employee alleges the auto darkening on welding hood did not engage and their eyes sustained minor flash burn

Employee alleges they were cutting with a cutting torch and the slag ignited solvent nearby causing second degree burns to their forearms and neck.

Employee alleges EE was grinding and sealing some material and some rust went into EE's left eye.

Employee utilized an air hammer to free a stuck part when part of the tooling broke off and punctured their arm.

Employee was using a torch to make holes in the fog washer and a foreign object blew into EE''s right eye. The Dr. had to use a needle to move the foreign object out of EE's eye by the cornea. Late reporting due to communication error.

Employee alleges they were grinding some material off of a table and a piece of metal blew into their left eye.

Employee noticed irritation in right eye and associated it with grinding metal the previous day. Waited 1.5 days to notify management at which point EE was sent for medical on 12/15, foreign object removed from eye. Employee was wearing prescription glasses during grinding instead of Company supplied goggles.

Employee was using cutting torch and a piece of hot metal slag fell down inside employee's boot causing a burn on employee's lower leg

Employee alleges was grinding surface to weld studs and the shrapnel bounced off of the wall and went into EE's grinding hood in EE's left eye.

Employee alleges they were shutting down plant and kicked the clutch out of gear hitting right elbow on the ladder bolt.

Employee alleges they were putting air in a tire and a release of compress air blew material in eyes.

Employee alleges they were in the process of breaking steel apart with a drill and they started feeling pain in the right shoulder. Employee started loosing time on 10/12/2018, but has returned to work on 10/16/2018.

Employee alleges EE was using a torch to heat up a bolt and a foreign object blew into right eye.

Employee alleges EE was cutting a piece of steel on a truck frame with a grinder and the wheel slipped from EE's hand and cut EE's left arm. EE received stitches.

Employee alleges he was using a cutting torch to cut a V bolt and he removed his gloves to do something and when he went to grab his gloves the torch burned his right fingers.

Employee alleges he was grinding metal and a piece of metal came off and hit the edge of his goggles and flew into his eye.

Employee alleges his safety glasses fogged up as he was welding, he removed them to clean them and the weld check cracked and released a piece of metal that flew into his eye.

Employee alleges he was kneeling on a belt drilling holes and he felt a pop in his left knee.

Employee alleges he was using an angle grinder that he had modified and the wheel came apart causing lacerations to his right leg.

Employee alleges he was grinding clips on a belt and a piece of metal got into his right eye.

Employee was using a hand grinder with a 4" cutoff wheel to cut an opening on an electrical box when the grinding wheel caught a corner, broke apart, and kicked the grinder back, striking the employee in the left hand, just below the base of his pinky finger causing a laceration.

Employee alleges after adjusting jaw crusher tension rods he backed up 8 feet and started up jaw. The bolt that attaches the tension bar yoke to the moveable jaw failed and the spring pressure caused the rod to release, become airborne and struck him in the left shoulder and left ear.

Employee parked the dozer on a small incline, stepped out onto platform then onto track. Dozer rolled backwards pinning employee's left foot in between dozer track pad and the platform. Employee fell onto the track then onto the ground. Employee did not set the parking brake. Employee injured left foot.

Employee was welding and had a steel hopper raised and secured with nylon straps. One of the straps failed causing the hopper to shift. Something hit the employee on his welding hood or shield causing injury.

Employee alleges he was installing door. It slipped pinning his 4 fingers on his right hand w/torch and steel plate burning his fingers.

Employee alleges he was loosening bolts on a blowbar with a impact gun and the bolts came loose pinning his hand between the crusher and impact gun bruising his left hand. *Late reporting due to employee was just scheduled for surgery on 12-18-09 and will start loosing time then.

EMPLOYEE WAS HOLDING IN POSITION A PIECE OF WATER PIPE ON ONE END AND THE CRANE WAS HOLDING THE OTHER SO THEY COULD BE WELDED TOGETHER. THE END THE EMPLOYEE WAS HOLDING DROPPED DOWN 6" TO THE GROUND CAUSING THE EMPLOYEE'S PINKY FINGER TO GET BETWEEN THE PIPE AND THE GROUND, CAUSING LACERATION.

Employee was using wheel grinder on a piece of plate steel. Foreign object in right eye.

Employee alleges he was using a torch to cut steel and it went down into his glove causing 2nd degree burn to his left hand and wrist.

EMPLOYEE WAS TEST RUNNING A 1710 CUMMINS ENGINE WHEN A FREEZE PLUG BLEW OUT STRIKING HIM ABOVE THE BROW AND SPEWING HOT LIQUID OVER EMPLOYEE'S NECK, HEAD AND ARMS.

Employee alleges he was getting ready to cut a piece of steel iron with a torch and he looked away at the tip of the torch for a minute as the torch was burning his leg.

Employee alleges he was drilling holes in beam and his glove got caught in drill, causing him to receive a broken left thumb.

Employee alleges he was drilling a hole, the drill slid down and he rolled out of the cab onto his back.

EMPLOYEE WAS WELDING IMPACT CRUSHER DOOR & KNOCKING OFF SLAG. THAT EVENING HE THOUGHT HE WAS SUFFERING FROM FLASH BURN. HE WENT TO THE EYE CLINIC & HE HAD A PIECE OF FOREIGN OBJECT LODGED IN HIS RT. EYE.

Employee was using power saw to cut a piece of plywood when he felt something in his left eye. (EE was wearing safety glasses).

EMPLOYEE WAS USING CUTTING TORCH TO CUT THE LEGS OFF A CONVEYOR. THE LEG FELL DOWN HITTING EMPLOYEE ON THE RIGHT SIDE OF THE FACE AND THE EAR. THE RIGHT EAR RECEIVED A LACERATION AS WELL AS A BURN FROM THE HOT METAL.

EMPLOYEE ALLEGES SHE WAS TRIMMING BUSHES AND THE HEDGE CLIPPER SLIPPED AND CUT HER LEFT POINTER FINGER.

EMPLOYEE ALLEGES HE WAS FEEDING THE IMPACTOR AND AS HE WAS TRYING TO KEEP UP WITH THE SECONDARY, HE HIT HOLES AND JAMMED HIS RIGHT SHOULDER.

EE ALLEGES HE WAS TAKING BOLTS LOOSE AND HE DROPPED THE BOLTS AND CUT HIS FINGER.

EMPLOYEE ALLEGES HE WAS WORKING AROUND WELDING AREA AND HIS EYES STARTED TO BURN.

EE WAS SPOT WELDING A PIECE OF PLATE STEEL. HE SLID OVER ROLLING HIS LEG OVER THE PLASMA CUTTER, CAUSING IT TO TURN ON AND BURN HIS LEG.

EE ALLEGES HE WAS TRYING TO LINE UP ELBOW WITH A PUMP. THERE WAS A CRACK IN THE WELD AND IT FLEW BACK AND HIT HIM ON HIS ARM, CUTTING HIS LEFT ARM.

EMPLOYEE WAS USING AIR CHISEL TO REMOVE HAMMERS FROM CRUSHER. A PIECE OF THE BIT BROKE OFF AND LODGED IN HIS CHEST.

EE ALLEGES AFTER GRINDING OPERATION, HE TOOK HIS SLEEVE ON HIS JACKET AND WIPED HIS EYEBROW AND HE FELT SOMETHING IN HIS EYE.

EE ALLEGES HE WAS GRINDING PIPE WITH A GRINDER AND HE STARTED HAVING PAIN IN HIS LEFT WRIST.

EE ALLEGES HE WAS HELPING DRILLER INSTALL 3 RODS AND COUPLINGS ON ROCK DRILL. THE OTHER EE MOVED FEED CONTROL LEVER AND CAUGHT INJURED'S HAND BETWEEN THE COUPLING AND THE ROD ARMS.

EE ALLEGES HE WAS FUELING LOADER. HE THOUGHT IT WAS FINISHED. HE PULLED OUT THE PUMP AND IT WAS STILL RUNNING AND FUEL SPLASHED OUT IN HIS FACE.

EE ALLEGES HE WAS GRINDING ON SHAKER TO REPAIR A HOLE AND A PIECE OF METAL HIT HIM IN HIS RIGHT EYE.
