Mostrando los 15While roof bolting, the chair in the roof bolter collapsed. Employee reached to stop the movement of the chair and fractured finger on left hand.
Employee was lining up a jack pad on undercutter #2. The steel slipped when attempting to line up the jack pad and pinched EE's middle-left finger between steel frame and pad.

When employee was running the under cutter the bug duster shear pin broke. When employee was clean around the bug duster the auger fell out and struck the employee on the left hand.
Employee was operating Fletcher Roof Bolter. A bearing plate for a bolt slid and struck his in left arm causing a laceration. He needed sutures.
Employee was using hand drill. Drill caught in wood and snapped back, causing a sprained right wrist. Employee was placed on light duty until sprain heals.
Employee was using a grinder grinding a hinge. The grinder caught on the wheel and kicked back and struck employee in the right index finger. Employee need three (3) stiches to close the cut.
EE WAS REMOVING CANOPY SUPPORT FOR A LHD WITH A CUTTING TORCH. WHEN CUT THROUGH, SUPPORT FELL STRIKING EE ON LEFT FOOT, BEHIND STEEL TOE. FRACTURED 4TH TOE ON LEFT FOOT.
EMPLOYEE WAS DRILLING WOOD REEL. DRILL STEEL CAUGHT. DRILL TWISTED AND TWISTED HIS HAND CAUSING A FRACTURE OF BONE IN HAND.
WHILE INSTALLING STEEL DECKING AROUND CV-015 DRIVE PULLEY, STEEL WAS BEING GROUND FOR A PROPER FIT. THE GRINDER KICKED BACK STRIKING THE BACK OFTHE RIGHT HAND.
EE WAS GRINDING DOOR HINGE WITH A HANE GRINDER. THE GRINDER KICKED BACK AND STRUCK THE EE'S RIGHT HAND. EE RECEIVED A LACERATION REQUIRING 4 SUTURES.
EE WAS STANIDNG BETWEEN MUCKER AND RIB. A LHD CAME BY AND STRUCK MUCKER IN FRONT TIRE, PINCHING EE BETWEEN MUCKER AND RIB.
