filters and knelt on knee wrong.

THE MINER GOT CAUGHT IN THE ROLLER OF THE TAIL PULLEY ON THE OVERHEAD CONVEYOR ON THE SCREEN BELT. LEFT WRIST AND FOREARM HAVE BROKEN BONES. RIGHT ARM HAS CUTS AND FRICTION BURNS. THE TIP OF RIGHT INDEX FINGER WAS CUT OFF AT THE FIRST KNUCKLE AND THE FINGER WAS SKINNED.

OPERATING SKID LOADER WHEN THE WIND PICKED UP AND BLEW A PARTICLE INTO MINER'S RIGHT EYE.

MECHANIC FINISHED WORK ON HOIST CYLINDER ON TRUCK TURNED TO GO BACK TO SERVICE TRUCK AND TRIPPED ON BUCKET OF SKID LOADER FALLING ACROSS BUCKET AND DRAW BAR. BRUISED LEFT SIDE, RIBS AND BACK.

MINER WAS WALKING TO THE TRUCK, SLIPPED ON ICE AND FELL TO THE GROUND. X-RAYS REVEALED TWO FRACTURED RIBS.

EMPLOYEE WAS PULLING THEMSELVES UP AND THEIR SHOULDER POPPED RESULTING IN A LEFT SHOULDER STRAIN.

TOSSING ROCK INTO THE BACK OF THE TRUCK. EE SAID FELT A POP IN LEFT SHOULDER.

REPLACING IMPACT HAMMERS. NUT WAS STUCK IN SOCKET. MINER PUT BOLT BACK IN THE NUT AND WAS HITTING THE NUT WHEN THE SOCKET SLIPPED AND HIT MINER'S LEFT INDEX FINGER.

Taking off layers of clothing getting ready for lunch break and a foreign material fell in employee's eye.

FELL BACKWARD WHILE PULLING A CONVEYOR BELT AND LANDED ON BUTT.

CLEANING SHOP FLOOR WITH A SHOVEL WHEN THE SHOVEL HANDLE BROKE. A PIECE OF WOOD PIERCED THE HAND AND WASN'T ABLE TO BE REMOVED WITHOUT A MEDICAL PROCEDURE.

Performing maintenance on drill when hand got caught in air valve.

Working on excavator and pressurized hydraulic fluid hit ee in the face causing injuries.

Preparing to pull screens from screen trailer when the screens fell hitting and pinning EE.

Tightening bolt when socket slipped and head hit hydraulic cylinder.
