Autoclave operator went to field to change over valves on Geho strainers. A technician was using a wrench on a stuck valve when the valve broke free and threw the wrench, which ricocheted and struck the autoclave operator standing nearby in the face. Fractured nose, facial lacerations, and put off work.

Ee was using a wrench to loosen a bolt, when the bolt came loose the wrench came back and struck the ee in the mouth, fracturing two front teeth. Released to full duty, tooth numbers 8 and 9 will need repair and caps.

Ee was removing a pipe plug from a strainer housing using a pipe wrench. When torqueing on the wrench, EE felt a sharp pain in the low back which worsened throughout the day. Seen at the ER, diagnose with a muscle spasm, and put off work.

Ee was cleaning Geho strainers. After isolation, the strainer housing did not fully depressurize. Ee used a wrench to poke through a layer of mud at the top of the housing, causing the housing to depressurize rapidly, releasing hot slurry that struck the ee on the left side of ee's face, and ee's left wrist. Seen at clinic, diagnosed with 2nd degree burns and released to full duty.

Task was to change leaf springs on a lube truck. While adjusting jack stand height, adjustment tube slipped and caught the right thumb of the tech. Minor laceration/nerve damage.

Mechanic working on the rebuild CBD pump split EE's lip. The employee was repacking the pump with a 2 foot bar on the packing pusher to push the packing ring further into the gland. The bar slipped off the gland and the other end of the bar contacted the employees lip. Seen at clinic and stitches were required, released to full duty.

EE was removing flange off pipe with hammer. Struck right thumb with hammer. Seen at clinic, diagnosed with a fracture to the thumb, released to restricted duty.

Ee was bending a piece of metal on trash dumpster for a chain latch when an adjustable wrench ee was using slipped and arm struck the bin, cutting the right forearm. Stitches required to close the wound, ee released to full duty at next shift.

Ee was aligning a nozzle with hole for pin, struck the inner metal nozzle with a metal hammer, and a metal shard came off the nozzle, striking the operator in the left arm. Ee seen at clinic and released to modified duty.

Ee was replacing cutting edges on a dozer, struck a piece of metal with a 4 lb hammer, and a piece of metal entered right thigh. Seen at clinic, and referred to orthopedist. Determined not to remove metal. Ee placed on restricted duty.

Ee was tightening bolts on a drive line and felt a sharp pain in right shoulder. Seen at clinic and placed on restricted duty.

Ee was removing a coupler from a splined shaft with a hammer. When the ee struck the coupler, a metal shard broke off, striking the ee in the left forearm. The metal had to be removed and stitches were required to close the wound.

Ee was checking air pressure in vehicle tires when EE felt a pop and pain in this right shoulder. EE reported the issue and returned to work. EE asked to see a doctor and was scheduled on 07/01/16. EE was seen on 07/01 and put on restricted duty. Restricted duty began on EE's next scheduled shift of 07/08.

Ee was removing outer insulation on a high voltage cable when the cable slipped and the knife he was using struck his right forearm causing a laceration. Ee was seen at the ER where stitches were required to close the wound. Released to full duty.

Employee was cutting ty-wrap holding coils of fiber together with utility knife and slipped cutting back of thumb. Employee had no lost time.

Ee was using a pry bar to adjust a piece of equipment. He felt a tearing sensation and pain in the right abdominal area. He was diagnosed with a torn muscle identified by CT scan and released to restricted duty. Further follow up on 02/23 resulted in the ee being put off work.

Ee was using a utility knife to cut open a box. The ee was not wearing gloves, and the knife slipped, cutting the ee's left thumb. He was seen by a doctor and stitches were required to close the small laceration. Ee was released to full duty.

Ee was loosening a nut on a dual brake can when the wrench slipped and his hand slid down the handle, cutting his hand. He received stitches to close the small laceration. Released to restricted duty.

While changing dozer ripper tooth, ee hit pin with 4 lb hammer and piece of metal embedded in left arm near elbow. Originally determined ok to not remove, but orthopedic surgeon recommended removal due to location of metal near joint. Surgery on 07/24/14. Ee full duty until that day.

Employee was using a hammer and a piece of metal struck him in the left arm. He was seen at the ER and the metal was removed; stitches required to close laceration. Employee released to full duty.

Employee was cutting a lemon with his retractable knife. Went to put the knife back on his belt and the blade was stuck open. He cut his left middle finger. Was seen at the ER where stitches were required to close the wound. Released to full duty.

Emp was breaking bolt loose with a breaker bar and the bolt broke loose. The bar caught the right thumb and bent it backwards. Seen at the ER, diagnosed with a fracture to the right thumb. Cast at the orthopedist, released to restricted duty.

Ee was retracting a steering cylinder. Thumb was struck by a hammer. Laceration to right index finger required sutures. Ee released to full duty.

Employee was cutting tie wrap with a pocket knife and cut his left wrist. Seen at ER where cut was closed with stitches. Released to full duty.

The employee was pulling on a pry bar throughout his shift.

EE was cutting plastic tubing with a razor knife, the blade cut through the tubing and contacted the back part of the ee's left thumb, cutting down to the tendon. The ee required surgery to repair the tendon.

Employee was working on an emulsion truck. The air acuator was stuck and he hit it with a wrench, causing it to move and pinch his right glove and index finger. He was seen at the ER and referred to an orthopedist. A portion of the bone was removed at the ER, and then more in surgery. He was released to full duty at his next shift on 10/14/2010.

While tightening a bolt, the socket broke and the employee's hand hit a sharp piece of metal. Taken to NNRH where he required stitches to close the wound. The employee was released to full duty.

Employee was using a wrench on a jumbo. Wrench slipped, emp hit rt elbow on a steel beam with force. He was seen at the ER, xrays negative for fracture, released to full duty. Went back to dr. on 06/07/2010, prescribed physical therapy and put on restricted duty effective 06/08/2010.

Employee was cutting a zip tie and cut his left middle finger with a razor blade. 4 stitches required to close the wound. Released to full duty with instructions to keep the wound clean and dry.

Employee was chipping cement from truck 35. He overextended his arms while holding a tool and felt pain is his lower right back. He was seen at the ER, diagnosed with a lumbar strained and placed on light duty for one day.

Employee was using a wrench to remove a u-joint from a lube truck. He exerted great force on the wrench and strained his right arm. He was seen at the ER, diagnosed with a strain and placed on restricted duty.

Employee pinched right index finger between reamer bit and hammer. Laceration to finger.

While lifting the right front wheel position on a Hitachi Haul truck to remove jackstand from under the front bumper, the injured employee was struck above the right eye by a 6"x5"x1.5" steel spacer placed on top of the lifting device. Resulting injury required 14 stiches to close the wound, the injured employee was treated and released to full duty the same day.

Employee was repainting a tire flange when it fell off the hangar striking his right ankle. He was seen at the ER and referred to a foot and ankle specialist. He will be having surgery to repair the fracture on 07/18/07.

EE was using a pry bar to dislodge a piece of scrap steel. When it would not come loose, he started to walk around to the other side to pry on it again, when the steel broke free and landed on his left foot. EE was wearing steel toed boots.

Employee using a chain wrench to remove an oil filter. The wrench broke through the filter causing the employee's hands to move forward, striking the right hand on the oil pan. He was seen at the ER and the laceration between the 3rd and 4th fingers was closed with 5 stitches. Released to full duty.

Jammed pipe fitting and 2 36" pipe wrenches, pipe would not turn. Employee slipped, hardhat came off in the fall and pipe wrench came free and hit employee in the head.

Employee was using a hammer to put a pin in place and a piece of the hammer broke off, embedding in the employee's left cheek. Metal left in cheek as efforts to remove were unsuccessful due to the depth of embedment.

Employee was assisting with instaling a noise shield on bolter #KMB001 and was adjusting the up right. He smashed his left index finger by using an 8 lb. hammer when the handle of hammer came in contact with the upright of the window frame. He cut and fractured his left index finger, and the laceration required stitches.

While breaking a wheel loose from UG LHD, using 5' cheater bar, felt strain in right lower shoulder area.

Employee was tying two tailpipes together. A strap broke punching his left leg against the pipe they were working on therefore breaking his left leg.

EE WAS TRYING TO PRY THE HEAD UP TO RELEASE THE CHAIN HOOK THAT GOT CAUGHT IN THE COM-A-LONG. THE PRY BAR SLIPPED CAUSING HIM TO LOOSE HIS FOOTING. HE FELL AND HIT THE RIB WITH HIS RIGHT ARM AND ELBOW.

Temporary service worker was walking pas the GET (ground engaging tool) bay during the process of changing a side protector on a CAT 922G loader bucket. While striking the protector loose (using a sledge hammer), a piece of metal from the side of the protector "flew off", striking the worker in the upper right thigh and lodging itself.

He was cleaning a mutt-tool (knife) with a rag; rag caught on knife and cut his right index finger (3 stitches).

Normal working conditions must of twisted knee while taking off tires. also dropped 6" socket on knee post accident drug test taken 12-21-04 results pending.

Employee's right little finger was caught between drill rod mast and pipe wrench causing fracture.

IE changing wear cap with 12lb sledge hammer. Chip of metal came off wear plate, lodging in ee's right arm. He was seen at the ER, told that the metal could remain in his arm & released to full duty. Upon follow up, he was referred to orthopedic surgeon,surgical removal was scheduled as metal was laying next to nerve. Surgical removal performed 06/22/04 as outpatient procedure.

Injured employee was changing a drill bit when drilling relief holes. The bit hung up on the hammer requiring the employee to obtain a better grip on the bit and in doing so did not realize his fingers were under the bit. When the bit came off, it smashed the right ring finger and little finger between it and the drill table.

The employee was hammering on the lip guard of a shovel bucket and piece of metal came off and struck his leg. He was taken to the ER for treatment where an X-ray revealed a piece of of metal embedded in his leg. The metal was removed and the wound closed with sutures.
