Miner was using a pipe wrench to loosen spray nozzles on wet screen. When the threads loosened, the miner's right hand came into contact with the spray bar above causing a laceration requiring stitches to close. Miner was released to full duty.

Mostrando los 18EE was trying to break frozen chunk of sand that was lodged in feeder opening. When tried to hit chunk with sledge hammer, hammer struck steel brace, bounced towards EE causing the end of hammer handle to make contact to the EE left leg near his knee. EE was taken to the ER for X-rays. No broken bones were found. EE returned to work. EE saw dr 03/02/15, given restrictions.
Miner was changing the shaft and bearings on the head pulley of the 40/70 bucket elevator. He and another miner were trying to place a pry bar between the belt and the pulley to position the belt. The miner' s finger was pinched between the pry bar and the belt.
EE was installing belt wiper on BC02 to divert stone. EE used his personal pocket knife to "mark" the belting in order to punch holes in the belting for mounting. While EE was "marking" the belt, his knife closed, causing a laceration to his right ring finger. EE was seen at local hospital, required 6 stitches to close wound, and released without restrictions.
On 3/28/2011, employee reported to his supervisor that he had an accident on 2/7/2011. Employee stated that he fell against hand rail while trying to loosen bin indicator with tool/cheater bar. Employee stated that he injured right shoulder when he fell against handrail. Accident under investigation at this time with insurance carrier.
Employee was emptying a bucket near Tower 2; he was striking the side of a bucket with a sledge hammer to dislodge material. A small piece of rusted metal flew off the bucket striking the employee in the right lower leg causing a small laceration. The piece of metal had to be removed from the wound.
EE was performing maintenance on a horizontal roller conveyor using a hammer. The hammer slipped and hit his left index finger while he was holding a punch. No modified work available and his accident, will be a lost time.
Employee was moving a 55 gal barrel of kerosene in the Process Bldg with a chain hoist. A clasp was hooked to the barrel edge and as it was moved from the pickup truck, the clasp released and the barrel dropped about two feet onto employees rt foot. Employee had a hairline fracture on top of foot; no medical treatment, just time off for swelling to go down.
The employee was removing material buildup from the walls of the Pit hopper from a standing position above the hopper. He was using a hollow bar about 11' long and weighing about 25 lbs. He performed about 6 downward thrusts with the bar striking the hopper walls and when he continued the movement a 7th time, he felt pain in his lower back.
The employee was attempting to support the transmission oil pan of a front-end loader with his hand. The mechanic broke the seal between the transmission oil pan and the transmission with a crowbar. The oil pan broke free and fell to the concrete slab. It pushed the employee's left hand down to the concrete slab and fractured the tip of the middle finger.
EE was shoveling along 50W stacker area. While shoveling sand next to concrete wall, the corner of the shovel hit the concrete wall, stopping the EE's motion. EE's right hand went numb and his shoulder started hurting. Diagnosis from medical clinic is strained right shoulder.
EE WAS USING UTILITY KNIFE TO CUT SMALL OPENING IN RUBBER CONVEYOR BELTING MATERIAL BEING INSTALLED AS AN END SKIRT BOARD ON A TYROCK WET SHAKER SCREEN. THE EE WAS IN THE PROCESS OF CUTTING A SMALL HOLE WHEN THE KNIFE SLIPPED OFF THE WORK AND STRUCK HIS LEFT INDEX FINGER, CUTTING A PORTION OF THE FINGERNAIL AND FINGERTIP.
EE WAS MANUALLY TURNING GATE VALVE WITH A PIPE WRENCH AND WHILE PUTTING PRESSURE ON THE VALVE FELT PAIN IN LOWER BACK (LOWER BACK STRAIN).
2 EE'S WERE CUTTING A PIECE OF PIPE. WHILE ATTEMPTING TO HIT THE PIPE WITH A HAMMER, ONE EE INADVERTANTLY STRUCK THE OTHER EE'S LEFT HAND FRACTURING HIS THUMB.
WAS TRYING TO SHUT RAIL CAR DOOR TO KEEP FROM LEAKING. USED SLEDGE HAMMER TO KNOCK TIGHT AND APPARENTLY SMASHED MIDDLE FINGER ON RIGHT HAND CAUSING POSSIBLE FRACTURE.
HOLDING A PIECE OF STEEL (RADIO ANTENNA MOUNT) IN LEFT HAND, & TURNING SCREWDRIVER IN RIGHT HAND SCREWDRIVER SLIPPED OFF SCREW INTO LEFT HAND
HE WAS TAKING OFF THREE PIPE END CAPS AND WAS STANDING ON EXTENSION LADDER TO REACH THE ENDCAP EITH A PIPE WRENCH. HE PULLED DOWN ON THE WRENCHAND IT SLIPED OFF THE LADDER AND CUT HIS LEFT TH UMB. HE NEEDED SUTURES.
USING UTILITY KNIFE TO CUT A PIECE OF RUBBER TO LINE A CHUTE. KNIFE SLIPPED CUT LEFT FOREARM REQUIRED STITCHES.
