ST Francois Quarry (CS54) Metal/Non-Metal

Employee alleges they were pulling a rock out of the jaw feeder and the strap came loose hitting them in the right arm. Employee received some cut and bruises. Employee went to the Dr. on 09/16/24 and has been placed on restriction. Employee started losing time on 09/17/24.

Employee alleges they were adjusting the teeth on a loader and their right hand got caught in between the fork and frame cutting the palm of their right hand. They received 5 stitches.

Employee alleges was unloading a truck and the co-worker threw a pipe off of the truck. The pipe hit employee on their left foot.

The employee alleges they were working at the plant with some flopping gates and screens and they hurt their lower back.

Employee alleges ee was moving some large culver pipes and one came off and ee tried to stop it from falling and ee cut left hand receiving 7 stitches.

Employee alleges ee was unloading a portable pump from a truck and ee tossed the chain around the front end of the loader and it got caught and came back and hit ee above right eyebrow. EE received stitches.

EE alleges was lifting screens out of a bucket in the loader, he felt something pop in his left elbow.

Employee alleges he was moving shot rock and the loader shifted causing him to slam his right elbow on the arm rest. Employee just started loosing time as of 12-14-2012.

Employee alleges he was lifting a stacker to slide in axles and the strap broke while he was attempting to put a pin in and it hit him in his left arm.

Employee alleges the gate was stuck and he was pulling on it and his hand got caught between the lever and the bolt cutting his right hand. He received stitches.

EE ALLEGES HE WAS REMOVING BEARINGS FROM SHAFT AND HIS HAND GOT CAUGHT BETWEEN THE PORTAPOWER AND THE ANGLE IRON.

EMPLOYEE ALLEGES HE WAS MEASURING BERMS AND AS HE WAS WALKING DOWN BERM HE SLIPPED ON SOME LOOSE MATERIAL AND FELL BREAKING HIS FORTH FINGER ON HIS LEFT HAND.

EE ALLEGES HE HAD FINISHED CHECKING OIL ON CRUSHER AS HE WAS GOING DOWN LADDER HIS FOOT SLIPPED OFF OF RUNG AND HE FELT PAIN IN HIS BACK.

CLIMBING LADDER TO HIS SEAT IN THE LOADER, EMPLOYEE SLIPPED AND FELL BACKWARDS APPROX 10' ONTO BUCKETS ON THE FLOOR.

DRIVE BELT CAME LOOSE ON SHAKER SCREEN. EMPLOYEE SHUT DOWN EQUIPMENT AND PROCEEDED TO ADJUST BELT. HAND SLIPPED AND GOT CAUGHT BETWEEN V-BELT AND PULLEY. INJURED 1ST AND 4TH KNUCKLE OF LEFT HA ND REQUIRING STITCHES IN BOTH FINGERS.

EE STRAINED HIS BACK WHILE SHOVELING.

EMPLOYEE WAS ATTEMPTING TO OVERRIDE A STARTER SWITCH ON AN ELECTRICAL BOX BY MANUALLY ENGAGING MAGNETIC STARTER.

HE WAS REPLACING FUEL TANK ON R-35 HAUL TRUCK. TANK WAS BEING POSITIONED AND PUSHED INTO PLACE BY ANOTHER EMPLOYEE. IT SLID TOO FAR AND SMASHED HIS LEFT THUMB AND FINGER.

EE WAS MANUALLY LOADING SMALLER ROCKS INTO JAW OF CRUSHER. HE PICKED UP BENT OVER A 25TH ROCK LA APPROX. ROCK AND STRAINED HIS LOWER BACK.

HE WAS REMOVING STONES FROM A BELT WITH A BAR. THE BAR SLIPPED AND SMASHED FINGER AGAINST METAL PLATE.

A FLAT ROCK FELL UNDER CONVEYOR BELT. THE BELT WAS STOPPED COMPLETELY. HE REACHED FOR THE ROCK AND THE ROCK TWISTED IN HIS GRASP, SMASHING HIS FINGERS.

I.NAME DRILLING. HAD JUST FINISHED ONE HOLE TURNED TO WALK AWAY WHEN ROCK FELL FROM WALL BEHIND AND STRUCK BACK. BRUISED BACK OF SHOULDERS,LEFT ARM.

INJURED WAS REPOSITIONING A HOSE ON THE DRILL WHEN HE WENT AROUND THE DRILL IT MOVED FORWARD SLIGHTLY AND ONTO FOOT.

I NAME WAS PUTTING SKIRTING ON JAW BELT WHEN HE STRUCK A BELT CLAMP WITH TH3E BAR HE WAS USING HE FLUNG HIS HAND AGAINST A METAL BRACE

EMPL WAS USING IRON BAR TO REMOVE LODGED ROCK FROM CRUSHER WHEN BAR CAUGHT & TWISTED LEFT WRIST.

FELL WHILE TAKING ENGINE COVER OFF OF LOADER 8'.

GETTING ROCK OUT OF THE CRUSHER PRY BAR KICKED BACK AND HIT ARM.

WHILE CLIMBING LADDER TO STORAGE AREA, LADDER SLIPPED ON CONCRETE, AND HE FELL LANDING ON LEFT FOOT ON FALLEN LADDER RUNG.

BRACING AS PULLING MOTOR INTO POSITION.

IN WAS WORKING ON GAURD OF FEED SCREEN WHEN FINISHED GOT ON FEED SCEEN ONE FOOT SLIPPED ON PLAT OF CRUSHER. FELL FACE FIRST ON FEED SCREEN ON ROCK KNOCKED OUT FORNT TEETH WHICH WENT THROUGH BO TTOM LIP.

WHILE EMPL APPLIED PRESSURE TO BELT FLASHING SECTION LET UTILITY KNIFE SLIP & JAB LEFT ARM.

IMPROPER HANDLING OF HEAVY PALNT(SCREEN CLOTHE) AND MOVING IN UNSTABLE BALANCE RESULTING IN STRAINED MUSCLE.SCREEN CLOTHE(244TH 58"X4' SIZE WIDE) QUARRY SUPPLY.
