Employee was installing a section of 20 inch steel pipe to a 90 degree fitting. Two employees utilized hand tools to attempt to free a frozen nut. Employees hand was rotated toward the pipe causing his right ring finger to be caught in between the open end wrench and the pipe. Employee received a laceration requiring sutures to correct.

While Mechanic's Helper was swinging sledge hammer, he missed intended target; causing a metal fragment to chip off.

EE WAS USING A 4' PRY BAR TO SLIDE A PIECE OF DECK PLATE THAT HE WAS WORKING ON IN THE CF MAINTENANCE SHOP. THE PRY BAR SLIPPED AND THEN SMASHED HIS LEFT INDEX FINGER (THROUGH HIS GLOVE) BETWEEN THE PRY BAR AND A PIECE OF ANGLE.

Miner was hammering a keeper pin into place on a dragline bucket tooth when he missed the pin, striking the tooth itself with the hammer. A sliver of metal chipped off of the tooth and struck the miner between the first and second finger of his left hand. It required three stitches to close the laceration and the employee was released back to work with no restrictions.

While trying to loosen bolts on plate; IW felt sharp pain in right shoulder

EE was attempting to insert a spike puller to remove a spike, when the action of hitting said spike puller dislodged a piece of an old weld that had adhered itself to the spike. When this piece became dislodged, it flew up & hit him in the neck causing a laceration that required two (2) stitches to close. EE was transported to ER for treatment. Released, no duty restrictions.

While attempting to check final drive on D8 Dozer #1702; IW was using a ratchet to pull up causing pain to in middle of back.

A contractor employee was in the process of removing electrical zip ties from bundled electrical cords using a pocket knife. The knife slipped and the employee was cut on his left thumb through his cotton work glove. The laceration required three sutures to close. The miner was released back to work with no restrictions.

Miner was tightening bolts on a piece of equipment when a bolt snapped. The sudden decrease in resistance caused the Miner's hand to fly back, mashing his right ring finger between the wrench handle and a nearby pipe. This resulted in the Miner receiving 5 sutures on the affected finger.

Laborer was moving tools & reports tweaking back muscle while pulling on rope. Laborer was immediately removed from jobsite to office to report injury and taken for medical evaluation. Local MSHA office and mine company personnel notified immediately.

Employee was driving wooden wedges under the tub of a 1370 dragline to tighten the cribbing under the tub. On his backswing, he hit some adjacent cribbing, altering the path of the sledge hammer and hit his left foot.

While cutting and placing rubber materials into an inlet chute employee was holding a rubber cutting knife when he accidentally dragged the knife across his left thumb.

Employee was attempting to unchoke an overflow line with a small piece of pipe when the obstruction broke free which caused his left hand to strike the overflow box.

SPROCKET SEGMENTS WERE WORN WHICH CAUSES SHARP EDGES.

Employee had attached a chain cumalong to the a section of grating and was applying upward pressure. He had also placed a 30" spud bar into the grating with the intention of applying leverage to the low spot. The weld and clip holding the grating broke loose causing the spud bar to be thrown upward and strike the employee on the nose.

EMPLOYEE WAS LOOSENING BOLT, WRENCH SLIPPED, HIS HAND HIT METAL SPUR CAUSING SMALL LACERATION ON RIGHT HAND.

EE WAS BOLTING PIPE TOGETHER WHEN THE TIP OF HIS FINGER WAS SMASHED BETWEEN THE HEAD OF A BOLT AND THE PIPE.

EE WAS SECURING A MOTOR WITH A COME-A-LONG WHEN THE CHAIN SLIPPED AND CAUSED HIS RIGHT ELBOW TO STRIKE THE TAILGATE OF THE PICKUP TRUCK.

WHILE TURNING WRENCH TO LOOSEN OIL FILTER, HE FELT PAIN IN BACK. PULLED MUSCLE.

EE WAS STRIPPING THE INSULATION OFF A PIECE OF ELECTRICAL WIRE WHEN HIS KNIFE SLIPPED AND LACERATED HIS LEFT THUMB.

EE WAS SECURING A DREDGE ANCHOR ROPE WHEN IT SLIPPED AND PULLED HIS HAND INTO A PINCH POINT. THIS CAUSED A LACERATION TO HIS LEFT INDEX FINGER.

EE WAS USING HIS POCKET KNIFE TO LOOSEN A SCREW WHEN THE BLADE BROKE AND BUT HIS FINGER. THE BLADE WAS CLOSED AT THE TIME IT BRAKE.

CUT BOLT WITH HACKSAW AND HIT THE END OF BOLT WITH HAMMER. BOLT BROKE AND HIT DOZER BLADE AND THEN INTO LEG.

EE WAS CUTTING A PIECE OF THIN METAL WITH A PAIR OF SHEARS. WHEN HE RELEASED THE METAL IT SPRANG BACK AND STRUCK HIS RIGHT WRIST AND HAND.

EE WAS TAKING A NUT OFF A LARGE BOLT WHEN THE WRENCH HE WAS USING SLIPPED CAUSING THE BOLT AND NUT TO FALL ON HIS RIGHT INDEX FINGER.

EE WAS TIGHTENING A BOLT WHEN HE FELT A POP IN HIS LOWER BACK.

CHANGING POSITION ON BLADE AND WAS HAMMERING WITH STEEL PIN, HIT FINGER.

WHILE CUTTING A ROPE OFF OF THE PRIMAING LINE HOSE, THE EE HAD HIS THUMB IN THE PATH OF THE KNIFE TO WHERE THE KNIFE COME IN CONTACT WITH HIS THUMB CUTTING ABOUT 1/3 OF THE LEFT THUMB NAIL.

EMPLOYEE WAS HELPING A VENDOR EMPLOYEE LOAD AN 8INCH HOLLAND PUMP HEAD ON TO A TRUCK WHEN THE CAHIN WHEN THE CHAIN USED TO SECURE A COMEALONG BROKE LOOSE FROM SITE ATTACHMENT POINT. THIS ALLOW ED THE PUMP HEAD TO FALL APPROXIMATELY 1 1/2 FEET ONTO THE EMPLOYEES RIGHT FOOT.

EE WAS USING A PRY BAR TO REMOVE A ROCK FROM A PUMP RACK BOX WHEN THE BAR SLIPPED CAUSING HIM TO HIT HIS WRIST ON A SHARP PIECE OF METAL.

EMPLOYEE WAS BOLTING MATRIX PIPELINE TOGETHER WHEN HE FELT IRRITATION TO RIGHT EYE.

WHILE EMPLOYEE WAS SHOVELING SAND OUT OF DITCH, EMPLOYEE BESIDE HIM ACCIDENTLY HIT HIS RIGHT RING FINGER TIP WITH A SHOVEL.

MAKING ADJUSTMENT OT CASTER WHEEL ON SHEEL BUBBY WITH 8 LB HAMER. HAMMER BLOW BLANACED OFF CASTER CAUSING LEFT HAND TO GRAZE CORNER OF TOP ANGLE CUTTING 1ST DIGIT LEFT HAND, 1S KNUCKLE. CUT TOOK 6 STITHCES XXXX RELEASED TO FULL DUTY STATUS.

WHILE WORKING IN STRUCTURE , EE WAS TAPING ENDS OF FLOOR IN PREPARATION FOR CONCRETE PORE. HE WAS INA SQUATTING BENDING POSITION FOR A PROLONGED PERIOD OF TIME. IMMEDIATELY NOTICED STIFFNESS I N BACK, LATER WAS VERY RESTRICTED IN MOVEMENT.

THE EMPLOYEE WS PRYING ON A HORIZONTIAL BEAM IN AN ATTEMPT TO MAKE A CONNECTION WITH A COLUMN. THE PRY BAR HE WS USING SLIPPED LOOSE, STRIKING HIM ON THE RIGHT CHEEK. THIS RESULTED IN A LACERA TION REQUIRING TWO STITCHES. THERE WAS NO RESTRICTED ACTIVITY.

VICTIM MOVING TOWARDS SHELL LADDER ON SCAFFOLD WHEN ANOTHER INDIVIDAUL ON ANOTHER LADDER NEXT TO VICTIM STRUCK A BULLPIN A GLANCING BLOW CAUSING THE HAMMER TO POP OUT OF HIS HAND STRIKING VICT IM ON THE NOSE.

EMPLOYEE WAS USING FURNITURE JACK TO MOVE A DESKWHEN HE RELEASED THE STOP TO LOWER THE DESK, THEJACK HANDLE FLEW UP AND HIT EMPLOYEE ON THE LEFTJAW CAUSING THE RIGHT JAW TO DISLOCATE.

EE WAS TIGHTENING A 3/4 NUT ON A PIN KEEPER ON THE DRAGLINE FAIRLEAD THE WRENCH SLIPPED OFF OF THE NUT AND HE LOST HIS FOOTING AND STEPPED BACKWARDS A FEW STEPS AND SAT ON A PRY BAR STICKING U P IN A FIVE GALLON PLASTIC BUCKET THE BAR PUNCTURE THE BACK SIDE OF HIS RIGHT THIGH. THE DR WANTED EE TO STAY OFF HIS LEG FOR A FEW DAYS.

INJURED WAS ATTEMPTING TO LATCH A RAILROAD CAR DOOR WITH A 6' PRY BAR. HE PLACED HIS RIGHT FOOT UNDER A SPRAY BAR TO INCREASE HIS LEVERAGE. THE BAR SLIPPED FROM THE DOOR LACH, CAUSING INJURE D TO LOSE HIS BALANCE. HE FELL ON HIS LEFT SID E WHILE HIS RIGHT FOOT REMAINED STATIONARY.

EE AND THREE PEOPLE WERE CHANGING BUCKET TOOTH ON THE 7-32 DRAGLINE. ONE OF THE TEETH WAS STUCK AND WOULD NOT COMEOFF OF THE BASE. ONE OF THE WORKERS STRUCK THE TOOTH NEAR THE TIP WITH A HAMME R. A SMALL PIECE OF METAL CAME LOOSE AND STRUCK HIM IN THE UPPER LEFT LEG.

DON WENT DOWN TO ADJUST THE HOIST FRICTIONS WHILE TIGHTENING THE ADJUSTING BOLTS ONE BECAME DIFFICULT TO TURN WHEN HE APPLIED MORE PRESSURE HE STATED THAT HE FELT A SUDDEN SHARP PAIN IN BACK
