On 2.20.24, EE was pulling & lifting on an empty battery tray on a CAT 349 Excavator trying to remove it. It did not move, so EE used pry bar to remove. EE reported back soreness the next day but did not want to go to doctor. EE requested to see doctor on 2.24.25 & doctor released to return work. On 2.25.25 doctor required physical therapy and allowed EE to return to work.

Employee was replacing screens on screen deck. While employee was pressing down on screen cloth with pry bar employee strained his upper back.
Employee was adjusting the position of a tire/wheel assembly with boom hands during tire installation. The tire/wheel assembly was pressed against the rock bar on unit. As EE reached in to move the rock bar out of the way, the tire wheel assembly moved forward "pinching" his right middle finger between the boom hand and rock bar.
While working on bin 24 breaking-up some build-up material, using a chipping bar, his right hand was placed at the end of the bar. a large chunk of material hit the tip of the bar, causing the end with his hand to go up striking an I-beam.
While removing a 70 Lbs chain fall from I beam located 10feet on roof, he went to lift the unit and sustained a hernia on left side of lower abdominal area
While cutting thick rubber conveyor material at the hot mix plant with a utility knife, the knife slipped and cut the employee above his left knee. The employee went to the clinic where his cut was sutured and returned to work the same day without restrictions.
While removing ball drop doors. while using a hammer he hit the fleshy area of his left hand between the thumb and the index finger.

While working on ID-360 fan, went to stop the fan from turning, using a pipe wrench. When the wrench got the shaft the wrench also went down pinning the right index finger against the frame of the shaft housing.
Employee was pulling on a wrench to loosen a bolt that was too tight and continued to pull, he felt a strain to right shoulder.
IP put too much tension on a belt wiper. IP decided to relieve some of it and in this process the wrench slipped out of his hand causing it to go 180 degrees around the tensioner and strike him on the right calf.
EE was working with a crew on the primary feeder removing a timing gear. The crew was using a porta-power to pull the gear. While He was pumping on the porta-power pump, one of the bolts on the gear puller broke. As a result, the porta-power ram fell from the feeder striking him on the left arm.
The employee was cleaning material out of a conveyor chute when he caught his finger between his cleaning bar and the frame of the chute.

EE WAS USING A PIECE OF ANGLE IRON INSTEAD OF A HAMMER TO SECURE THE LID OF A 55 GALLON DRUM. HE STRUCK HIS RT. SMALL FINGER WITH THE ANGLE IRON. THIS RESULTED IN A FRACTURE TO THE TIP OF THE FINGER AND DAMAGE TO THE NAIL.
EE WAS RELEASING THE LOAD BINDER ON A USED OIL TANK WHEN THE BINDER SUDDENLY RELEASED AND STRUCK HIM ON THE RIGHT FOREARM.
EE WAS CLEANING A CHUTE WITH A PICK. AS HE WAS CLEANING A CHUNK OF COMPACT DIRT AND CLAY BROKE FREE AND LANDED ON HIS RIGHT LEG AND FOOT. \
EE STATES THAT HE HUNG A 1 1/2 TON COME-A-LONG ON A PAD EYE. HE THEN PUT THE COME-ALONG IN THE FREE WHEEL POSITION. THEN HE STARTED PULLING THE SLACK OUT OF THE CHAIN. THE FREE WHEEL DISENGAGE D CAUSING THE HANDLE TO STRIKE HIM IN THE MOUTH.
EE STATES THAT HE WAS HELPING THE IRON WORKERS OPERATE A CHAIN FALL AND A SHAKEL FELL APPROX 10', STRIKING HIM IN THE MOUTH, CAUSING CONTUSION OF THE MOUTH AND 2 FRONT TEETH.
WHILE PRYING TO MOVE A SECITON OF DEMO'D VESSEL THE LOAD SHIFTED AND CAUSED TH ESLEEVER BAR TO SHIFT RAPIDLY CUTTING EMPLOYEES RIGHT AR JUST BELOW THE ELBOW (INSIDE)-LACERATION 1 TO 1/2 INCHES .

EE'S WERE DOING MAINTENANCE ON CRUSHER, HAMMER SLIPPED FROM EE'S HAND WHILE HAMMERING, STRIKINGOTHER EE WITH HAMMER.
WHILE STRIKING A BOLT WITH A HAMMER, THE HAMMER GLANCED OFF THE BOLT, HITTING HIS FINGER.

EMPLOYEE WAS USING A STEEL HAMMER TO LOOSEN AN ANVIL IN A CRUSHER. HAMMER WAS MUSHROOMED AND A PIECE BROKE OFF AND PENETRATED HIS CALF.
EE AND ANOTHER OPERATOR WERE REPLACING A DOOR ON FINISH MILL 1 USING A BUILDING CABLE HOIST. HE WAS USING THE HOIST BLOCK AS A HAND HOLD TO STEADY HIMSELF. HE WAS WEARING LOBES AND AS THE CABL E WAS RETRACTING INTO BLOCK HIS GLOVE CAUGHT ON CABLE VURRS AND PULLED HIS FINGER INTO PULLEY.
EMPLOYEE WAS USING A BELT KNIFE TO CUT A PIECE OF CONVEYOR BELTING. HE WAS EXERTING A LOT OF WEIGHT ON KNIFE TO CUT THROUGH BELT. WHEN KNIFE CUT THROUGH BELT EMPLOYEE LOST HIS BALANCE AND WAS UNABLE TO STOP HIS FORWARD/DOWNWARD MOTION AND HIS ARM FOLLOWED THROUGH ONTO HIS RIGHT LEG CALF.
EMPLOYEE WAS IN MATERIAL TUNNEL CLEANING OUT A FEEDER WIOTH A CLEANING BAR USING IT TO BREAK UP MATERIAL STUCK TO INSIDE OF FEEDER HE STRUCK THE MATERIAL WITH THE BAR THE BAR WITH HIS HAND ;ST RUCK THE CORNER OF THE FEEDER
EMPL WORKING IN CLINKER STORAGE TUNNEL BREAKING UP BUILDUP ON WALKWAY NEXT TO CONVEYOR.USING POKE BAR TO BREAK BUILDUP.AS HE RAISED BAR TO STRIKE ,TOP OF BAR STRUCK OVERHEAD PIPES AND LOOSE BU ILDUP OF CLINKER DUST CAME LOOSE AND FELL DOWN OVER EMPL HEAD & BODY.SOME DUST GOT INTO EYES AROUND SAFETY GLASSES.

EMPLOYEE WAS USING A NON LOCKING POCKET KNIFE TO CUT HOLE IN A PIECE OF CONVEYOR BELTING. WHILE BEARING DOWN ON KNIFE TO CUT HOLE, THE KNIFE CLOSED ON HIS FINGER.
EMPLOYEE WAS USING A PRY BAR TO LOOSEN SOME MATERIAL BUILD-UP. AS HE BROUGHT BAR DOWN IT STRUCK A ROCK AND THE SUDDEN STOP AND JAR CAUSED HIS LOOSE GRIP TO SLIP AND TWIST HIS LEFT WRIST.
EMPLOYEE WAS REPLACING THE DISCHARGE DOORS ON THE COLKE GRINDING MILL.HE WAS USING A LARHE AHMMER TO REPLACE THE KEEPERS.THE AREA HE WAS WORKING WAS CRAMPED,HAD POOR LIGHTING AND HE WAS RUSHI NG TO COMPLETE.HE HIT THE EDGE OF THE KEEPERS AND THE HAMMER SLID PAST AND HIS LITTLE FINGER STRUCK THE FLANGE.
