Employee stated that while loosening bolts on spiral pump motor he felt pain in his left shoulder. Employee finished the shift and did not notify his supervisor until two days later.
Williamstown contractor stated that while assisting other miners in placing a locomotive back on the track rails he sprained his left knee while using a lifting jack.

Injured employee was attempting to release a chain hoist after completing a belt splice. The employee left his left forth finger on the chain and as the hoist suddenly released his finger was caught between the chain and the belt resulting in a fracture.
Injured employee sustained a laceration to his left middle finger while assisting in moving the 10 Right section conveyor belt. While attempting to position a bottom roller he placed his hand on a bed rail. A fellow employee was attempting to bend the tab on the bed rail and inadvertently struck the injured's hand with a hammer.
Injured employee sustained a laceration to his left hand while cutting metal bands from a cube of block. Three sutures were required to close the wound.
Injured employee sustained a laceration to his left wrist/hand while operating a roof bolter on 1 North Mains. The employee was inserting a 12' cable bolt into the hole when the plate caught on the bolt. He used the bolt wrench to strike the plate when the plate came loose and moved down the bolt striking his hand. Three sutures were required to close the wound.
EE was trimming a piece of belt off the bottom of a door when he was finishing the cut. The knife came through the belt and into his left arm causing the injury. EE will return to work when the results of a drug screen are available.
Injured employee was prying on a take-up cart to break it loose. After he broke it loose he stated his back was sore. He finished the shift with pain in his back. Employee did not begin to miss work until 12/10/2013.
Injured employee sustained a laceration to his left arm just above the elbow while using a razor knife in an attempt to cut a rope in the mine shop. The laceration required eight sutures to close. The employee finished the shift and did not miss any scheduled work.
Injured employee was using an axe to cut nylon bands on a bundle of cribs on the 17 Left Longwall when he struck himself on the left shin causing a fracture. Employee finished the shift and began missing scheduled days the following shift.
Injured employee was attempting to pull a rib strap out that was hanging loose with a pull rope when he felt pain in his left shoulder.
Injured employee was attempting to turn the trolley pole on #47 jeep while in 87 spur of the 1 South Mains haulage. He was using a handle off a top belt roller. The roller handle was placed through the harp on the pole & a track jack handle was placed through the roller handle. The bar slipped & struck the left side of his face, fracturing his nose.
Injured employee stated that while using a pick to break up compacted rock dust at 27 spur of the East Mains haulage she felt a pain in her left shoulder.
Injured employee was using a razor knife to cut tape off of a broken troley pole when he lacerated his right index finger. The wound required 4 sutures to close.
Injured employee was cutting metal bands on prop setters and was struck on his left eye when the band sprung back resulting in a scratched cornea.

Employee was holding the valve bank in the operator's compartment of a scoop in an attempt to remove a fitting. The wrench he was using slipped off the fitting causing his finger to get caught under the valve bank.
Employee stated that he and two fellow employees were attempting to move the track into position on the 16 lefty section unitizing pro bars. While lifting the bars up to slide the track all of the weight of the track came back onto the injured employees bar resulting in a strain to the lower back.
No equipment running at the time, conditions good/setting spads before mining coal started.
Employee stated he was attempting to align the belt using a pry bar. While prying the belt over, the bar slipped and the injured employee fell back onto the rib and mine floor. He felt pain in his lower back radiating down his leg.
EE stated he was holding a metal tie in position on the 60lbs rail while fellow ee was hammering it to install the lugs onto the rail. The hammering resulted in the rail being knocked from a lifting jack being used to raise the rail. The injured ee left hand was caught between the metal tie and the mine floor, causing a laceration and a fracture to his left hand
Employee was removing the planetary gear from the long wall shearer using a 3/4" drive ratchet. The ratchet slipped off the bolt causing him to strike his right thumb on the plan line resulting in a fracture.
Employee stated he was installing a 4 inch clamp onto a 4 inch nitrogen line using a cheater pipe. The pipe slipped off the clamp striking the employee in the front of the mouth.
EE was raising belt structure at 20 block on 1-South belt. He was using a "bumper" jack. While doing so, the jack slipped out allowing structure to drop. He had his right index finger pinched between a belt chain and belt hanger, while adjusting the slack.
Employee was changing bit inserts in the C/M head using a joy insert utter. He was using a chisel as a shim behind the puller. While doing so the chisel slipped out striking him on the left hand causing a laceration.
Employee was cutting piece of belt with utility knife on old 2 west No.1 belt line, knife slipped and cut thigh above the right knee and lacerated leg 3 to 4 inchs long.
Employee were removing debris from the track. While removing debris, a timber dis-lodged from the mine roof and began to fall. The injured employee grabbed the timber to deflect it and at the same moment another employee tried to use a pick to deflect the timber away. The injured employee was struck with the pick and his outer-most metecarpal was fractured.
EE said he was positioning a chain break on the conveyor chain of company #46 shuttle car to separate the chain to repair it. While placing the chain break on chain, it became wedged on chain. Injured ee placed his hands under it to remove and reposition it. Chain break fell approx 3" & struck hand, resulting in a fracture to the left ring finger.
Injured employee was struck in the mouth by a slate bar being used by the track crew. He was supervising the track crew attempt to roll a rail using the slate bar when the bar slipped from the hands of a co-worker and struck the injured in the mouth, causing a laceration requiring 6 sutures to close.
Employee was punching a hole in panning tin with a rock pick when the head broke off, striking injured employee, causing minor lacerations.
Employee was using a slate bar to pry against stuck belt that was being pulled in. When the belt broke loose, the slate bar slipped and caused his left forearm to be caught between the bed rail and the belt
Employee was making a splice in a conveyor belt. While pulling the belt together with a chain ratchet, the top stucture broke off, causing the handle from the belt stucture to strike the employee on his jaw.
Injured employee stated that he and witness were changing a top roller at 49 block on the 4 Right belt. While the witness was prying the roller up into place the IE was attempting to guide it into position with both hands. The pry bar slipped off the roller and the injured left middle finger was caught between the roller handle and the belt.
The employee stated that he was using a jack and bar to move a frame work when the bar slipped and caused him to fall to the mine floor and injure his right shoulder.
The employee stated he was taking slack out of the tram chain on no.1 mine jeep. He had a lady slipper and was going to knock out a link in the chain when he hit his thumb causing a laceration.
While making connection (pipe) the injured employee had his hand on the tong handle. Another employee shoved on the tong to put it in place. The injured employee's hand was struck between the tong and the drill pipe. His third finger (right hand) was fractured.
While setting the pipe tong, the EE's finger was pinched between the tong handle & tong jaw. Left index finger was injured requiring sutures.
Employee stated that he was holding a piece of phone cable in his hand that was tied around oxygen/acetylene torches and hoses. When he went to cut the cable with a utility knife, the knife slipped lacerating his right forearm.
EE stated while installing 3' resin grouted bolts as belt hangers, the bolt was 1/2 way in the hole, the wrench slipped as EE went to grab the bolt. His left pinky finger was caught between the bolt and wrench when the bolt fell from the hole. 3 right section #3 entry #1 block.
EE stated while making a belt splice with a flexco tool and hammer, he placed his left thumb on the tool and struck it with a 3lb hammer. This resulted in 1 suture.
The employee stated he was cutting metal bands off the guards to be installed on a new belt storage unit when the band flipped up and struck his arm. He received five sutures to close the wound.
The EE stated he was swinging a 8lb. sledge hammer to knock blocks out of a stopping when he missed the block and his right arm hit the sharp edge of the gunnite on the stopping. He received six sutures.
EE stated he was going to change a light bulb on the section loading machine. Using a razor knife, he attempted to cut rags that had been wrapped around the light. During this process he cut him- self above the left wrist. The injury required sutures.
The employee stated he was using a utility knife to cut the belt out of the storage unit when the knife blade slipped out and stuck his right arm above the wrist. The cut required two sutures.
EE stated that he was using his left hand to steady a block that he was going to cut for a footer. When he swung the axe with his right hand the axe handle hit his belt and spun around, causing the head of the axe to hit his left hand.
The employee has been diagnosed with degenerative changes in his cervical spine.**Employee thought he pulled a muscle while loosening bolts.**
EMPLOYEE STATED HE WAS REMOVING BOLTS OUT OF A LIGHT IN THE LOADER TO REPLACE THE BULB. EE SAID THE RATCHET BROKE HURTING HIS RIGHT ARM AND ELBOW. EMPLOYEE DID NOT BEGIN MISSING WORK UNTIL 10/24/2005.
EE STATED HE WAS HELPING COWORKER SKIN A CABLE THAT WAS GROUNDED. EE HELD THE CABLE WHILE THE COWORKER CUT A RING AROUND THE CABLE. UTILITY KNIFE CUT EE'S LEFT MIDDLE FINGER - 2 SUTURES.
The employee was using a 5 ton lifting jack to raise the belt drive frame which was caught on the I-beams for the belt drive pad. The ee had one foot on the handle of the jack and one foot on the bar when his foot slipped and was caught between the handle of the jack and bar in the lifting jack.
