On 03/18, EE reported that EE was having back pain that potentially stemmed from a task EE performed on 03/11. On the evening of 03/11, the employee was using a metal rod to clear debris from a bin level sensor. Employee states pain was minor starting on 03/12 but worsened to the point of reporting on 03/18. Incident reclassified to Recordable on 03/20 after visiting clinic.

Small metal shard from hammer flaked off and entered hand. Was removed by doctor and 4 stitches to close

Third party temporary lab technician working un-supervised, took her personal pocket knife out to punch a new hole in her belt and cut her left index finger. She was given 6 stitches and prescription pain medication to take as needed at home off duty; and was released for full work duty.

The employee used a rounded tip box blade cutting tool to cut a bad section of a hose out. Dust blew from the hose and into the face of the employee causing the employee to cut his hand with the cutting tool.

EMPLOYEES WERE CHANGING JOURNAL ASSY. UPON INSTALLING REPLACEMENT JOURNAL, THE TOOL USED TO REMOVE / REPLACE JOURNALS SLIPPED OFF JOURNAL & POPPED UPWARD SMASHING EMPLOYEES FINGER BETWEEN TOOL & BOLT.

EE ALLEGED WHILE LINING UP BOLT HOLE IN THE MILL WALL. HE STRUCK A SPUD BAR WITH A HAMMER & A SMALL CHIP FLEW OUT & LODGED IN HIS LOWER CHEST AREA.

EE ALLEGED HE WAS ASSISTING ANOTHER EE DISASSMEBLE #2 FEED SCREW. HE WAS HOLDING A DRIFT PIN WITH PLIERS WHILE BALL WAS SWINGING THE HAMMER. THEHAMMER MISSED THE DRIFTPIN & HIT HIS LEFT THUMB
