IP was using a air powered cutoff tool to trim a bolt. IP was wearing safety glasses. Debris passed between the safety glasses and the IP's face and entered the IP's eye. IP reported the injury the following day and was taken to clinic for treatment. IP was put on light duty for 3 days.

The employee was working on the secondary plant discharge line removing a bolted cover on a check valve. He was positioning a wrench on a nut when co-worker activated a 3/4" drive impact wrench on the bolt head. This made the wrench spin with the nut and EE's finger was caught in a pinch point between the wrench and the pipe flange.

The employee was cleaning material out of a conveyor chute when he caught his finger between his cleaning bar and the frame of the chute.

Employee was descending the ladder from his R60 Haul Truck. As he reached the ground he felt a pain in his back.

Employee was cleaning under a conveyor belt and felt pain in his lower back.

Employee was greasing a dry screen when a rock bounced off the screen and hit him in the neck.

Employees were replacing an idler on frame. As they rotated it in place, the idler bracket pinched the employee's finger against the frame.

Employee was operating a loader bucket. The loader bucket dropped to the gound by itself and it jerked the entire piece of equipment.

Employee was cutting a conveyor belt (piece). The utility knife slipped and cut the employee on his right leg.

Employee does not know exactly when injury occured. It is an on-going process. The diagnosis is tennis elbow. The employee at first gave it some time for the pain to go away. However, it has not been determined if it is work related but is under company doctor's care.

Lost control of haul unit vehicle while driving to the pit.

THE EE WAS WALKING DOWN THE STAIRS AND HE SLIPPED AND FELL. THE STAIRS WERE VERY NARROW WHICH CAUSED HIS FALL. HE STRAINED HIS RIGHT KNEE.

EE WAS USING A DIE GRINDER WHEN THE WHEEL BROKE APART CAUSING A FOREHEAD LACERATION.

EE WAS RELEASING THE LOAD BINDER ON A USED OIL TANK WHEN THE BINDER SUDDENLY RELEASED AND STRUCK HIM ON THE RIGHT FOREARM.

EE WAS DESCENDING THE LADDER OF HIS LOADER WHEN HIS RIGHT FOOT STEPPED ON AN AIR HOSE WHICH CAUSED HIS FOOT TO ROLL. HE FELL ONTO HIS BACK.

REPLACING TRACKS ON 235C EXCAVATOR. EMPLOYEE CUT FINGER BETWEEN TRACKS AND PIN.

CAN FELL INSIDE AIR CONDITIONER VENT AND HE REACHED IN TOO FAR TO RETRIEVE IT FINGER TOUCHED BLOWER FAN.

FOLDING UP CHUTE ON STEMMING TRUCK. LAST TWO FINGERS ON RIGHT HAND GOT CAUGHT IN PINCH POINT.
