INJURED EE WAS ATTEMPTING TO LIFT A COME-E-LONG UP TO ANOTHER EE BY SWINGING IT [BY HAND] UP TO HIM & INJURED EE PULLED A MUSCLE IN HIS SIDE.
EE DORMIRE WAS BRINGING A PICK UP INTO THE MAINTENANCE GARAGE TO GET ITS BRAKES REPAIRED. WHILE SLOWLY DRIVING INTO THE GARAGE THE BRAKES FAILED. THE PICK UP HIT A CART WHICH IN TURN, STURKC E E EE CAUSING HIS INJURES.

EMPLOYEE WAS CHANGING BLOW-EARS ON AN IMPACT CRUSHER TWO OF THEIR MPLOYEES WERE ASSISTING EMPLOYEE WAS USING A 12-LB DEAD-BLOW SLEDGEHAMMER TO DISLODGE THE BLOS-BARS WHEN HE SWUNG THE HAMMER C ARELESSLY & HIT HIMSELF ON THE LEFT KNEE WAS SENT TO DOCTOR RETURNED TO WORK SAME DAY
