Working on loader felt wrist pain down to elbow taken to medical facility released to full duty had follow up on 8/21 also had leg pain got chiropractic treatment with restrictions could not return to work

A miner was working in the Longwall Tailgate, removing Backflow ventilation structures. As EE was cutting the ties holding the top rails, EE reached up to cut off the brace and when EE cut the tie, the rail had tension on it, causing it to spring back toward the miner, striking EE in the lip, resulting in EE's teeth being pushed through EE's lip.

While using a utility knife to remove a hose from an air pump, enough force was applied by the injured employee, to break the blade of the utility knife. A piece of the broken blade remained in the hose and the force being applied also caused the arm to strike against the broken blade after the blade broke. This resulted in a laceration requiring 3 stitches.

During repairs on the longwall a mechanic felt a pop in back as EE pushed on a 30 MM end wrench. EE returned to duty. On 3/27/2017 this mechanic sought chiropractic care, identified as medical treatment by the Yellow Jacket and converting it to a medical treatment case on that day.

While preparing to install a new belt splice, the bottom belt moved unexpectedly when a pinch clamp failed, causing the belt to toss the cutting tool up, striking the employee in the mouth, breaking a tooth.

On March 7 this employee was removing a staple from a hydraulic fitting, using a pry bar. The bar slipped, causing the employee's arm to swing across chest. EE felt a pop in shoulder. No medical treatment was provided until June 28, at which time EE received shoulder surgery.

Employee was struck near the left eye (under his safety glasses) with a high pressure plug from the main manifold block after removing the retaining staple from the plug. Employee received 6 stitches under his left eye.

Employee was using a 30# sledge hammer to dislodge the rock teeth from a loader bucket. A piece of metal chipped off the tooth and struck the employee in the arm. The metal was removed and the wound closed with 2 stitches.

Employee was cutting clips from a Kennedy stopping. The flag had tension and when he cut the clip, the flag came loose and struck him on his lip, causing lip laceration, contusion and chipped teeth.

While employee was bent over to disassemble T valve on waterline hose, the valve was stuck so employee pry it apart with hand tools. When employee stood up he felt sharp pain in his lower back. Reliance Medical Group released him to restricted duty and no underground work

Employee smashed finger tip when chain hoist slid on truss leg. This caused slack in the chain pinching finger between chain and driscol pipe. employee received 3 sutures and released to full duty.

EE became fatigued using his right hand to cut strips in lagging on pulley. Switched to his left hand and shortly after slipped and poked himself in the left thigh just above the knee. He sustained a 3/8" cut that was managed with first aid. After his shift he went to the dr for a Tetanus shot and was convinced that he need stitches to prevent infection. Reportable injury.

Employee swung a 2# sledge hammer to remove the bolt and the hammer ricocheted off the cylinder striking employee on the right knee cap, resulted to contusion and released to light duty.

Employee was prying up a tail gate on a pick-up truck with a screw driver. The screw driver slipped out of his hand and hit employee in the mouth chipping a lower tooth. Employee continue to work and did not report the event until the end of shift the following day (4-6-11).

While knocking steering bushing out of cylinder, employee struck hammer against punch. A piece of metal broke off punch and lodged into employee's arm. Employee was transported to emergency room and received suture and released full duty.

While troubleshooting a hydraulic, employee was trying to loosen a fitting by pushing down on the wrench. When the fitting broke loose, he hit his chin on a piece of belting that was covering the power pack which chipped his tooth. Employee was released full duty.

employee's left hand was lacerated against a metal plate guard after his hand slipped free of the wrench used to break some fittings loose on hydraulic control valve. Employee was transported to Reliance and received sutures.

while moving water trap into position to hook up. employee pinched left thumb between prybar and water trap. causing to fracture thumb.

Employee was using 90lb rock hammer to break up rock in the headgate. The hammer slipped out of the employee's hands striking another employee causing injury to the second employee'e hand.

Using a pry bar to bend chock block holder back in to place when he felt pain in the groin area.

While employee was hitting the bolt with a hammer the bolt came loose striking him in the mouth. He didn't think he needed to seek medical attention until 7-31-07 & was referred to Periodontics. Periodontist removed his front loose tooth on 8-14-07. Required a partial denture implant.

Emp was holding a straight edge to cut a template out of cardboard, while using a utility knife to cut the template, employee's hand was placed along the edge and while cutting along the straight edge the little finger of his left hand was cut. Emp received 6 stitches to close wound.

Employee reported pain in his wrist and up into his forearm. Employee has been swinging a sledge hammer extensively for several shift while working on shield repair. He was examined at Mesa Occupational & Sports Clinic and as been placed on restricted duty.

Employee was breaking loose a high pressure hose with a pipe wrench and hammer. When the fitting of the hose broke loose, the fitting struck employee's hand resulting in a laceration. Employee received four sutures at Mesa Clinic.

The employee, a tire repair person, was assisting another employee replace a tire when a pry bar he was using to hold a lug nut slipped and pinched his left middle finger. Injury required 5 stitches.

Emp states that he was removing a 1/4" plastic tie wrap from a high PSI hydraulic hose using a 3" razor knife when he cut his left thumb. Emp received 7 stitches.

2/8/04 EE REPORTED HIS INJURY. WAS USING A RATCHET WRENCH & EXPERIENCE A POPPING SENSATION IN HIS SHOULDER. PAIN PERSISTED & SOUGHT MEDICAL ATTENTION & A STRAINED TYPE INJURY WAS DIAGNOSED & RELEASED FULL DUTY. WITH THE LONG TERM CONDITION BECAME REPORTABLE WHEN SURGERY WAS PROGRESS ON 3/16/05

A PIECE OF METAL CAME OFF THE WRENCH AND STRUCK EE ON THE LOWER LIP, WHILE POUNDING WITH A HAMMER ON THE WRENCH BIT.

WHILE EE WAS MAKING A TEST LIFT ON THE JACK LEG DRILL, THE HAMMER SUNG AND PINCHED EE'S FINGER CAUSING AN AVULSION ON HIS SECOND RIGHT KNUCKLE.

WAS HOLDING BOLT CUTTERS AGAINST HIS CHEST TO GET LEVERAGE TO CUT A MESSENGER CABLE. CUTTERS FLIPPED BACK & HIT HIM IN THE CHEST.

EMPLOYEE RECEIVED A LACERATION TO HIS RIGHT HAND WHILE TIGHTENING LUG NUTS ON THE SHIELD HAULER WITH A WRENCH AND CHEATER BAR. THE CHEATER BAR FLEXED BACK AND PINCHED EMPLOYEE'S HAND BETWEEN THE FRAME OF THE SCOOP AND THE BAR, WHILE BEING BY TWO OTHER EMPLOYEES.

EMPLOYEE WAS KNOCKING OUT THE PUNCHOUT HOLE ON THE BREAKER PANEL, WHEN A CLOUD OF DUST CAME OUT AND GOT SOME MATERIAL IN HIS EYE.

EE WAS HANGIUNG A NEW ROLL OF LEAKY FEEDER CABLE FROM THE RIB MESH USING A BAR THRU CABLE ROLL. THE ROLL SLIPPED CAUSING THE ROLL AND BAR TO STRIKE HIM IN THE LEFT HAND AND WRIST.

EE WAS REMOVING RIGHT SPINDLE RETAINER PLATE, PLATE SPUN, STRIKING ON LEFT INDEX FINGER. PRY BAR STRUCK FINGER CAUSING THE LACERATION RESULTING INTHREE STITCHES.

EE WAS IN THE PROCESS OF REPAIRING THE HYTORQUE WRENCH ON THE BOLT, THE WRENCH TWISTED PINCHING THE PALM OF THE LEFT HAND CAUSING A LACERATION, REQUIRING TWO STITCHES.

EMPLOYEE CUT LEFT THUMB WITH UTILITY KNIFE WHILEREPAIRING LOW VOLTAGE WIRE FEEDER CABLE OF WELDING MACHINE. THE CUT REQUIRED ONE STITCH.
