Changing out DA ram cylinder. Very tight fit in relay bar. Laying on stomach with 12 # sledge arms extended forward driving ram to relay bar pocket. Felt a twinge in shoulder. IE stated that he was helping move/install water pipe using a come-along tied to a rope. The rope broke, allowing come-along to fly back and strike employee
He was cutting a flap off a piece of belt when knife blade broke cutting his wrist and received 9 sutures.
Employee was using channel locks to open a metal band to access T3 channel. Metal band while opening struck employee's right arm causing a laceration. Medical Treatment.
