Employee was using a slate bar to remove a rock from on top of the continuous mining machine. In the process of prying the rock off of the continuous mining machine, the rock slid down the slate bar and smashed the employees little finger on his left hand.
Employee was helping set timbers at 13 brk on 3 South belt line. He was using a pick hammer to set the cab wedges. He swang to hit the cap wedge with his hammer. When he did the hammer bounced back and the sharp end of his pick hammer struck him in the face.
Employee was using a pair of chain hoists to pull in belt on 4 South. In the process of operating the chain hoist the clamp came loose off of the belt and caused the employee to fall off the "A" frame where the employee was standing beside of the rock box. This caused the employee to strain his right knee, ankle, and put a small bruise on his face.
Employee was using a utility knife to cut a dust hose for a roof bolter. The employee cut his left wrist as he was cutting the hose.
Employee was helping another employee remove a tram motor from a shuttle car. The other employee was tapping on the gear with a hammer in order to try to remove the gear. A piece of the gear penetrated the employee's right forearm.
Employee cut a metal band on a roll of belt and the band struck him in the face chipping his tooth. *Management did not deem the accident to be reportable until 7/20/2014.
Employee was making a cable splice with a spice knocker. He struck the splice knocker with a ball peen hammer and a piece of the splice knocker chipped off and embedded in his left hand.
Employee was jacking a tub back onto the rail. In the process of pushing the tub back on the rail, the jack creeled over and struck employee on the right middle finger causing a laceration.
Employee was changing bits on the continuous mining machine. One of the bits was broke off in the bit lug. Employee was using a hammer trying to beat the broken bit out of the lug. At this time a piece of metal flew back striking the employee in the left forearm.
Employee was helping change bits on the shear. Another employee was using a pick hammer to get mud out of a bit insert. A piece of the pick hammer chipped off and hit the employee in the face.
Employee was replacing a broken tie rod on a shuttle car when the pliers he was using slipped. *Did not become reportable until 7/31/13.
Employee was using a utility knife to cut a hole threw a piece of heavy muddy line curtain. The utility knife slipped, cutting his left index finger.
Employee was using a sledge hammer to knock out a stopping and caught his hand between the hammer handle and the stopping this caused a laceration that required 3 stitches to close the wound.
Employee was installing continuous miner cable into the main junction box on the continuous miner. He was using a ball peen hammer to guide the cable into the junction box. The hammer slipped and his right ring finger was caught between the hammer and the top of the junction box.
EE was leveling belt structure with a jack when his hand slipped off of the jack handle which then flipped up striking him in his right eyebrow causing a laceration which required 9 sutures to close.
EE was tensioning 10' post tension cable bolts in the #2 S. belt track entry. While holding the tensioner he was unaware that his left index finger was between the ram and the tensioner. When he instructed his co-worker to retract the ram the tensioner retracted amputating the tip of the EE's left index finger.
Employee was cutting line curtain when the knife slipped and cut his right index finger.
EE was changing bits on the longwall shearer while using a hammer to set a bit a small piece of metal became airbourne and became embedded in EE's right eye. EE was wearing RX safety glasses at the time of the accident.
Employee was using a chain hoist to turn over a relay bar on the longwall face when the hook slipped off striking the employee on the right forearm causing a laceration which required 4 sutures.
EE was repairing wire on 2 way radio with a knife. Knife slipped and he cut his right leg just above the knee.

EE was using a utility knife to cut some length off of a strand on his respirator and cut his left thumb.
Foreman was operating a chain hoist, when chain broke and hoist hit him in the back of the head. This resulted in a laceration that required two (2) stitches.
Employee was using a pipe wrench to unscrew a 2" pipe apart when he slipped and twisted his knee.

Un-lugged 80 lb steel rail from ties and was moving it over with a slate bar when he felt pain in his back. He continued to work until 5-25-05.

HE WAS DROPPING HARD LINE DOWN, HE TOOK THE BELT LOOSE AND THE CHAIN CAME LOOSE HITTING HIM IN THE MOUTH.
INJURED WAS USING A SLEDGE HAMMER TO HELP TIGHTEN A BOLT WHEN IT STRUCK HIS RT. THUMB RESULTING IN A FRACTURE.
INJURED WAS ASSISTING ANOTHER EMPLOYEE SETTING A TIMBER WHEN HIS RIGHT WAS STRUCK WITH A HAMMER.THIS RESULTED IN A LACERATION TO MIDDLE FINGER ON RT HAND.
INJURED WAS USING A HAMMER TO REPAIR A DOOR HINGE WHEN A PIECE OF THE HAMMER CHIPPED OFF STRIKING HIM IN THE FOREARM.
INJURED WAS USING A KNIFE TO CUT A BUCKET WHEN THE KNIFE SLIPPED. THIS RESULTED IN A LACERATIONTO HIS LEFT THUMB.
