EE WAS ASSISTING IN PULLING SLACK OUT OF LONG-JOHN BELT AT #7 HEAD DRIVE, A 1/2" ROPE WAS BEING USED, ROPE BROKE, A STRAND OF THE ROPE CAME OFF STRIKING EE IN THE CORNER OF HIS LEFT EYE. EE WA S APPROXIMATELY 15-20 FEET AWAY FROM WHERE ROPE BROKE. EE SUSTAINED A PUNCTURE WOUND TO HIS LEFT EYE. EE WAS WEARING SAFETY GLASSES.
EMPLOYEE STATED DURING THE PROCESS OF THREADING CYLINDER SHAFT BACK INTO CLEVIS ON WEIGH-BIN AT THE LOADOUT, THE 15" CRESCENT WRENCH HE WAS USING SLIPPED OFF CYLINDER SHAFT ALLOWING HIS RIG HT HAND TO STRIKE A PIECE A 2" ANGLE IRON. EE SUSTAINED LACERATION TO BASE OF HIS RIGHT THUMB REQUIRING 6 SUTURES.
