Miner was installing a toggle plate in the jaw. The bar being used to center and seat the plate slipped striking the miner above the left eye causing a minor laceration.

Employee was helping replace V-belt on motor under feeder. The hook at the rachet end of the come-a-long came off the beam which allowed the come-a-long to strike employee in the head on the right side of his hard hat. Employee was knocked unconcious. Was taken by air ambulance and then later released.
Employee was using a razor knife to cut a plastic zip tie off to repair a broken guard. His left hand was in the line of fire and cut left index finger on top of knuckle. Requiring 3 stitches to close.
Employee was prying a 8 to 10 inch rock from a plugged chute. As the rock broke free, the bar moved and pinched left ring finger between bar and motor mount. No stiches were required but occupational therapy will be required to sustain movement and feeling in finger.
Employee was inspecting railcar doors. He assisted another employee with closing a door with a jack. The jack handle came up and struck the EE in the cheek as he hammered the lock down on the door.
EE was performing regular maintenance at the jaw crusher. While turning a wrench, he felt a pop in his R. index finger. Pain radiated towards his palm. He thought that it would get better but it did not. He reported this incident one week later on 5/9/05. He was taken to the doctor and is being treated for an inflammed tendon.
THE EE WAS CUTTING SKIRT RUBBER WITH A UTILITY KNIFE. THE KNIFE SLIPPED AND CUT THE EE ON THE TOP OF THE LEFT HAND. THE CUT REQUIRED 5 STITCHES.
EMPLOYEE PICKING UP CRUSHER HEAD WITH PULLEY. HAD HOLD OF CRUSHER HEAD WITH HAND. CHAIN SLIPPED, SMASHING FINGER.
EMPLOYEE WAS SMASHED IN RIGHT HAND BY 12 LB SLEDGE HAMMER ON 3 MIDDLE FINGERS, 26 STITCHES, NO FRACTURE. SLEDGE HAMMER WAS BEING SWUNG BY ANOTHER EMPLOYEE.
EMPLOYEE WAS HITTING A BEARING RACE WITH SHOP HAMMER AND A PIECE OF THE RACE BROKE OFF AND STRUCK HIS LEFT ARM AND EMBEDDED A PIECE OF STEEL INTO IT.
UTILITY KNIFE SLIPPED WHILE CUTTING CONVEYOR BELT AND STRUCK EE IN THE LEG JUST ABOVE THE RIGHT KNEE.
EE WAS USING A SLEDGE HAMMER WHEN HE CAUGHT HIS FINGER BETWEEN THE HANDLE AN A CONE LINER.
EE WAS USING HIS RIGHT FOOT TO PUSH ON A RATCHET IN ORDER TO TIGHTEN A SCREEN BOLT.
EE WAS TIGHTENING A NUT WHEN RATCHET SLIPPED ANDHE FELL BACKWARDS HITTING HIS LOWER BACK ON HANDRAIL.
JAW OPERATOR WAS BUSTING A ROCK OUT OF GRIZZLY FEEDER WITH A SLEDGE HAMMER. A CHIP OF ROCK FLEW SOME 25' UPWARD, HITTING EE IN THE FOREHEAD.
EE WAS REMOVING THE BOLTS ON A LOADER BUCKET CUTTING EDGE. ONE OF THE BOLTS WAS TURNING IN THE HOLE. HE TRIED TO HOLD THE HEAD OF THE BOLT WITH HIS HAND AS HE LOOSENED THE NOT. THE HEAD WAS WO RN AND HAD A SHARP EDGE THAT CUT HIS FINGER. HE WAS TAKEN TO CHOCTAW MEMORIAL HOSPITAL E.R. WHERE HE RECEIVED SEVEN STITCHES IN HIS RIGHT MIDDLE FINGER.
INSTALLING SHANK ON LOADER, STRUCK TOOTH WITH HAMMER. A PIECE OF METAL WENT THROUGH EMPLOYEE'S SAFETY GLASSES.
HE HAD A 16 INCH PIPE WRENCH THAT WAS TIGHTENING A CHAIN CLAMP, THE WRENCH SLIPPED, AND HIT HIS THUMB.
EE WAS CLOSING ROLL OUT CHUTE USING A 4' BAR ANDTWISTED HIS BACK.
STRUCK PIN WITH A 20 LB HAMMER, A PIECE OF METAL STUCK IN HIS LEFT ARM, RESULTING IN STITCHES.
EE WAS JUMPING THE STARTER TERMINAL WITH A SCREWDRIVER, AND SLIPPED. HIS LEFT INDEX FINGER WAS CRUSHED BETWEEN THE RING GEAR AND THE STARTER DRIVE.
EE WAS USING PRY BAR TO CLOS RAIL CAR DOOR, BAR SLIPPED, HITTING EE IN FACE.
