The EE was performing services on the AC condenser of a 770 Caterpillar haul truck. The EE right hand slipped during repair and came into contact with the condenser fan causing a laceration to their right pinky finger that required stitches.

Employee was removing a roller frame on a conveyor belt by using a grinder to cut the bolts that held the frame. When EE went to start cutting the grinder bounced back off the bolt and contacted the right leg above the knee twice.

Employee was using a grinder and cutting disc to cut a u bolt on a trailer. While cutting the bolt the disc broke striking employee in the wrist and chest area causing lacerations one to the wrist area and one to chest area.

Spider cap was being lowered down on 8000 crusher - finger was placed in pinch point area

Employee was driving the long stick over to tower 5 to clean out around it. He started going down a slight incline. He lost control of the equipment causing it to run into/and under conv #14

Employee was putting the foot on the out rig of the crane. Out rig was extended out catching employees foot between the pipe and the out rig.

The employee was using a torch to cut a piece of metal off of a loader bucket when the metal struck his nose causing a laceration.

Employee was cleaning around plant with a small hand controlled compact mini-loader (Dingo) and backed it into his right leg bruising his foot and ankle.

Employee was installing screens on pep screen. Two other employees were told by the employee installing the screens to hand it to him and release it. When the two employees released the screen, the employee inside grabbed it and it slipped out of his hands and down onto his right leg pinching it between the screen and the cross member of the pep screen.

EE was using a 1-1/2" impact wrench, his hand slipped on the wrench and the locking pin punctured his right hand with a small puncture between fingers.

Employee was torching old wear metal off tail pulley. When torching, He hit a air or trash pocket in steel. That caused hot metal to shoot up and go under his glasses hitting him in the right eye.

This accident injured a contractor who was working by himself at our mine site. His was reparing the hammer (rock breaker). Somehow his hand became caught in the shaft by the cylinder. His hand received major abrasions to all four fingers.

Primary operator was greasing Jaw crusher. Mechanic was working on rock breaker. He did not realize that the breaker was under hydraulic pressure or that he was below greasing. He was checking a leak on a hydraulic line and when the hydraulic oil leaked out the hammer lowered and pinched operator who was below. No broken bones or internal injuries to operator.

Performing welding, machining and grinding operations and metal debris entered left eye.

Employee was welding during his shift. He woke the next morning with his eyes scratchy, swollen, hurting. His face looked as if he had been sunburned.

Employee was welding when a piece of slag fell in his boot causing a burn to the right foot.

The miner was welding on an empty oil drum that was not ventilated. The drum exploded causing the lid to hit his upper body and jaw causing injury.

CONTRACTOR WAS USING AN AIR LANCE TO MOVE BUILT UP MATERIAL. SOME OF THE MATERIAL BLEW BACK IN HIS FACE AND AROUND HIS SAFETY GLASSES GETTING A PARTICLE IN HIS EYE.

CONTRACTOR WAS INSPECTING THE CYCLONES TO TAKE MEASUREMENTS WITHOUT WEARING PROPER SAFETY EQUIPMENT.

EE WAS WELDING AND RECEIVED BURNS TO BOTH ARMS &STOMACH.

A HOIST CABLE BROKE. IT CAME INTO THE CAB OF A DRAGLINE ONTO THE OPERATOR. THE CABLE HIT HIS LEFT LEG, KNEE AND RIGHT HAND.

INJURED WAS AFENDING TO CRANE LIFTING A HAND RAIL. WHEN HE PULL ON THE CHAIN, WHICH SLIPPED UPWARD PINNING HIS LEFT THUMB AGAINST THE HANDRAIL. INJURED PULLED THUMB FREE. IN PAIN INJURED PULLE D GLOVE OFF, FOUND LACERATION AND SWELLING TO LEFT THUMB.

EMPLOYEE WAS IN THE PROCESS OF TURNING OFF THE CRUSHER AND A ROCK FROM THE CRUSHER FELL AND HIT HIM ON HIS RIGHT SIDE.

DRAGLINE JUMPED AND HE FELL OUT BACK DUE TO OIL & MINERAL SPIRITS ON FLOOR.
