Mostrando los 24The pigtail of a crane swung and caught the employee. Knocked off forehead and caused a laceration on EE's forehead.
While scooping dirt, the fire extinguisher at the employee's feet discharged spraying employee's left side and face. EE drove the skid steer to the air compressor and blew the powder off person and the cab of the skid steer and let the supervisor know. EE was still experiencing shortness of breath days later so EE got an inhaler to assist in reducing symptoms.
Two employees were standing on the plant structure using weed burners, there was an employee standing on each side of the conveyor leading to the hopper. One of the employees was wearing ice cleats, which caused that employee to lose balance but not fall. That employee then lost control of the torch and it passed in front of the injured employee's face.
Employee was grinding loose packing material from a cone crusher and in the process of setting down EE's grinder it caught EE's pants and cut EE's leg as it hadn't come to a complete stop before this happened.
Employee was grinding off old welds on a conveyor drive pulley to replace pulley lagging when a piece of metal went pass EE's safety glasses and face shield and entered EE's right eye
Employee was using an air compressor through a 2" line to clean out under a tail pulley and as he went to set the line down the ball valve turned on which caused the line to flair around and strike the lineman in the head causing a 2.5" cut required 4 staples to close.
Employee was welding above his head, a piece of slag from welding popped off and got into employee's left eye
Drill steel that sits in holder became loose while setting up at new hole. As employee began drilling new hole, drill steel fell out of its holder, employee tried to catch the drill steel. Employee felt pain in his left arm just above the elbow.
Employee was operating a D8 Dozer pushing up material, spun his tracks, this caused and buildup of material behind the tracks. As the employee backed up over this material the rear of the dozer slammed to the ground once it broke the center of the pile. The employee was twisted the right, looking back as this happen, jarring his back.
EE was rolling a screen in position with left hand and using a small hand grinder on the screen. Grinder caught on screen, kicked the grinder back striking ee on left thumb.
Operator of shovel was loading large boulder onto rear end of haul truck(60 ton haul truck). The boulder slipped out of bucket and dropped into bed of truck. The bed raised up and then came back down with force causing whiplash type injury to haul truck driver.
EE was using a torch to cut a weld. He turned his head and accidently pulled the torch across his hand.
EMPLOYEE WAS USING A CUTTING TORCH & A SPARK BURNED HIS KNEE.
CLOSING UPPER WINDOW ON RENTED CRANE, WHEN THE WINDOW SLIPPED & SLAMMED SHUT ON RIGHT THUMB. RT HAND 3RD FINGER CRUSHED & LACERATED.
EMPLOYEE WAS WELDING IN THE OXFORD GARAGE & CAME TO WORK THE NEXT DAY & REPORTED THAT HE HAD AN INJURY TO HIS EYES CAUSED BY A LOOSE LENS IN THE WELDING SHIELD. HE WENT TO THE CLINIC AFTER WOR K ON 12/5/02. HE WAS GIVEN TWO PRESCRIPTIONS AND ADVISED TO DO NO MORE WELDING UNTIL NEXT RECHECK ON 12/6, AT WHICH TIME HE WAS DISCHARGED FROM CARE WITH NO RESTRICTIONS.
EMPLOYEE WAS DOING A LOT OF WELDING ON THIS DAY. HE WAS OVEREXPOSED TO WELD FLASHES AND WAS NOT WEARING HIS PERSONAL PROTECTIVE EQUIPMENT. HE DID NOT REPORT THIS INCIDENT TO HIS SUPERVISOR UNT IL 10/30/02 OF WHICH HE THEN WENT TO THE CLINIC AND WAS DIAGNOSED WITH KERATITIS. NO RESTRICTIONS WERE GIVEN AND HE RETURNED TO WORK DOING HIS REGULAR JOB.
THE INJURED EE AND 2 MECHANICS WERE INSTALLING A BOOM ON A MAINTANCE CRANE. BOOM SECTION FELL AND PINNED EE TO THE CRANE TRACK AND GROUND. FRACTURING LEFT LOWER LEG AND INJURY LEFT SHOULDER.
GRINDING STEEL, FLECK OF STEEL GOT IN RIGHT EYE.
EMPLOYEE WAS OPERATING A DOZER ON TOP OF THE RAPPILE AND WAS WEARING HIS SEAT BELT. WHILE BACKING UP, HE DROVE OVER A MOUND OF FRESH MATERIAL FROM THE STACKER WHICH CAUSED THE MACHINE TO JOLT AND THIS JARRED HIS BACK.
EMPLOYEE WAS WELDING ON TRUCK BOX. WHEN HE CHIPPED HOT SLAG OFF OF WELD, A CHIP FLEW INTO HIS MOUTH.
RUNNING DOZER OVER RUTS. GOT BOUNCED AROUND. HURT NECK.
CUTTING STEEL WITH TORCH. TOOK OFF DARK GLASSES TO CHIP & CLEAN CUTTING. PIECE OF STEEL HIT LEFTEYE.
