While doing pre-op grader right rear side, notice the engine compartment door was open, crawled up to close the door, the latch handle was broken, went back to cab to get pliers to close the latch, after closing the latch, crawling back down slipped on the small lower foot step. Reached to grab the hand rail to keep from falling off the grader and somehow cut left hand.

Reported neck was hurting from loader hitting or rock dropped in bed of truck. Went to Urgent Care for treatment on 10/6, did not notify employer until next day. Employer requested 2nd opinion from Reliance Medical Center. Released to full duty 10/7 with follow up appointment on 10/14, follow up referred chiropractic treatment. Received chiropractic treatment on 10/20.

Was at pond 11 checking water pump fuel level. While walking slid off the berm into the mud. Right foot stuck in the mud about 5" deep and I twisted ankle, pain also felt in the lower knee area.

During the start of shift crew stretching an operator felt a slight pulled back muscle. While heading out to the field the operator felt pain and got worse in route to the work area. After doing their equipment pre-op inspection the operator called the supervisor to inform them of the pain.

Filling transmission fluid on dozer assisting Mechanic. Mechanic started unit to fill filters and check oil level. Filter housing cracked causing oil to spray on oiler on the right side of face and right eye.

Employee operating portable hydraulic jack pinched left pinky finger between the jack handle and the shop floor from not engaging the hydraulics and not maintaining control of employee's balance. The left pinky finger was broken.

Working on loader felt wrist pain down to elbow taken to medical facility released to full duty had follow up on 8/21 also had leg pain got chiropractic treatment with restrictions could not return to work

Employee dismounting boarding ladder on last step knee gave out and fell. Employee did not want medical evaluation. On 11/15/2022 employee, requested medical evaluation. Seen at medical facility, employee was released to full duty. On 1/09/2023 went in to medical medical facility to have knee checked. Employee was given a Do Not Return To work notice and had surgery scheduled.

Disassembling crushers Thursday 11/10/22 at end of shift, employee lower back was hurting. 11/14/23 low back pain was still noticeable hurting. Employee released full duty 11/14/23. Had follow up appt. Did not notify Safety Dept of getting more treatments. Safety Dept notified on 7/13/23 by WC Insurance Co.

Employee stepped off of step stools last/bottom rung rolling EE's left ankle resulting in a sprained ankle resulting in the inability to wear EE's steel toe boots (PPE) to return to EE's normal duties.

Employee was working on replacing a broken fitting on the shear slough plate. When the fitting going to the cylinder was removed, the sloughing plate dropped to the stops on top of the shear and pinned the employee between the shear body and the sloughing plate. Employee quickly got chains connected to sloughing plate to raise slough plate and remove the employee.

Employee was taking a wooden crib block off a pallet, turned around to walk away when the first row of cribs toppled over and hit EE's lower left leg. Employee later went to urgent care on June 4th and was prescribed antibiotics for an infection, on June 6th EE's personal doctor wrote a note to be off work till June 13th.

A mechanic was clearing tools from an area where EE had just completed work on a Letourneau loader. When the mechanic picked up a pump assembly EE felt something protrude through EE's cut resistant gloves, upon removing EE's glove EE saw the cut on EE's left index finger. The wound was cleaned and the employee received stitches.

Employee was packing a turbo drill and felt pain in their shoulder. Note: Employee DID NOT start missing work because of this incident until April 29, 2022.

While pushing the HG drive the roof failed on the LW face from the HG shield 3 & 4 tips to the MG 2 motor and crusher throat to the block. Mining halted, air readings were taken and notification calls were made. Mining resumed, additional support installed.

Employee felt pain in right knee while adjusting an external speaker. Was seen by the medical provider, released to full duty. Had 2 more follow-up visits then referred to an orthopedic doctor where a tear in the meniscus was discovered. Employee had surgery May 16th.

While cutting out the TG during the 5th pass of the day, the roof failed on the LW face, above Shield 175 & 176, resulting in blocking travel off the TG. Mining halted, air readings were taken and notification calls were made, mining resumed under the TG Blockage procedures, with MSHA approval.

LW Shearer was cutting into the TG. Roof failed around 2 roof bolts, adjacent to Shield 176, the last shield on the face. Fall was estimated to be 4 ft. wide, 8 ft. long and 8 ft. high. The entry remained open. The fall is reported because it went above the anchorage of the installed roof supports. Ventilation was unaffected. The fall was immediately reported via the hotline.

A chunk of coal slid down the ranging arm on the shearer, then fell off onto the walkway. Some pieces struck the employee on the top of EE's right foot, resulting in a fractured toe.

Monday - Employee lifted a pin for a hydraulic cylinder from the ground, carried it to the truck, hauled it to the Weld Shop and handled it 2 more times. Tuesday EE started feeling discomfort. Wednesday EE reported the incident and was taken for medical evaluation, where EE received chiropractic care.

Employee was throwing a rock out of the walkway on the longwall face when a rock fell out from between the shields causing injury to employee's foot.

Mechanics were loading a swing cylinder for a motor grader onto a pickup truck. One mechanic stumbled while moving it to the truck, causing the load to shift suddenly and the other end of the cylinder dropped, pinching the other mechanic's finger between the end of the cylinder and the truck bed.

Employee was walking along the LW face. EE stepped down from the shield toes, onto the DA Relay bar. EE's foot slipped and EE fell, striking the side of EE's head against the base lift cylinder, causing a laceration to EE's right ear.

Employee developed pain in neck and shoulders after about a week of carrying materials on shoulder. Employee sought medical evaluation on 3/24/2021 and was found to have inflammation of neck and shoulder muscles.

Fletcher operator attempted to pull in to XC-42, between 5 & 6 Entry, the Fletcher contacted the mobile dust pod already in that crosscut and also contacted an employee working on cleaning the Pod. The contacted employee was caught between the two units, resulting in bruising to the back and legs.

The Mine examiner noticed a roof fall had occurred in an entry adjacent to the examiner's normal route. It is unknown when this fall occurred. Intersection in East Mains, Entry #1, at Crosscut 49 had fallen. The entire intersection fell in and is approximately 20 ft Wide and 19 Ft Long. It fell above the 6 ft resin bolts and cable trusses that had been installed.

Employee slipped while dismounting from a light truck. Leg slipped out from under employee, tried to catch self. Strained left leg & left shoulder.

A safety chain had been installed as an anti-topple device for the walking gob-side walking shield. As the shield was being moved, the chain broke and a piece of it struck the EE in the face, causing a laceration.

Was helping attach Huesker mesh to the shield tips, in preparation for LW recovery. As EE stepped across the panline they felt a pinch in lower back. The following day EE was stiff and sore. EE was examined at Medical and received Chirpractic treatment from their own chiropractor.

EE slipped and fell while walking from the shower to EE's locker, striking head on the floor, resulting in a laceration which required stitches.

Preshift examiner stopped at XC-49 to examine a temporary fuel storage unit. While there they heard the roof working. Examiner moved outby the area and called the Responsible Person to report the need for additional support. While making that call, the roof fell It is located at XC-49 in Entry 2 of GR405. The fall extends into each x-cut and rib to rib in 2 Entry.

While dismounting from a piece of equipment in the shop, a miner twisted a knee. Miner was evaluated by an off-site medical professional and was released to return to duty. An MRI test was conducted later and identified a torn ligament in the knee and required surgery on 3/24/2020 making it a reportable injury.

Employee injured a shoulder while replacing a component on a LeTourneau loader. A MRI was conducted later on identified a torn rotator cuff. Surgical repair completed on March 10, resulting in this being reportable and lost time began. Info entered in MSHA database on 3/11/20. A review of that system indicates the form did not save properly. Re-entered.

On 9/02/19 the employee felt a pop in EE's shoulder while changing a tire on the small water truck. On 9/03/2019 the employee was seen at the medical provider and placed on restricted duty.

Day shift employee while getting tools ready for work picked up the tool bag and felt a pinch in back with a sharp pain. Reported it to supervisor, an EMT checked employee out, and employee was brought outside for further evaluation. Employee was taken to a medical provider where employee received chiropractic treatment and was released to full duty.

While assisting another miner unhook a trailer, the injured miner's foot was caught between the trailer tongue and the hitch on the tow unit. The injured worker is a Dependent Contractor, reporting to the mine operator's supervisor.

Employee injured shoulder while lifting the cover off of the 8MVA PC in LW-404. The employee was examined and released to duty that day. An MRI was conducted later on and indicated surgery would be necessary. That surgery occurred on September 16, 2019. That medical treatment and subsequent lost time made this injury recordable as of that date.

While tensioning truss cables, used in the Minova In-Roof truss system, using the truss gun one of the truss shoes broke free from the roof causing the truss gun, cable and dog bone keepers to fall and strike the employee on the right shoulder. Employee received chiropractic care.

Employee was standing on Shield 176 and was operating the pan push function, using the in-shield controls. EE left hand was resting on the gob wing. As the TG drive pushed forward, ee middle finger on the left hand was pinched by the gob plate as it moved closer to the shields during the push.

An employee came to work on April 8th, stating they had hurt their back on April 2 or 3rd. They had not reported anything to anyone until that time. The employee was seen at Reliance Medical on April 10th. They received chiropractic care and was placed on Modified Duty beginning that day and triggering MSHA reportability.

A miner was working in the Longwall Tailgate, removing Backflow ventilation structures. As EE was cutting the ties holding the top rails, EE reached up to cut off the brace and when EE cut the tie, the rail had tension on it, causing it to spring back toward the miner, striking EE in the lip, resulting in EE's teeth being pushed through EE's lip.

A surface operator reported seeing smoke rising from near the reclamation area and ash dump. Foreman found that some spontaneous combustion of some coal material in the spoil had occurred and sent dozer to extinguish the fire. Fire was not extinguished within 30 minutes so MSHA was called. Fire was extinguished at 08:53.

An employee injured an arm on 9/26/2018. Employee did not receive treatment for this injury at that time. Later on they had an MRI indicating a muscle tear and received surgery on 11/08/2018. The incident became reportable that day.

Roving Security Officer reported flames on the northeast side of the stockpile. No surface production personnel available during graveyard shift. None scheduled to arrive until 6:00 am. Fire was reported out at 6:25 am.

While operating dozer, it hit a rough spot while changing, jolted the operator around causing pain in lower back.

The driver was driving on a raised dirt road when the berm of the road broke and the mixer truck went over the edge. The truck rolled onto its side. The driver was taken to the hospital and received 12 stitches on forehead.

Employee was injured when ee tried to lift a 25 lb block of oak up onto the stageloader. As ee lifted, the block struck against the corner of the SL, causing a shock load to right arm. EE felt a pop and a burning sensation.

While using a utility knife to remove a hose from an air pump, enough force was applied by the injured employee, to break the blade of the utility knife. A piece of the broken blade remained in the hose and the force being applied also caused the arm to strike against the broken blade after the blade broke. This resulted in a laceration requiring 3 stitches.

Employee's back became stiff and sore after several shifts of replacing/repairing overhead lights around the facility. On February 6th the employee's back went into a spasm while ee was at lunch. Inflammation had developed causing low back pain and spasm.
