A miner was injured after slipping and falling as EE walked to EE's equipment.

Loading a shot in the extreme heat, became dehydrated.

Miner was kneeling while performing maintenance. When miner stood up, EE felt pain in EE's right leg.

EE was handing tools to another ee

The employee was installing a lower tool bushing in the primary crusher Rammer E68 hammer. EE was using a (hand held) hammer to drive in the lower tool bushing retainer pin. One of the times EE struck the pin sliver broke off the end of the 1 ¼" diameter lower tool bushing pin, and embedded in the employee's arm.

Employee was lifting a bucket containing an aggregate product sample for QC testing onto pick-up truck bed. While lifting, EE incurred a thoracic strain, resulting in restricted work duty.

After installing new forks, EE used forklift to facilitate loading of damaged forks into service truck. As EE slid and shimmed the second fork into the truck, a fork on the forklift shifted, allowing the heel of the damaged fork to drop in between the forks on the forklift. As it dropped, a bur on the damaged fork caught sleeve of EE pulling arm into the crush point between forks.

Pulling in large cables for electrical work. Felt a pop in EE's wrist. Pulled a tendon in wrist.

EE WAS CUTTING BOLT OUT OF A TRUSS FOR DEMO, AFTER THE LAST BOLT WAS CUT AND STARTED TO REMOVE THE TRUSS IT HUNG UP, ONCE IT BECAME FREE THE SHEETING BENT BACK AND CAUGHT EE IN THE MOUTH. HE WAS TREATED AND HAD STITCHES IN HIS UPPER LIP.

We told a contractor that we would no longer need their services on 01/29/2015. We saw this contractor 2 times after this date and there was no mention of an injury. We asked him to return our companies equipment on 02/02/2015 and he then told us that he hurt his back and stomach. I do not have anymore detail on the injury because this is all the detail that the contractor gave us.

**THIS IS A DUPLICATE REPORT OF THE CONTRACTOR'S 7000-1** Removed power to main switch of 4160 volt in MCC substation and locked out removing power to building. Removed buss bars cover to show other electrician what the scope of work was. While showing him, the injured contractor employee's hand touched the buss bar and arced through a wrench he was holding causing a flash.

Employee was operating skid steer when emergency brake activated at speed causing employee to hit head on cross-member and causing a neck strain

After operating a Caterpillar Articulating Haul Truck, EE began to complain of knee pain. EE has been diagnosed with a torn Meniscus and will undergo surgery to repair the injury.

While exiting a yard loader, EE slipped on snow and strained his knee.

EE was working under raised hood on loader in shop. Hood fell striking employee on shoulder.

Blaster was coming down office steps @ quarry office and slipped. It was raining and steps were wet. Starting falling and tried to catch himself. He grabbed railing and felt something pop in his arm.

Employee attempted to sit on the mid rail of a railing. The mid rail is removable so that screen can be replaced. The mid rail became dislodged and the employee fell to the level below.

Employee was stemming holes and heard a pop in his wrist.

Crew was loading gas shot during the hottest part of the day. Employee was stemming holes and became overheated. Employee passed out and was transported to local hospital - heat related.

Employee was removing and replacing the worn teeth on a Caterpillar 992G loader bucket. Employee struck the tooth with a sledge hammer and a piece of the tooth came off and punctured the employee's stomach. He was taken to a local ER where they did an MRI and decided to leave the small piece of metal in him. He received stitches and was released to full duty.

Employee was digging ice and rock out of the VS-3 tower chutes when he felt a twinge in his wrist.

Employee was walking around a truck when he slipped on some ice and fell back striking his head and left shoulder on the ground.

Employee was hurrying to get a 992C loader started and into the pit, when he climbed the ladder with his lunch pail in his hands. He reached for the handrail at the top of the ladder, missed it and fell to the ground striking his buttocks and knocking the wind out of him.

Employee was cleaning off a catwalk and lifted a steel discharge chute when he felt pain in his lower back.

EE WAS WALKING ON A CATWALK WHEN HE LOST HIS FOOTING AND FELL DOWN ONTO HIS LEFT KNEE CUTTING IT. THERE WAS NO OBSTRUCTIONS ON THE WALKWAY THAT HINDERED HIS TRAVEL.

EE WAS OPERATING A CAT HAUL TRUCK INT HE AREA WHERE WE WERE DIGGING S UMP WHEN HE STARTED PULLING OUT OF SUMP THE MATERIAL INT HE BED SLID BACK CAUSING THE TRUCK TO STAND UP ON ITS TAILGATE. E E WAS BANGED AGAINST DOOR AND INSIDE OF CAB CAUSING THE LACERATIONS TO HIS HEAD, FACE AND NECK.

STEMMING LOADED BLAST HOLES (SHOVELING GRAVEL). STEMMING MATERIAL MORE COARSE THAN USUAL.

WHILE UNLOADING CONVEYOR PARTS FROM A TRUCK, THE EE LIFTED ONE END JUST HIGH ENOUGH TO PUT A SLING UNDER IT AND FELT A PINCH IN HIS BACK.

EMPLOYEE WAS ADJUSTING AN IDLER ARM WITH A HAMMER ON BF-5 HE STRUCK HIS LEFT INDEX FINGER ON THE FRAME DURING THE BACK SWING.

WHILE WELDING SPARKS ENTERED THE SIDE OF THE GOGLES HE WAS WEARING CAUSING WELDER FLASH BURN TO HIS RIGHT CORNEA.

ROCKS STUCK IN C7 CHUTE FELL ON MY BACK, SHOUYLDERS AND HEAD.
