The roof bolter operator had completed drilling a 5 ft. test hole that required using 3 sections of drill steel. As the operator removed the lower section of drill steel the 2nd section fell on the drill head, striking the operator on the lower lip resulting in a laceration which required 5 sutures to close the wound.

Due to an accumulation of water in the #4 heading the bolter crew was experiencing vacuum problems on roof bolter. The employee changed the dust bags and was cleaning the mud and coal particles from the dump valve canister. His right hand was caught in the dump valve due to his co-operator setting the stab jack on the machine which also activates the dump valve canister.

The employee was checking the amount of supplies on the truss bolting machine. He was positioned on the operator side of the machine with his left foot near the front tire. As the bolter was trammed forward, the employee's foot was caught-under the machine tire resulting in a fracture to the employee's foot.

The bolter operator was backing out of the #3 cross cut after completion of bolting. Emp. was positioned in the tram deck of the machine looking over his shoulder with his left arm positioned outside of the confines of the operator deck. His left arm was caught-between the machine & the coal rib resulting in scratches & bruising to the arm.

Employee was tramming a single head roof bolter & was operating the tram levers with his right hand and his left hand was positioned inside an empty canopy post. An extra section of drill steel had been inserted into the canopy post that extended 18 inches above the machine. The drill steel contacted the mine roof and the operator's middle finger was caught between the steel & post

The employee was installing a 5' torque-tension roof bolt in a 40" mining height which required the bolt to be bent. The operator had started the bolt into the mine roof and was attempting to straighten the bolt when he felt pain in his back.

The CONTRACTOR ee was riding a track vehicle to examine beltlines. The vehicle derailed in a switch at the No. 4 Drive. The injured was using a hydraulic jack to raise the vehicle & pull it over onto the track. As he was raising the vehicle, the jack swiveled causing the jack to fall and strike the ee's finger causing a laceration & fracture.

The employee was pushing a roof bolt into the hole he had just drilled. The bolt bent, catching his finger between the roof bolt head and head of the bolter.

EE trammed the dual head roof bolting machine from No. 6 heading to No. 5 heading. He backed the machine up into No. 5 heading to turn around & take bolter to No. 1 heading. As he was backing up the bolter-cable fell off the back of the bumper. EE attempted to put the cable back on the bumper with the bolter still moving & caught his hand between bolter & coal lying on rib.

The fan blades on the exhaust mine fan broke out. A piece of a fan blade came through a small opening between the fan housing shroud & the connecting tubing & struck the contractor employee on the left foot behind his big toe causing a laceration that cut tendons & fractured his foot.

Employee was roof bolting back area. Installing roof bolt and had to bend the roof bolt to place bolt into the head of the roof bolter. Holding the roof bolt he raised the head up to the bolt. As he placed the roof bolts head into the head of the bolter, he accidentally hit the rotation lever instead of the up lever to raise the head. His glove caught on the bolt and his fingers

Bolting machine broke down because the drill head became fouled. As the EE was attempting to free the head, the bolts in the bottom of the drill feed jack broke, allowing the lower part of the jack to become free as the EE was attempting to free the head up and he placed his left leg under the head of bolter. It suddenly became free and fell approx 36", catching his left leg.

The EE finished bolting the face of the #3 heading on the 002-0 section. He backed the machine up approx 40' out of the face and attempted to turn the machine around in the heading. As he was turning the machine, he twisted his body to the left to look at the location of his cable and in doing so, placed his elbow between light mount and coal rib.

EMPLOYEE WAS USING A HAND HELD HYDRAULIC DRILL DRILLING A HOLE FOR AN ANCHOR PIN TO ANCHOR TAILPIECE, WHEN THE GASKET IN THE TOP OF THE DRILL BLEW OUT CAUSING HYDRAULIC OIL TO HIT HIM IN THE ARM.

A PIECE OF SLICKEN SIDED DRAW SLATE MEASURE OVERALL FELL FROM BETWEEN THE 4TH AND 5TH ROWS OF BOLTS FROM THE STIRKING THE EE ON THE HEAD WHILE HE WAS RUNNING THE CONTINUOUS MINER IN THE NO. 2 HEADING. THE AREA OF THE DRAW SLATE THAT STRUCK EE APPROX. REMINDER MINER DECK. THE RESULTING BLOW PUSHED THE EE HARD HAT DOWN ON THE FOREHEAD CAUSING SMALL LACERATION JSUT ABOVE THE LEFT EYE

EMPLOYEE WAS CUTTING AN I-BEAM APART. AS HE WAS CUTTING THE I-BEAM, IT SHIFTED AND FELL ON EMPLOYEE'S FOOT.

EMPLOYEE WAS WORKING LEVER ON MANUAL CABLE PULLING CONVEYOR BELT BACK TOGETHER. WHEN EMPLOYEE COMPLETED PUSHING CYCLE ON HOIST HE RELEASED LEVER. HOIST DID NOT CATCH AND LEVER CAME BACK STRI KING HIM IN THE RIBS.

EMPLOYEE PREPARED A ROOF BOLT TO PLACE IN A HOLE HE WAS DRILLING IN THE ROOF. DUE TO THE AREA BEING WET, THE EMPLOYEE PLACED THE ROOF BOLT ON THE FOOT OF THE ATRS TO KEEP IT OUT OF THE WATER. AS HE WAS DRILLING, THE BOLTER HEAD STOPPED UP AND HE LOWERED THE HEAD TO CLEAN OUT THE BLOCKAGE. AS HE LOWERED THE HEAD THE ROOF BOLT HE PLACED ON THE FOOT OF THE ATRS ROLLED TO AN AREA WH

THE EE WAS DRILLING A 13/8 IN 10' DEEP TEST HOLEIN THE INTERSECTION OF THE LAST OPEN CROSSCUT IN THE NO.2 HEADING. AS HE WAS ADDING MORE DRILLSTEEL & PUSHING THE STEEL INTO THE HOLE, HE FELT A SHARP PAIN IN HIS LOWER BACK

EE WAS PULLING A SHIELD WHEN A ROCK WAS THROWN FROM THE SHEAR & STRUCK HIM ON THE RIGHT TEMPLE

WHILE INSTALLING A BOLT IN THE TOP A PIECE OF CANNEL COAL FELL AND STRUCK HIM ABOVE THE RIGHT EYE. 4 STITCHES WERE REQUIRED.

ROCK FELL FROM BETWEEN STRAPS & STRUCK HIM ON THE HEAD. SUTURES WERE REQUIRED.

EE WAS OPERATING A BOLTER WHEN THE WRENCH BROKE STRUCK HIM ON THE MOUTH-SUTURES WERE REQUIRED ON HIS LOWER LIP

ON 12-17-96 EE WAS PERFORMING OVERHEAD WELDING DURING WHICH A PIECE OF HOT SLAG ENTERED HIS LEFT EAR. AT THE TIME OF THE ACCIDENT THE EE WOULD NOT GO TO THE DR. SINCE THE ACCIDENT THE EAR HAS BECOME INFECTED. THE ATTENDING PHYSICIAN INSERTED A TUBE IN THE EE'S EAR ON 2-18-97. DUE TO THE EE HAVING THE TUBE PROCEDURE PERFORMED BY A SPECIALIST IN DAY SURGERY, THIS EE WAS NOT ABLE TO M

EE WAS TRAMMING SHEAR DOWN FACE, WHEN ONE SHIELD JACK BUSTED. THE FLUID HIT EE ON THE BACK SIDE.

THE EMPLOYEE WAS OPERATING A JOY 10-CM CONTINUOUS MINER. WHILE MOVING THE MINER IN THE NO. 9 ENTRY INTERSECTION, TWO CROSSCUTS INBY SPAD NUMBER 634, THE MINER CROSSED AN UNEVEN SECTION OF MINE FLOOR. THIS CAUSED THE REAR OF THE MINER TO RAISE, CAUSING THE EMPLOYEE TO BUMP HIS HEAD.

HE WAS WELDING A LUG AND THE WELD BROKE LOOSE AND THE LUG HIT HIM IN THE MOUTH.

AIR HOSE FLEW OFF AND HIT UNDER LEFT EYE. STITCHES REQUIRED AND BRUISE WAS UNDER LEFT EYE.

EMPLOYEE WAS STRUCK BY A PIECE OF DRAW ROCK WHILE BOLTING THE #3 HEADING THE ROCK FELL FROM THE SIDE OF THE ATRS BACK TO THE PERMANT SUPPORT

A PIECE OF DRILL STEEL HUNG AGAINST ATRS ON BOLT MACHINE. THE PIECE OF STEEL CAME LOOSE, HITTING EE ON THE RT. SIDE OF HIS HAT, CUTTING THE TO OF HIS RT. EAR. THIS INJURY REQUIRED STITCHES.

EMPLOYEE WAS HIT BY A SMALL PIECE OF DRAWROCK BEHIND LEFT EAR. THIS INJURY REQUIRED 2 SUTURES.

THE ROOF DRILL OPERATORWAS IN THE PROCESS OF INSTALLING A ROOF BOLT WHEN A PIECE OF DRAW SLATE FELL OUT BETWEEN THE BOLTS IN THE SECOND OUTBY ROW OF BOLTS STRIKING HIM ON THE BOTTOM OF THE LEF T FOOT

EE WAS CLEANING OUT THE TRAY, WHEN A SMALL PIECE OF ROCK WAS THROWN FROM THE FACE, BY THE SHEAR, HITTING EE ON THE LEFT HAND. THIS INJURY REQUIRED STITCHES.

A SMALL PIECE OF ROCK FELL FROM THE SHIELD TIP, HITTING EE ON THE LEFT UPPER LEG. THIS INJURY REQUIRED SEVEN STITCHES.

EE WAS INSTALLING BOLTS, WHEN HE PUT TO MUCH PRESSURE ON THE WRENCH, CAUSING THE WRENCH TO BREAK HITTING EE ON THE THUMB.

EE WAS SITTING NEAR THE LEFT RIB APPROX. FOUR ROWS OF BOLTS BACK FROM THE FACE WHILE REPAIRS WERE BEING MADE, WHEN A PIECE OF ROCK FELL STRIKING HIM ON HIS LEFT SHOULDER. THE BOLTS WERE WITHIN 336" OF THE RIB WHERE THIS PIECE FELL.

EE WAS CUTTING A PIECE OF METAL, WHEN A PIECE OF HOT METAL WENT DOWN HIS BOOT.

EMPLOYEE WAS LOADING COAL FROM A BARRIER PILLAR AT SPAD #749 ON "M" SECTION WHEN A MOUTNAIN BUMP OCCURRED. THE BUMP FORCED COAL FROM THE SURROUNDING COAL RIBS, SOME OF WHICH ENTERED THE OPERA TOR SCOMPARTMENT CUASING UPPER BODY CONTUSIONS.

WHILE IN THE PROCESS OF INSTALLING A ROOF BOLT, THE EMPLOYEE CAUGHT HIS RIGHT THUMB BETWEEN THE MINE ROOF AND A SECTION OF DRILL STEEL. THE ACCIDENT CAUSED A LACERATION TO THE RIGHT THUMB WHIC H REQUIRED SUTURES.

EMPLOYEE WAS HOOKING UP A CHAIN ON THE BACKHOE OUTRIGGER, WHEN HE ACCIDENTLY HIT THE LEVER, PULLING THE CHAIN AGAINST HIS FINGER. THIS INJURY REQUIRED STITCHES.

EE WAS STANDING BESIDE THE BOLTER WITH HIS HAND ON THE CANOPY WHEN A PIECE OF DRAWROCK FELL STRIKING HIM ON THE HAND. HIS HAND WAS BRUISED. THE ROCK WAS APPROX. 32" LONG BY 11' WIDE X 1" THICK .

A PIECE OF DRAW ROCK FELL, HITTING A PIECE OF BOLTER STEEL CAUSING IT TO FLY UP AND HIT DANIEL O THE FOREHEAD. THIS INJURY REQUIRED STITCHES.

A PIECE OF DRAWROCK FELL STRIKING EE ON THE LEFT ARM, WHILE DRILLING A HOLE TO INSTALL A ROOFBOLT. THIS INJURY REQUIRED STITCHES.

EE HAD FINISHED DRILLING A TEST HOLE AND STARTED TO WALK AROUND TO GET A CAN OF PAIN, WHEN A PIECE OF DRAWROCK FELL HITTING EE ON THE UPPER BACK.

EMPLOYEE WAS INSTALLING A STRAP, WHEN A SMALL PIECE OF METAL HIT HIM IN THE EYE. EMPLOYEE HAD TO HAVE THE PIECE OF METAL REMOVED BY A O.D.

PINNER STEEL PINCHED HIS THUMB & FOREFINGER WHEN IT JOINS TOGETHER

OPERATING A 685 ROCK TRUCK BEING LOADED BY SHOVEL. SHOVEL SWUNG AROUND TO DUMP BUCKET WHEN LOSTPOWER TO SHOVEL SWINAND HOIST. BUCKET CAME AROUND AND HIT BACK OF TRUCK.

DRAWROCK FELL, HITTING EE ON THE RIGHT LEG. THIS INJURY REQUIRED STITCHES. THE DR. PUT EE ON RESTRICTED DUTY FOR 10 DAYS.

THE INJURED CAUGHT HIS LEFT THUMB BETWEEN THE MINE ROOF AND SECTION OF DRILL STEEL WHILE PREPARING TO DRILL A ROOF BOLT HOLE. THE ACCIDENT RESULTED IN THE AMPUTATION OF HIS LEFT THUMB, BELOW T HE FIRST JOINT.

OPERATING RUBBER TIRED DOZER - RAN OVER ROCK JERKING HIM - STRAIN TO LOWER BACK.
