Mostrando 50 de 233The east brake on the Production Hoist was recirculating all oil to the tank. Valve V 10 was not closing. Replaced the valve and found a lot of rubber shavings in port holes leading into the valve. Flushed out port holes and installed valve. Everything tested good.
During the hoist exam when the hoisting engineer started EE's shift, EE discovered that a bolt was broken on the north brake pedestal on the service hoist. The hoist was locked out until repairs are made.
Employee was filling water tank on piece of mining equipment. When getting off of the tractor that was hauling the water tank, employee slipped and fell to the ground.
Employee was changing bits on undercutter. While changing the bits, employee tripped on a piece of rock, causing EE to fall against the undercutter bar. When falling against the cutter bar, employee bruised EE's left hand.
Employee was operating a small front-end loader. Employee said while operating the unit, EE received a Perianal Abscess.
Employee prying a cylinder pin and the pry bar slipped and struck their wrist, hand and forearm.
While doing maintenance on the undercutter, EE and another miner were driving a cylinder pin out and the hammer EE was holding while another miner was hitting it with another hammer, and their arm was forced down onto the cylinder, cutting their forearm requiring 3 stitches
While driving a kubota utv, some salt dust fell in eyes and when they went to wipe the dust from their eyes, they swerved to the right and drove into a chunk of salt and rolled the utv onto it's side. When they rolled the UTV, their mine belt had a harness ring on the back and that's what they landed on, fracturing their L2 vertebrae.
Employee was picking up a chunk of salt to place on a conveyor. Chunk of salt fell on left foot, causing bruise.
While roof bolting, the chair in the roof bolter collapsed. Employee reached to stop the movement of the chair and fractured finger on left hand.

The employee was using a utility knife to cut a plastic pail. While cutting the pail the employee slipped with the knife and cut their left forearm resulting in getting stitches.
Employee was hand scaling the mine roof when EE stepped into a hole in the mine floor. Employee sprained right knee.
Maintenance pickup truck caught on fire. No one was underground at the time the fire was discovered. The Company was in the middle of a maintenance shutdown. All vehicles had not been used underground for two weeks prior to the fire. No one had been underground except the personnel working on the Production Shaft rope change for two weeks prior.
Employee was installing frame of air door from a man basket attached to a telehandler. The basket came off of the forks of the telehandler. When to employee jumped down, EE bruised EE's left heel and left hip.
Employee was stacking 35-pound calcium bucket bags on a pallet. When the employee twisted around to stack the bag, they strained their lower bag.
Employee was troubleshooting a non-working light switch in a tight area. Employee stood up and hit head on an overhead beam. Employee started to fall and caught self, and doing so, twisted lower back causing a back strain. Employee missed three days of work.
Employee was lifting an under-cutter cable to make a repair on the cable. When EE was lifting the cable, EE felt EE's right shoulder pop. Possible rotator cuff injury. Employee started losing time on 09/19/2023.
Employee was using a down comer pipe on a piece of equipment when it slipped. Employee tried to catch it and fractured right thumb.
Employee was lining up a jack pad on undercutter #2. The steel slipped when attempting to line up the jack pad and pinched EE's middle-left finger between steel frame and pad.
Employee was stepping out of a Company Truck. When the employee placed weight on left leg, left knee started hurting badly.
Employee was stepping on a ladder to place a hooking strap to remove stored mafia blocks, the ladder slipped causing the employee to lose footing. Employee fell forward striking the mafia block, causing an abrasion on left elbow and also a sore midsection.
The auxiliary cage in the service shaft was removed and a maintenance inspection took place on the cage. Several items were repaired. Being that this unit is the 2nd conveyance coming into and out of the mine, all personnel were removed from the mine. The cage was prepared and placed back in service on Friday, April 14, 2023.
Employee was changing the ride control actuator on a frontend loader. The actuator slipped and slid, striking the employee's right hand. EE's middle finger on EE's right hand received a fracture. Placed on light duty for 14 days.
Employee was spreading forks on a forklift and strained back.
Employee was stepping out of the cab of a machine scaler. Employee slipped and hit shoulder and back on the tracks of the unit and then the mine floor. Employee did not start losing time from work until Monday, April 3, 2023.
Employee was hand scaling with a hand bar in a lift basket. The hand bar slipped and caused the employee to pinch the right little finger between the bar and the basket railing. Laceration required stitches to close wound. Placed on light duty for 10 days.
While striking a wrench to break loose a bolt on the final drive assembly of a LHD, the bolt broke free and pinched EE's left index finger between the wrench and another bolt, causing a laceration. Five (5) sutures were required to close the wound.
Employee was grinding a weld on a plate of steel. The grinder caught the plate and bounced up and cut employee's face under their face shield. Small cut on face required stitches.
Employee was trying to empty a wheel barrel of salt into a bucket of a frontend loader. Operator of frontend loader rolled the bucket of loader to be flat on the ground. Injured employee did not realize that their foot would be under the bucket. Bucket pinched foot, causing a contusion. Employee started losing time on 10/11/2022.

Employee was removing a fuel line. EE cut wrist on a holding clamp. Employee receives sutures for repair of injury. No lost time from work.
The Production Hoist motor would not operate. The motor had to be replaced for the hoist. The hoist became operational at 7:30 a.m. on March 1, 2022. The Production Shaft is the second way out of the mine.
Employee was attempting to plug coupling of undercutter trailing cable into a circuit center. While walking over to plug the cable in, employee stepped on the trailing cable and twisted EE's right ankle/knee and fell to the ground.
Employee bumped left knee when entering personnel carrier. Knee began to swell. Employee went to doctor on 2/17/2022. Employee began losing time from work on 2/17/2022.
Employee was turning to sit down on the wire truck. When EE turned, EE twisted knee and dislocated knee cap. Lost Time Injury.
Employee was exiting a Company Polaris Ranger. When exiting, employee twisted EE's back, causing a strain/sprain to lower back. Employee began losing time from work on 1/4/2022.
Employee was traveling to a work area in a Polaris ranger with their foot outside of the cab. Employee's foot hit the rib causing a broken ankle.
Employee was cutting cable ties with a utility knife. Knife slipped and cut and stabbed left hand. Employee received sutures for laceration.
Employee was filling oil in KB-09. Employee fell off of the maintenance truck while getting more oil, striking knees and left arm. Employee began losing time from work on July 2, 2021.
An employee was removing the spreader bar for the cables of the south skip of the Production shaft. The spreader bar slipped and struck another employee in the rib cage. Injured employee received a fracture of the left rib below the breast.
Employee was sliding a grating into a location. When sliding the grating, the grating slipped and pinched fingers on both hands. Lost tip of left little finger and had lacerations on the others.
LHD Unit Number 15 caught on fire. The mine was evacuated and no injuries were reported. Unit was left to burn out. Mine Rescue Teams entered the mine and made sure fire was out on the LHD. After verification that fire was out, the Teams explored the rest of the Mine, with authorization from MSHA.
Employee was climbing ladder to the second deck of the surge bin to grease the feeders. When climbing, employee felt pain in right arm bicep.
Employee was picking up hoses to clean the floor. While picking up the hoses strained their back.
Employee was tightening down bolts with a 4" ratchet on a new conveyor drive unit. Employee felt a strain in employee's back.
The brake on the service hoist became out of adjustment when the hoisting engineer was doing the hoisting checks. The brake was adjusted after trouble shooting and placed back in service.
Employee was loading 20 foot cable bolts onto a truck. When turning, employee felt something pop in lower back.
Employee was replacing light bulbs. Employee was inserting old bulb into cardboard casing, causing bulb to break. A piece of shattered bulb went into right thumb. Employee needed stitches.
