Employee was washing along a conveyor when employee lost water pressure. Hose had become disconnected at barb. While tightening up hose clamp with screwdriver, tool slipped and impacted left hand causing injury.

Employee was using a pry bar to break material loose from hopper. Bar slipped and pinched employees finger against the handrail upright.

EE was using a 3lb hammer to connect equipment. Set hammer on conveyor belt and place hand on structure. Hammer fell, striking the left thumb of EE.

A 350 HP motor was being set in place by employee. EE was attempting to line up mounting bolts with motor base when the motor shifted causing his finger to become stuck between wrench handle and motor platform.

Employee was replacing roller on conveyor. During installation of the new roller, the employee attempted to strike the roller in order to tap it into place. In doing so, he missed the roller can and hit his left index finger causing it to become lodged in between the roller can and frame. EE suffered severe laceration to left index finger and received eleven stitches.

Replacing plastic guiding chain on JLG 600S boomlift. Plastic Chain links began to burr up and ee used pocket knife to shave off burred plastic. Knife slipped and stabbed ee in left arm causing laceration.

Miner was removing gear box with a pry bar. The pry bar slipped causing his finger to strike conveyor frame. Finger was cut causing him to receive stitches.

Employee was attempting to cut screen with a utility knife and the knife slipped and cut employee on the upper thigh of the right leg.

The employee was in the process of removing a nut from a bolt. Wrench slipped cathing the right ring finger between wrench and equipment.

FEED DOOR AT PLANT WAS CHOKED AND EE HAD TO DIG IT OUT WITH 6' DIGGING BAR AND STRAINED SHOULDER.
