Employee was installing split steel and didn't know his glove was caught. Employee went to Good Samaritan Hospital sustaining a fracture to his finger which requires surgery and will be off work for an uncertain period of time.
Injured was grinding on piece of metal in the shop. The grinding wheel kicked striking his left hand resulting in a laceration on the top of his hand which required stitches.
Injured was trimming top out of the #3 face on the #1 unit when a piece of carbide struck the injured in the head causing a laceration which required stitches.
Roof Bolter on Unit#5 was trammed up on the right foot of EE by another ee who was at the side tram controls. EE was leaning over the mid section of bolter on the right side of bolter. He was cleaning off space for some bolts and did not hear bolter start up due noise from forklift.
Injured was bolting in the #9 entry using a split steel and went to put in second steel and missed the chuck. This caused the steel to flex and contract striking his right hand index finger.
Was guiding steel into chuck. Misalignment occurred causing a springing action of steel. Tore a laceration into left hand.
Employee was tramming the bolter when he lost his balance falling and trammed over his foot.
Injured had pinner steel fall out and strike his thumb on his left hand. Injured initially received first aid only. On 08/20/2013 injured was still having discomfort in his hand and went to see doctor for x-rays and discovered fracture in his thumb.
Employee was roof bolting when he felt pain shoot in his back. At the time of the incident employee didn't take any medical treatment. Employee is now having pain and is seeking medical treatment.
Employee was roof bolting when drill boom came in contact with his leg resulting in rolling his ankle. Employee went to Good Samaritan Hospital where he was ordered to remain off work for an uncertain period of time.
Employee was operating the roof bolter when he placed his hand on the canopy then raised the boom smashing his hand between the roof and the canopy. He went to the hospital sustaining a fractured pinky.
Employee was installing roof bolts and as he was removing the drill steel rock fell hitting his canopy. A part of the rock that had fell broke off and flipped under the canopy striking his neck and back. Employee was immediately taken to hospital where he sustained contusions and abrasions.
Employee was pulling down a roof bolt and as he pulled down on the bolt his arm struck the plate resulting in a laceration. Employee went to hospital where he received sutures and returned to work his following shift.
Injured fractured his right pinky finger, after catching the end of it with the retention jaws on the Sandvik Bolter.
Injured was Roof Bolting on the #3 Unit, injured was using a split steel when he went to insert second steel it caught the lip of the pod and buckled the steel which struck the palm of his hand. This caused a closed fracture of the distal end of the radius.
Smashed finger while roof bolting
Injured states that drill chuck fell and struck his left wrist causing to swell. Injured was taken to doctor for x-rays and discovered had a hair-line fracture on his Radial Styloid.
Injured was moving miner from a completed cut in the #6 Entry to begin mining in the #9 Entry. The tail piece of the miner caught the injured employee and wedged him at the corner of the #6 Entry.
Employee was unclogging a dust hose when it came unclogged and the hose became uncontrollable with rock dust flying out striking employee. Employee went to Good Samaritan Hospital sustaining fractured ribs, employee returned to work the next day.
A miner operator was cutting in the #5 entry at the 169-xc on the #1 unit. A piece of Carbide come of a bit and struck the operator in the left arm. The operator received three stitches making this a reportable accident.
Employee was operating Miner #9 and started to cut the corner and hit a roof bolt with the head of the miner. Part of the bolt flew off and hit him in the nose resulting in a laceration requiring stitches.
Employee was operating a roof bolter when the pinner steel broke and struck him in the mouth. Employee immediately came out and went to Vincennes Hospital, sustaining a laceration that required 3 stitches. Employee returned to work the following workday.
Miner operator was cut on his forehead over his left eye by a piece of car-bite from a miner bit. Operator received stitches for his laceration making this a reportable incident.
Injured was loading the roof bolter in the 3-left cut on unit #2, was finished loading cable bolts and had went back to the scoop. The operator of the bolter started the machine to set over to load more supplies, injured darted back beside the machine and was struck by the machine. Injured was taken to local hospital and treated for a pelvic fracture and was released.
Injured was roof bolting on the #3 unit when his steel become clogged, he removed the steel and got the steel unplugged. When he placed his steel back in the pot, it became wedged and caused the steel to bow. Injured lowered the pot back down and steel sprung and struck his hand. Fracturing the knuckle in his hand.
Injured was roof bolting on the #2 unit and his drill steel had become lodged in the mine roof temporarily and then fell and struck his left hand. Injured was taken to local hospital for treatment for his hand. Discovered that there was a fracture to the metacarpal bone, making this a reportable accident.
Injured was roof bolting outby on a travelway, when he placed his hand on an area of the bolter and hit a wrong lever and smashed his right ring finger. Injured was taken to local hospital and received treatment and is awaiting surgery for his finger which was broken.
Employee was roof bolting when he drilled a crooked hole into very hard roof strata. When the drill pot was lowered, the steel snapped back and struck the employees right forearm, resulting in a simple fracture to the right radius.
Employee was installing a roof bolt with a pin board attached. As the employee depressed the boom raise lever with his left hand, he attempted to adjust the pin board with his right hand. The employee did not release the boom raise lever or the pin board and his right index finger was caught between the pin board and the mine roof at the first joint.
EE was rock dusting when hose slipped out of his hand and dust blew into his left eye. Management was made aware that days missed from work were attributed to incident on 10/09/09.
Employee was walking to equipment when bolter cable came untied from roof and struck employee on the side of his head. Employee sustained a concussion.
Employee was roof bolting with 2 piece steel. When he inserted 2nd steel, he put too much up pressure on steel. Steel snapped out and struck left hand causing laceration requiring sutures.
EE was inserting additional drill steel while roof bolting; steel in the roof fell out stiking him on the right hand-palm area. He missed work due to swelling and pain on 6/21/2008.
Employee inserted 2nd steel while roof bolting and raising boom, steel bowed then popped into chuck causing steel to strike right hand. He sustained contusion and swelling to the right hand. He missed work on 1/14/08
Employee attempted to hook cable ropes onto tail of moving miner when tail struck employee. Employee sustained a simple fracture of the pelvis. This accident occurred on #2 unit at 27 crosscut, between #3 & 4 entries.
EE WAS ROOF BOLTING ON THE LEFT SIDE OF THE ROOF BOLTER. EE HAD LEFT HAND RESTING ON THE ARM OF THE DRILL POT ASSEMBLY WHILE DRILLING A HOLE, AS THE DRILL POT WAS GOING UP, LITTLE FINGER GOT CAUGHT BETWEEN THE ARM AND THE CANOPY OF THE ROOF BOLTER
EE WAS ROOF BOLTING. HE HAD COMPLETED DRILLING A HOLE AND WAS ROTATING THE STEEL TO HELP GET IT OUT OF THE HOLE WHEN THE WRENCH CAME OFF OF THE STEEL AND OUT OF THE DRILL CHUCK, STRIKING THE EE ABOVE THE RIGHT EYE, CAUSING A LACERATION.
EMPLOYEE WAS ROOF BOLTING AND WHEN HE DRILLED A HOLE THROUGH A PIECE OF ROCK, HE TURNED BACK TO GET HIS GLUE & ROOF BOLT TO INSTALL WHEN THE ROCK HE HAD DRILLED THROUGH FELL HITTING HIM ON THE TOP OF THE FOOT.
EE was roof bolting when a piece of rock 2'x2'x2" fell out between the roof bolts, striking the ee on the left shoulder and back area.
EMPLOYEE POSITIONED BOLTER FOR FIRST ROW OF BOLTS, A ROCK FELL FROM BETWEEN ROWS OF BOLTS AND STRUCK EMPLOYEE ON RIGHT ANKLE. EMPLOYEE SUSTAINED A FRACTURED RIGHT ANKLE.
AN ACCIDENT OCCURRED ON #1 UNIT IN #2 ENTRY JUST OUTBY THE LAST OPEN CROSSCUT. AS THE CONTINUOUS MINER BEGAN TO BE TRAMMED FROM #2 ENTRY TO #7 ENTRY THE VICTIM STEPPED BETWEEN THE TAIL & THE R IB. THE TAIL OF THE MINER STRUCK THE VICTIM IN THE ABDOMINAL AREA. EE RECIEVED CONTUSION TO THE ABDOMINAL AREA.
AS EE POSITIONED HIMSELF TO INSTALL ROOF BOLT, BELT OR LAMP CORD CAUGHT ON CANOPY VALVE LEVER CAUSING THE CANOPY TO LOWER. EE RECEIVED A LACERATION TO THE RIGHT INDEX FINGER.
EE WAS ATTEMPTING TO INSTALL 12'CABLE BOLT IN THE SAME HOLE FOR THE 2ND TIME. AS HE RAISED THE DRILL POT HE STARTED THE ROTATION. THE END NUT CAME OFF OF THE BOLT STRIKING EE IN THE LT. EYE. THE EYE HAS BEEN STITCHED, DAMAGE TO THE EYE IS UNDETERMINED AT THIS TIME. LOCATION #1 UNIT, XC28, CROSSCUT 3-2.
A SMALL PIECE OF ROOF ROCK FELL AND STRUCK VICTIM ON THE RIGHT ANKLE. SPRAINED ANKLE AND SMALL BONE CHIP.
EMPLOYEE TRAMMED BOLTER AND PINCHED LEFT THIGH BETWEEN ROOF BOLTER AND COAL RIB. EMPLOYEE SUSTAINED 2 INCH LACERATION AND CONTUSION TO THIGH. NO FRACTURES.
AS EMPLOYEE WAS ROOF BOLTING IN CROSS CUT 2-1 IN #5 ROOM, A PIECE OF DRAW ROCK FELL FROM BETWEEN THE BOLTS MEASURING 3' X 5' X 6" THICK. THE ROCK BROKE EMPLOYEE'S RIGHT ARM.
EMPLOYEE MOUNTING CRANE TO OPERATE IT WHILE CONSTRUCTING PREP PLANT. THIS REPORT LATE DUE TO WORKER THINKING HE HAD A ROUTINE MUSCLE SPASM.
