South Pasture Mine Metal/Non-Metal

While ascending spillway stairs employee slipped and fell causing EE to twist EE's ankle. Sprain was treated conservatively. Employee's personal Physician ordered X-ray due to discomfort. Company Physician had X-ray repeated which revealed a fracture. Employee was placed on restricted duty on 2/11/20.

6 man crew was planting vegetation in 18" of water. Worker was bit on left side by an alligator. Pre-job inspection showed no alligator presence. Spotter in place did not see gators until incident occurred. Employee taken to emergency room treated for injury on left hand, significant damage to thumb. Also cuts on torso under left arm and abrasions on back and stomach.

While performing inspections/PM EE felt something in left eye. EE did not report to supervisor and went to a walk in clinic the next day. A foreign object was found and removed from left eye. EE reported when returning for normal shift 12/11, Restrictions - No driving CMV - other accommodations have been made.

A front-end loader traveling on a mine road broke through into a water conveyance ditch and overturned. A contractor noticed the overturned equipment and responded to the site. Contractor employees immediately notified Mosaic supervision who activated the site ERT. ERT contacted 911.

Employee was dismounting off of the payloader when they fell, landing on the left wrist. Employee misjudged the distance from the bottom step to the ground and fell to the ground. The ground was uneven where the employee was dismounting off of the payloader.

Walking to the water jack, the employee's right leg slipped out from under EE and caught self by planting left leg. Emp was seen by company nurse on 2/13 given ibuprofen and ice packs. On 2/16 company nurse re-evaluated emp placed employee on Restricted duty due to the swelling and complaint of pain.

Employee exited the truck and closed the door on right hand ring finger. Resulting an approximate 2 inch avulsed area on the finger.

The sealing water hoses for the pit pump had been disconnected and were being stowed on the pump sled by 2 members of the float crew. As the flanged end of the hose was being rolled up on the sled the employees gloved left hand was caught between the flange and the pump gearbox, causing an avulsed area on the left index finger.

Right hand caught between end of pipe wrench and helpers stand. Resulted in broken index finger & laceration of index, middle and ring fingers - 9 stitches total.

Employee A and B were working on water line tank. Employee B stopped swinging the hammer to let Employee A use the impact to loosen the bolt. Employee A reached for the spinning nut and Employee B swung the hammer at the same time striking Employee A.

In the process of bolting pipe together the Employee wiped the flange off with his hand a metal splinter stuck into his right ring finger. A local anesthetic was administered and the fragment was removed. The employee was given a written prescription for antibiotics and sent for an x-ray at a local hospital to ensure all metal from the splinter had been removed.

Employee was sliding a fork on the forklift and pinched his right thumb between the cage and the fork.

Employees were tasked with changing out the paddles on the C Log washer. Employee was attempting to step over a spray water brace on top of the log when he stumbled. He put his left arm against the guard on the side of the log washer to catch his fall. Employee then reported that he felt like his shoulder was dislocated and he was in pain.

Employee was changing out a ballast in wall mount light turned to set it down and felt a pinch in his stomach. He though it was the harness pinching him, and keep right on working.

While the 852 drag line was walking, an operator was walking in front of the machine to ensure power cables were clear of the tub. Due to recent rainfall, the area was saturated and muddy. As the operator was walking thru the mud, he felt a pain in his lower back as he pulled his foot out of deep mud.

Technician was exiting a dragline via stairs and sprained his right ankle as he stepped into a depression on the ground caused by the dragline shoe

Lifetime of heavy labor resulting in a hernia.

employee cleaning tools & materials off bed of maint truck. Emp bent over to pick up choker off ground, had hand resting on edge of storage box and lid fell on his left hand, causing a fracture of the 3rd metacarpal of left hand.

Employee was installing a section of 20 inch steel pipe to a 90 degree fitting. Two employees utilized hand tools to attempt to free a frozen nut. Employees hand was rotated toward the pipe causing his right ring finger to be caught in between the open end wrench and the pipe. Employee received a laceration requiring sutures to correct.

While Mechanic's Helper was swinging sledge hammer, he missed intended target; causing a metal fragment to chip off.

while removing bolts from the belly pan it fell; causing blunt trauma to pinky and ring fingers.

Employee was exiting the float truck bed area, utilizing manufactured steps for egress. Instead of stepping straight down, the employee stepped to the left corner of the bumper step. The employee lost his balance striking his left shin on the left corner of the step bumper, resulting in a laceration that required stitches to correct.

WHILE BACKING UP; OPERATOR HIT ROUGH AREA CAUSING HIM TO JOLT STRAINING HIS BACK.

employee was injured replacing the liner in a small trash can in the Electrical Dept's lunchroom. As he was tearing a new bag from the roll, his right index finger contacted a large piece of glass sitting on an adjacent desktop. The employee was transported to our clinic where 8 stitches were provided to treat the laceration

Small space that mechanic was working in limited visibility and was only able to rely on touch/feel.

WHILE HAULING FILL; OPERATOR HIT HOLE IN ROAD CAUSING SHARP PAIN IN HIS BACK.

While hauling fill; IW hit rock on haul road causing pan to jolt.

employee working was lubricating a 3/4 inch air ratchet with oil. While clearing excess oil from ratchet, oil mist sprayed into eyes. Employee treated and evaluated with chemical conjunctivitis in both eyes. He was prescribed anti-inflammatory and antibiotic medications and released to return to work with no restrictions.

While dismounting machine, IW stepped on uneven ground conditions, causing foot to slip.

MS EMPLOYEES WERE MOVING CRIBBING AT THE CF HARDEE BUCKET YARD. EE'S WERE WORKING TOGETHER, STACKING CRIBBING ON A FORKLIFT. WHILE STACKING THE CRIBBING, INJURED EE GOT THE PINKY AND RING FINGER ON HIS LEFT HAND CAUGHT IN A PINCH POINT.

EE WAS USING A 4' PRY BAR TO SLIDE A PIECE OF DECK PLATE THAT HE WAS WORKING ON IN THE CF MAINTENANCE SHOP. THE PRY BAR SLIPPED AND THEN SMASHED HIS LEFT INDEX FINGER (THROUGH HIS GLOVE) BETWEEN THE PRY BAR AND A PIECE OF ANGLE.

Miner was hammering a keeper pin into place on a dragline bucket tooth when he missed the pin, striking the tooth itself with the hammer. A sliver of metal chipped off of the tooth and struck the miner between the first and second finger of his left hand. It required three stitches to close the laceration and the employee was released back to work with no restrictions.

While trying to loosen bolts on plate; IW felt sharp pain in right shoulder

While operating pan on dusty haul road; dust entered IW eye.

EE was attempting to insert a spike puller to remove a spike, when the action of hitting said spike puller dislodged a piece of an old weld that had adhered itself to the spike. When this piece became dislodged, it flew up & hit him in the neck causing a laceration that required two (2) stitches to close. EE was transported to ER for treatment. Released, no duty restrictions.

Mechanic was using a lathe to fab part for a track torch. Metal shavings spun around & caught his left index finger, causing a laceration. He was treated by the facility Nurse & sent to local clinic where he received 11 sutures and was returned to work with restrictions.

A mechanic was using a cutting torch to split a 20 pipe for a wear band. He cut thru the pipe & stored energy in the pipe caused it to spring open striking his left ring finger was struck by pipe. He was evaluated by facility Nurse & sent to local clinic for x-ray, which confirmed a fracture. His finger was set & splinted. He was released to return to work with restrictions.

While attempting to turn disconnect switch off; plastic casing came off release handle causing hand to slip.

A Maintenance employee was working in the Washer structure. Employee was standing on the ground directing a crane into place when he was struck in the mouth by a falling piece of rock. The employee was taken to Occupational Health Partners where he received 13 sutures. The employee was released to return to work with no restrictions.

EE was dismounting machine when his foot slipped causing his leg to hit track pad growser.

While attempting to check final drive on D8 Dozer #1702; IW was using a ratchet to pull up causing pain to in middle of back.

While using chipper hammer to clean rails off; hammer hit metal causing a piece of metal to come off corner of hammer and lodging into middle finger of right hand.

Employee was stepping over a matrix pipe line carrying a small aluminum ladder. Employee stated that he felt a pain in his lower back. A subsequent examination and evaluation by a physcian resulted in the diagnosis of a muscle strain. Employee was allowed to return to work on modified duty with work limitations pending full recovery.

A contractor employee was in the process of removing electrical zip ties from bundled electrical cords using a pocket knife. The knife slipped and the employee was cut on his left thumb through his cotton work glove. The laceration required three sutures to close. The miner was released back to work with no restrictions.

Miner was tightening bolts on a piece of equipment when a bolt snapped. The sudden decrease in resistance caused the Miner's hand to fly back, mashing his right ring finger between the wrench handle and a nearby pipe. This resulted in the Miner receiving 5 sutures on the affected finger.

Employee was engaged in cutting, grinding, and welding activities when he noted discomfort in his right eye. A subsequent examination later the same day by an Optometric Physician revealed the presence of a paint chip. It was removed and the employee was allowed to return to work with no restrictions.

Operator hit bump in haul road causing him to lose control of truck. Dump truck came to rest in ditch after hitting guard rail.

Employee did not have proper foot placing and twisted left ankle. In an attempt to break fall, employee injured left hand. Employee suffered sprain/strain to ankle and two fingers. No fractures.

Employee was driving a vehicle alongside of a materials pipe line, when a stud on the right front tire struck a pipe flange, causing the wheel to suddenly jerk. This in turn caused the steering wheel of the vehicle to unexpectedly spin, yanking the emploee's left thumb downward and causing a "Bennett Fractue".

Employee was attempting to empty a trash drum when he heard a popping sound in his right elbow. The initial doctor evaluation reccomended immobalization of arm until further testing. A subsequent examination by orthopedic specialist revealed a damaged right bicep muscle.

Laborer was moving tools & reports tweaking back muscle while pulling on rope. Laborer was immediately removed from jobsite to office to report injury and taken for medical evaluation. Local MSHA office and mine company personnel notified immediately.

Employee was operating an open cab tractor when he was struck on the right temple by a piece of PVC pipe that was lying in the back of a parked truck. Employee was briefly knocked unconscious and was taken to the hospital for treatment and evaluation. Employee was released and allowed to return to work the evening of the incident.

Employee was driving wooden wedges under the tub of a 1370 dragline to tighten the cribbing under the tub. On his backswing, he hit some adjacent cribbing, altering the path of the sledge hammer and hit his left foot.

An employee was removing lube piping while another employee about 10 feet behind him was operating a grinder. A piece of metal from the grinding hit the inside of his safety glasses and bounced into his eye.

Employee was lining up a trunion pin on a dragline bucket and his right pinky finger was caught in a pinch point.

While installing a mechanical gearbox in the Benification Plant, employee's left index finger was pinched between the gearbox and an adjacent handrail.

While replacing rubber coated material screens, employee struck his left index finger. The subsequent x-ray revealed a fracture.

Employee was working on the back end of a moving trailer when he lost his footing and fell to the ground. A Straw Blower machine attached to the back of the trailer ran over the employee's pelvic area.

Employee was climbing on to an excavator when his foot slipped off the track. He used his right hand to catch him self and this is when his right hand struck the track and caused a small laceration to his index finger. He recieved two stiches in his right index finger and no medications. Returned to work the same day.

While cutting and placing rubber materials into an inlet chute employee was holding a rubber cutting knife when he accidentally dragged the knife across his left thumb.

While disconnecting water hose, it slipped hitting operator.

Employee was washing down the ground floor of the washer when he slipped and fell to the concrete floor.

EE was picking up power cable to move and slide over pipe when cable slipped pinning right hand between the pipe and hot stick. Contusion to top of r/hand. Treated & released to full duty w/MMI @ 0% PPI & dscharged from care. No meds prescribed. One time visit, first aid status. Conduct a JSA and retrain for the task as necessary to prevent reoccurrence.

Operator slipped, dirt on steps.

Employee was attempting to unchoke an overflow line with a small piece of pipe when the obstruction broke free which caused his left hand to strike the overflow box.

Employee was using a 3" hole saw bit on a 1/2" variable speed drill when the pilot broke causing the hole saw to bite into the metal and rip the drill out of his right hand.

Machine turned over landing on worker.

While removing a 20" blind from between two flanges on a pipeline, the bucket on the PC 400 excavator struck employee on his left middle finger and smashed it between the bucket and the pipe.

Employee was cleaning out a pickup truck when he struck his elbow on the bed toolbox.

Employee had opened several valves during work tasks on 8-8-06. On 8-9-06 his left shoulder was hurting. Employeee did not report a work related injury until 10-3-06. On 10-25-06 a Doctor stated his injured shoulder may may be work related.

Employee was working on a sand/clay pump when his breathing became labored and his head began to throb. Employee was transported to hospital via EMS.

Employee was installing a 1/4 inch steel plate into a log discharge chute when he felt a sharp pain in his lower back.

Employee was attempting to un-choke a secondary log using a piece of galvanized pipe when he struck his right middle finger on the edge of the overflow chute.

Employee was removing a wedged shackle from a fairlead when the shackle loosened which allowed the fairlead cable to strike him.

Was crossing over a pipe on the West Side Float Crew, with bolts in both hands, and did not see hole, similar to pothole on other side. As he crossed over pipe, right foot went down into pothole, causing him to lose his balance. As he tried to regain balance to avoid falling, twisted right ankle.

OPERATING SCRAPER, HAULING FILL. HIT HOLE IN THE ROUGH TERRAIN (STRAINED HIS BACK).

Employee was placing a trommel screen into a lifting box when the screen dropped and smashed his finger between two screens.

SLIPPED ON WET TRACKS

Employee was working on a flange when his right ring finger was smashed between the flange and work table.

Employee was operating a small boat when it became stuck in the mud. While trying to free the boat by pushing with a plow he strained his right shoulder.

Strained right ankle. Contributing factor uneven ground twisted ankle!

Employee was climbing onto a bulldozer when he reached for the hand grab and missed it which caused him to fall backwards to the ground.

WINDY CONDITIONS, NOT WEARING SAFETY GLASSES, CONTRIBUTED TO DEBRIS ENTERING EYE.

At time of accident, there was neither unsafe acts or conditions contributing to this injury. (MOUNTING DOZER, FELL AGAINST TRACKS & LACERATED SCALP.)

No mechanical hazards or violations of safety rules were noted as reason for accident. Weather conditions were not a factor. (He lacerated his ring finger dismounting equipment)

SPROCKET SEGMENTS WERE WORN WHICH CAUSES SHARP EDGES.

Employee was stranding in the back of a pickup truck retrieving a flashlight from a tool box when he tripped on materials stored in the truck bed and fell out of the truck.

Employee stepped into a puddle of water that was actually a hole that had been washed out by a leaking underground pipe.

Employee was using an oxy-acetylene torch when the head of the lit torch came in close proximity of his left forearem and burned it.

Employee had attached a chain cumalong to the a section of grating and was applying upward pressure. He had also placed a 30" spud bar into the grating with the intention of applying leverage to the low spot. The weld and clip holding the grating broke loose causing the spud bar to be thrown upward and strike the employee on the nose.

EMPLOYEE STEPPED DOWN FROM COMPACTOR TURNING HIS LEFT LEG CAUSING MUSCLE STRAIN. WET CONDITIONS WERE A CONTRIBUTORY FACTOR.

Employee was operating a Genie Z-80 Manlift on the North side of the washer. While attempting to scope in the basket, a pin retainer made contact with a section of conduit and caused the manbasket boom to become jammed. Upon trying to unjam the boom, the boom suddenly thrusted upward and caused the employees head to strike a pipe.

Installing muffler on equipment (pan) lacerated knuckle on right hand.

WHEN STOPPING A 120 HIGHREACH IT JERKED UPON STOPPING AND CAUSED AN EE TO STRIKE THE EDGE OF THE BASKET AND INJUR HIS BACK.

Employee was working on the 1370 dragline. He was stepping down off bottom step of the drag trough crossover stairs when his left foot slipped on the steel plate floor which caused his weight to shift and twist his right knee.

WHILE WORKING ON D-8 MECHANIC PUT HIS RIGHT KNEE DOWN ON A BOLT THAT WAS ON THE GROUND.

EMPLOYEE STEPPED DOWN OFF A L320 LOADER AND FELT AN UNUSUAL SENSATION IN HIS BACK.

EE WAS PULLING MAST PINS ON THE 862 DRAGLINE, WHEN THE FIRST PIN VAME OUT, THE LINK THAT IT WAS IN SHIFTED AND PUSHED THE PIN TO THE SIDE TRAPPING HIS HAND BETWEEN THE PIN AND A HANDRAIL .

EE WAS USING AN AIR ARCH TORCH WHEN A FOREIGN MATERIAL ENTERED HIS LEFT EYE.

EE WAS DESCENDING STEPS WHEN SHE SLIPPED AND FELL TO A LOWER LEVEL.

EMPLOYEE WAS FUELING MOBILE EQUIPMENT WHEN A FOREIGN OBJECT ENTERED HIS RIGHT EYE.

EE WAS USING A SHOVEL WHICH AGGRAVATED AN EXISTING BACK CONDITION.

EE WAS WALKING UNDER A SULFURIC ACID PIPE WHEN SEVERAL DROPS OF ACID FELL ON HIS ARM AND SHOULDER. THE ACID DRAIN LINE WAS CLOGGED AND DIDN'T ALLOW ACID TO DRAIN FROM LINE AS NEEDED.

DRIVING D400 DUMP TRUCK - HIT HOLE IN HAUL ROAD - GOING TOO FAST.

EE WAS WALKING UNDER AN OAK TREE WHEN HE STUMBLED AND LOST HIS BALANCE. AS HE TRIED TO REGAIN HIS BALANCE HIS LEFT FOOT TURNED SIDEWAYS AS IT CONTACTED THE GROUND.

DOZER OPERATOR CAME UP FROM BEHIND EE ON PAN, HIT HIM TOO HARD JARRING HIS BACK AND NECK.

EE WAS BENDING OVER TO PICK UP MATERIALS WHEN HE FELT A STRIN IN HIS LOWER BACK.

EE WAS FABRICATING AN ALUMINUM SLIDE GATE BOX FOR THE PEBBLE BIN. WHEN EE ATTEMPTED TO REPOSITION THE BOX, WHICH WAS RESTING ON METAL SAW HORSES, THE BOX SLIPPED AND PINCHED HIS FINGER BETWEEN THE BOX AND SAW HORSE.

EMPLOYEE WAS LOOSENING BOLT, WRENCH SLIPPED, HIS HAND HIT METAL SPUR CAUSING SMALL LACERATION ON RIGHT HAND.

WHILE BEING LOADED PAN PULLED AWAY FROM DOZER, WHEN DOZER CAUGHT UP WITH PAN, HE HIT THE PAN TOO HARD.

MECHANIC WAS USING RATCHET WITH A PIPE TO LOOSEN MOTOR MOUNT BOLTS. SOCKET SLIPPED OFF BOLT CAUSING MECHANIC TO CUT HIS FINGER.

EMPLOYEE WAS PULLING ON A WASHDOWN WATERHOSE WHEN HIS FOOT CAUGHT ON A SECTION OF GRATING. WHENN HE TRIED TO RECOVER HIS BALANCE HE SLIPPED ON THE WET FLOOR AND HIS LEFT KNEE INJURED WHEN HE F ELL.

EE WAS BOLTING PIPE TOGETHER WHEN THE TIP OF HIS FINGER WAS SMASHED BETWEEN THE HEAD OF A BOLT AND THE PIPE.

EE WAS SETTING UP A JOB WHEN HIS WELDING STINGER WAS DRAGGED ACROSS THE EQUIPMENT AND ARCHED. HIS LEFT HAND WAS CLOSE TO THE ARCHING AREA OF THE STINGER.

EE WAS DRILLING OUT PACKING GLAND STUD, HOLES WITH A 90 DEGREE DRILL WHICH CAUSED HIS RIGHT HAND TO BE UNDER PRESSURE AND IN AN AWKWARD POSITION FOR APP 25 MINUTES. THIS TASK CAUSED NUMBNESS I N HIS RIGHT HAND AND FINGER.

WHILE USING CUTTING TORCHES TO REMOVE CUTTING EDGE BOLTS ON PAN, SPARK WENT IN EAR.

EMPLOYEE WAS HOLDING A MOTOR BASE BOLT WITH HIS HAND AS THE MOTOR WAS BEING LOWERED INTO PLACE, THE EMPLOYEE'S HAND WAS CAUGHT BETWEEN THE BOLT AND THE MOTOR BASE. EMPLOYEE'S HAND WAS PLACED I N A PINCH POINT.

WHILE IN THE PROCESS OF ROLLING PIPE EE FELT A PAIN IN HIS BACK WHICH CAUSED HIM TO FALL TO HISLEFT KNEE AND BRUISE IT.

EMPLOYEE WAS ADJUSTING A VALVE POSITIONER WHEN THE ACTUATER ARM MOVED AND PINNED HIS HAND AGAINST THE PLATE.

EE WAS HELPING TO REMOVE BLADE FROM TRACTOR-HIT SHIN ON BLADE SCRAPING SKIN OFF SHIN TO FOOT-POSSIBLE HAIRLINE FRACTURE ON RIGHT FOOT BONE.

EE STEPPED OFF TAILGATE OF TRUCK.

BACKED INTO DRAINAGE DITCH.

EE WAS WALKING IN AREA OF THICK BRUSH WHEN HE HYPEREXTENDED HIS RIGHT KNEE.

EE WAS MOVING A 4-0 POWER CABLE BY HAND WHEN HE FELT A PAIN IN HIS LOWER BACK.

EE WAS CHANGING OUT A PUMP SHELL AT #2 MATRIX BOOSTER PUMP WHEN HE SLIPPED ON THE PUMP SHELL AND FELL. AT THAT TIME HE ONLY EXPERIENCED MINOR PAIN IN HIS LOWER BACK AND REFUSED MEDICAL TREATME NT. ON 9/30/02 HE REPORTED TO THE NURSE COMPLAINING OF SEVERE LOWER BACK PAIN AND WAS REFERED TO A DOCTOR.

CLIMBING OFF BACK HOE (MACHINE) OIL ON TRACK, HE SLIPPED, CAUGHT WITH LEFT HAND AND HELD ON, PULLING MUSCLES IN SHOULDER.

EE WAS ACTING AS A FIRE WATCH WHEN A BALL OF SLAG FELL, HIT A BEAM, BOUNCED 15' & FELL DOWN THE SHIRT COLLAR & BURNED HIS UPPER BACK.

EMPLOYEE WAS WALKING ACROSS LOG TEETH TO ACCESS A COME-ALONG. HIS FOOT SLIPPED OFF A TOOTH & HE FELL INTO A GUARD.

EMPLOYEE WAS MOUNTING DUMP TRUCK; HAND SLIPPED ON HANDRAIL. PULLED MUSCLES IN CHEST AND LEFT SHOULDER.

DISMOUNTING MACHINE, TURNED WHILE ON THE STEP, SLIPPED INJURING ANKLE.

TAKING BELLY PAN OFF SCRAPER, WHEN HE TOOK LAST BLT OUT, BELLY PN FELL ON HIS RIGHT ARM.

WHILE DISMOUNTING MACHINE, GRABBED HANDRAIL, HANDRAIL BROKE.

EE WAS RESOCKETING A DRAG CABLE WHEN THE END OF THE CABLE WHIPPED OVER AND STRUCK HIM ON THE RT.FOOT.

OPERATOR WAS DISMOUNTING MACHINE, CABLE ON STEP BROKE.

GOING TOO FAST ON HAUL ROAD, HIT BUMP OR HOLE, JARRED NECK. CERVICAL STRAIN.

EE WAS WORKING ON AN AUTOMATIC LUBE SYSTEM ON THE 1370 DRAGLINE. WHEN A LUBE FINE RUPTURED AND ALLOWED HIGH PRESSURE LUBRICANT TO STRIKE EE IN THE CHEST.

HYDRAULIC OIL SPRAYED ON HIM WHEN HOSE BURST. HE WASHED IT OFF WITH MECHANIC SOAP. THIS CAUSED AN ALLERGIC REACTION.

EE WAS BACKING A MANLIFT WHEN THE LEVEL INDICATOR CAUSED THE LIFT TO ABRUPTLY STOP. HE WAS CARRYING A 1/2 X 4 X 7' PIECE OF ANGLE IRON ON TOP OFTHE BASKET WHICH STRUCK HIM IN THE CHEST AREA WH EN THE MANLIFT STOPPED.

EMPLOYEE WAS STEPPING INTO A JON BOAT WHEN HIS RIGHT FOOT BECAME STUCK IN WET CLAY, AS HE PULLED HIS LEG HE FELT A POP IN HIS RIGHT KNEE.

EMPLOYEE WAS REMOVING BOLTS WITH A 1/2 INCH IMPACT WRENCH WHEN THE SOCKET FLEW OFF AND STRUCK HIM ON THE BACK OF THE RIGHT HAND.

EE WAS SECURING A MOTOR WITH A COME-A-LONG WHEN THE CHAIN SLIPPED AND CAUSED HIS RIGHT ELBOW TO STRIKE THE TAILGATE OF THE PICKUP TRUCK.

EE WAS AIR GOUGING ON A DRAGLINE BUCKET WHEN A SMALL PIECE OF SLAG FELL INTO HIS WORK GLOVE AND BURNED THE MIDDLE FINGER ON HIS LEFT HAND. THIS ACCIDENT REPORTED UNTIL 6-19-01 AFTER THE BURN B ECAME INFECTED.

EE REPORTED TO THE NURSE ON 5/15 WITH WHAT APPEARED TO BE A SLAG BURN ON HIS RIGHT WRIST. HE STATED THAT HE WAS WORKING IN THE MAINTENANCE SHOP ON 5/11 AND WASN'T SURE WHEN IT HAPPENED. THE BU RN BECAME INFECTED AND NEEDED MEDICAL ATTENTION.

WHILE TURNING WRENCH TO LOOSEN OIL FILTER, HE FELT PAIN IN BACK. PULLED MUSCLE.

EE WAS PLACING A PIECE OF CRIBBING UNDER AN OUTRIGGER WHEN HE SMASHED HIS RIGHT RING FINGER BETWEEN THE CRIBBING AND THE OUTRIGGER.

EE STANDING ON PUMP BEARING ASSEMBLY WHEN HIS FOOTING SLIPPED AND CAUSED HIM TO FALL INTO A PUMP SKILL DISCHARGE FLANGE.

DURING THE DAY EE WAS USING A HAND HELD PORTABLE GRINDER IN A CONFINED SPACE. SOMETIME DURING THE DAY A PIECE OF METAL ENTERED HIS EYE BUT IT DID NOT EFFECT HIM UNTIL HE GOT HOME THAT NIGHT. E E WAS WEARING A GRINDING SHIELD AND SAFETY GLASSES DURING THE GRINDING TASKS.

EE WAS IN THE PROCESS OF CONNECTING TWO 100' JOINTS OF 24" PIPE. ONE JOINT WAS LYING ON THE GROUND WITH ONE END CRIBBED WITH A WOODEN BLOCK. THE OTHER JOINT WAS BEING MOVED INTO PLACE WITH A L OADER WHEN IT STRUCK THE FLANGE OF THE ONE ON THE GROUND. THIS CAUSED IT TO SLIDE ONTO THE EE'S FOOT.

EE WAS REMOVING ROOTS FROM AN AREA THAT HAD ALREADY BEEN CLEARED HE BROKE THE ROOTS LOOSE & WAS IN PROCESS OF PICKING UP THE ROOT BALL, WHEN THEFRT END LOADER BUCKET HIT THE ROOT BALL STOPPING THE LOADER SUDDENLY EE CLAIMS HE WAS WEARING SEAT BELTS BUT THE SUDDEN STOP CAUSED HIM TO HITTHE STEERING WHEEL

EE WAS STRIPPING THE INSULATION OFF A PIECE OF ELECTRICAL WIRE WHEN HIS KNIFE SLIPPED AND LACERATED HIS LEFT THUMB.

EMPLOYEE WAS CLOSING A VALVE AT THE SAND/CLAY MIX STATION WHEN SAND BLEW INTO HIS LEFT EYE. A GRAIN OF SAND EMBEDDED INTO HIS EYE AND REMOVED BY DOCTOR. EMPLOYEE LATER GIVEN AN ANTIBIOTIC TO P REVENT INFECTION OF THE EYE.

EMPLOYEE WAS PLACING BOLTS INTO A PIPE FLANGE. WHEN HE HIT ONE OF HTE BOLTS WITH HIS HAND HIS RIGHT HAND HIT THE PIPE FLANGE AND KNOCKED OFF THE FINGRNAIL ON HIS PINKY FINGER.

EMPLOYEE WAS CONNECTING TWO 20" FLEXIBLE PIPES WHEN THE ONE BEING GUIDED BY A SYNTHETIC SLING SLIPPED ON THE SLING AND STRUCK HIS RIGHT KNEE.

EMPLOYEE WAS USING A WEED EATER WHEN HE CONTACTED POISON IVY VINE.

THE GRATING LOCK DOWN PINS CAME LOOSE. AS THE EE STEPPED ON THE GRATING, IT GAVE WAY. HE FELL TO NEXT LEVEL, APPROX. 10-12 FT.

EE WAS SECURING A DREDGE ANCHOR ROPE WHEN IT SLIPPED AND PULLED HIS HAND INTO A PINCH POINT. THIS CAUSED A LACERATION TO HIS LEFT INDEX FINGER.

EMPLOYEE WAS PASSING THROUGH A SWINGING PLATFORM GATE WHEN THE WIND BLEW THE GATE DOWN ON HIS HEAD.

EE WAS WALKING ACROSS REBAR MAT WHEN HE SLIPPED AND STEPPED THROUGH TOP MAT SCRATCHING HIS SHIN WHICH THEN BECAME INFECTED.

EE WAS DRIVING A TRUCK AND BACKING UP, WHEN ANOTHER VEHICLE HIT HIM IN BACK TAILGATE.

EE WAS USING HIS POCKET KNIFE TO LOOSEN A SCREW WHEN THE BLADE BROKE AND BUT HIS FINGER. THE BLADE WAS CLOSED AT THE TIME IT BRAKE.

WATER TRUCK HAD JUST MADE A PASS AND THE DIKE WAS WET. THE DUMP TRUCK DRIVER DID NOT ADJUST SPEED AT CURVE AND THE TRUCK SLID OVER THE DIKE AND LANDED ON ITS SIDE.
