Employee was using a pry bar to hold a guard in place near a conveyor belt so that he could repair the guard. The bar came into contact with the moving belt causing the bar to spin and be pulled from his hands.

Employee struck the tooth with a sledge hammer and a piece of the tooth came off and punctured the employee's stomach. He was taken to a local ER where they did an MRI and decided to leave the small piece of metal in him. He received stitches and was released to full duty.
Employee was digging ice and rock out of the VS-3 tower chutes when he felt a twinge in his wrist.

ANOTHER EE WAS DRIVING A BEARING ONTO A SHAFT WHILE EE WAS HOLDING A BLOCK OF WOOD ON IT. THE EEMISSED THE BLOCK & STRUCK EE HAND CAUSING A LACERATION OF THE LEFT RING FINGER-NEEDED 5 STITCHES
EMPLOYEE WAS KNEELING ON A CATWALK CUTTING A PIECE OF RUBBER WITH A UTILITY KNIFE. THE KNIFE SLIPPED AND CUT HIS LEG.
EE WAS USING A UTILITY KNIFE TO CUT A PIECE OF RUBBER HOSE WHEN KNIFE SLIPPED CUTTING HIM IN LEG.
