A maintenance repairman was checking bearing shaft side to side clearance on a feeder vane, when another employee moved the bearing shaft. Their fingertip was cut off during the movement. They sought medical treatment for care of the finger tip.

Employee was removing rocks from between the intermediate conveyor belt and felt pain in their shoulder while using a prybar. They sought medical treatment and were placed on restricted duties.

Employee was cutting a pallet that was stuck between a feed conveyor and palletizer table. When the pallet was cut loose, the palletizer table the employee was kneeling on dropped 6-8" and the employee's knee struck the table. The employee sought medical treatment and was placed on restricted work duties.

An employee was shimming the secondary crusher and trying to remove a metal wedge. As the employee used a hammer to knock the wedge out a piece of metal from the top of the wedge flew back into the employee's mouth. The employee sustained a cracked tooth.

Employee was setting spring tension on the door of a coal mill with a 36" pipe wrench. While applying pressure to the wrench, the wrench slipped from its purchase, causing the employee to lose their balance and fall to the grated surface EE was standing on. During the fall, the employee fell onto their left ankle with all of their weight, breaking EE's ankle.

Employee was helping mill tender change two FK line valves. While pulling down on the wrench to tighten the valve, the employee heard and felt a pop. The employee sought medical treatment two days later and was diagnosed with a broken rib. The employee was placed on restricted work duty.

An employee reported pain in their shoulder. The pain is potentially from a previous reportable injury. The employee sought medical treatment and was placed on restricted duties until a diagnosis is made.

An employee was changing valves at the loadout silos. When the person put a pipe into the fitting to move the valve, the bar slipped and the employee felt a pop in their shoulder. The employee was placed on restricted duty 6 days later.

Employee was tightening a bolt on a rotary feeder when the socket slipped and the right pinky finger came into contact with a metal edge causing a small laceration.

Employee was installing bolts in kiln hood doors with an adjustable wrench and strained/popped left shoulder while tightening the bolts.

While trying to drive a wedge in between the end box and the body of the secondary sizer, the employee smashed EE's pinky finger on EE's left hand between the hammer EE was using and the head of the wedge.

Employee was changing F.K. line valves on top of group 3. The bar that is used to loosen valve bolts slipped out striking him in the upper lip. Requiring 7 stitches. I filled this out when it happened on the MSHA website but it appears it never went through.

Employee was walking to tool box with hands full of tools. He tried to open a door by sticking the 48" pipe wrench through the handle to open the door. He caught his right pinky finger between the pipe wrench and door handle causing a small fracture.

Employee was using bar to try to break loose material going to a raw mill FK pump. When he pulled back on the bar he hit the end on a handrail behind him catching his finger between the bar and the handrail.

Employee was opening up a package with a razor blade knife. When he cut through the package, he cut his finger.

While using a 3 lb. hammer to drive chisel between airslide and valve, employee felt sudden pain in right shoulder. Please Note: Employee did not receive restrictive duty until 11/14/2012.

Employee was working in robo lab loosening a bolt when it slipped and cut right hand.

Employee stated that while turning a slugging wrench, the wrench had a burr, which cut his right hand during the turn.

Employee was loading equipment onto a trailer for transport. He was using a hammer to remove the pin on the frame lock when a piece of metal came off and flew into his neck, lodging close to the jugular vein. Will be removed surgically on Tuesday, May 15. Missed work on 5/10/07, but was released to return light duty as of 5/11/07. Will miss day of surgery also.

Claim was reported on 2/21/06 - While cutting cardboard tubing with a knife, the knife slipped and punctured leg.

Rock wedged in chute of crusher, using a metal bar to to unchoke the chute. The employee was punching out the chute, rock fell inside the chute catching bar and pinning right index finger against beam.

NOTE: THIS WAS A FIRST AID CASE UNTIL 9/15/03, AT THAT TIME THE EE WENT TO SEEK MEDICAL ATTN. EE WAS USING A SLEDGE HAMMER & LINER BAR WHILE CLEANING PLUGGED FEEDER CHUTE. THIS REPETITIVE ACTION CAUSED TENDERNESS TO HIS RT. ELBOW.

EMPLOYEE WAS USING AN 8 LB SLEDGE HAMMER TO BREAK UP ROCK THAT WAS CHOKING THE CRUSHER AND STRAINED HIS BACK.

PUSHING ON AN 18" WRENCH TO TIGHTEN HYDRAULIC HOSE ON ONE OF THE DRIVE CYLINDERS IN COOLER RESULTING IN A LOWER LUMBAR STRAIN.

TRYING TO BREAK UNION ON A DRAW LINE APPROX 2 1/2".

WAS USING PUNCH BAR TO REMOVE BUILD UP FROM THE BOTTOM OF THE BYPASS CONDITIONING TOWER; A LARGE CHUNK OF CONCRETE FELL ON BAR CAUSING THE BAR TO STRIKE HIM IN THE FACE.

I WAS PULLING A SADDLE ONTO THE CONVEYOR WITH A WINCH AND A C-CLAMP. THE C-CLAMP CAME LOOSE AND STRUCK ME IN THE RIGHT HAND.

EMPLOYEE WAS USING UTILITY KNIFE TO CUT RUBBER SKIRT ON #1 ROCK BELT WHEN THE KNIFE SLIPPED CAUSING HIM TO CUT HIS LITTLE FINGER ON LEFT HAND.

EE WAS USING A HAMMER TO LOOSEN DUST BUILDUP IN DUST COLLECTOR PIPE WHE DUST BROKE FREE AND BLEW INTO HIS RIGHT EYE.

EMPLOYEE WAS USING A 12 GAUGE SHOTGUN TO REMOVE BUILDUP IN IRON ORE HOPPER WHEN HE MASHED HIS RIGHT INDEX FINGER BETWEEN THE GUN AND THE DOOR.

EMPLOYEE WAS USING SLEDGE HAMMER OT RAP ON COAL PIPE TO LOOSEN BUILD-UP WHEN HE SUFFERED A STRAIN TO HIS NECK SHOULDER.

SPACKING CHECKERED PLATE WHILE CHECKING COME-A-LONG AND THE COME-A-LONG SLIPPED AND PINCHED HIS RIGHT MIDDLE FINGER.

EMPLOYEE WAS LCEANING FIRST COMPARTMENT OF #5 COOLER WHEN BURNER PIPE CASTABLE FELL STRIKING HIM ON THE BACK OF THE HEAD AND BACK.EMPLOYEE WAS KNOCKED OUT AND FELL FORWARD.

THE MAN WAS PUMPING A PORTA POWER PUSHING OFF A FALK COUPLING, WHEN THE RAM JUMPED OFF FROM BEHIND THE COUPLING & HIT HIM IN THE HEAD. THE RAM JUMPED OUT WHEN THE COUPLER POPPED LOOSE. 10 STIT CHES WERE REQUIRED.

EMPLOYEE WAS USING A LONG PIECE OF PIPE TO PUNCH FEED CHUTE IN MILL WHEN HE HANDED THE PIPE OUT IT GOT CAUGHT IN CLASSIFIER OVERHEAD & STRUCK EE

EMPLOYEE WAS USING A METAL BAR TO REMOVE CONVEYOR SCREW FROM TROUGH WHEN BAR SLIPPED TRAPPING HIS FINGER BETWEEN BAR AND COLUMN.

EMPLOYEE WAS PUNCHING MATERIAL IN HOPPER WITH METAL BAR AND STRUCK HIS LEFT LITTLE FINGER.

EMPLOYEE WAS TIGHTENING COOLER PLATES WHEN DUST FELL FROM GRATE SUPPORTS AS HE WAS MOVING RATCHET, COVERING HIS FACEA AND FALLING OVER GLASSES.

INJURED WAS USING A SCREW DRIVER TO CLEAN SEALING RING OF COVER PLATE THE SCREWDRIVER SLIPPED OUT OF THE RING AND HIS LEFT LITTLE FINGER WAS LACERATED BY SHARP EDGE OF COVER PLATE-REQUIRING 3 STITCHES.

EMPLOYEE WAS USING A METAL BAR TO PUNCH BUILDUP ON KILN AND HIT BACK OF HAND ON DOOR LATCH CAUSING LACERATION TO BACK OF LEFT HAND.
