IE was bent over the drill head of roof bolter attempting to take suction hose off to get it unplugged. His knee bumped the joy stick causing the drill head assembly to move pinching his left hand (little and ring fingers) in a pinch point on the boom causing injuries to his fingers.

During the reaming of a bolt hole to allow a cable bolt to fit, the drill steel fell from the roof striking IE left middle finger.

IE was making splice, machine slid off belt rails pinching right hand between rails and belt splice machine. IE was wearing metacarpal gloves.

After drilling hole for roof bolt & putting up channel, realized hole needed enlarged to accept bolt. As he started larger drill steel thru channel into initial hole, had his hand on channel to assist getting steel thru hole in channel, when drill steel contacted channel causing his finger to be pinched between channel & roof causing laceration & fracture to right index finger.

A Roof Bolter Operator Trainee, while being trained, got his fingers caught between the drill steels as he was putting the drill steels together to drill a hole. Employee was wearing the proper metacarpal gloves at the time of the accident. Laceration to his finger required stitches.

IE was checking torque on bolter, he had gauge wrench in chuck and put pressure on it, it checked ok, when he went to release pressure, he pulled lever too far and spun wrench into his right wrist, hand and hip area pushing him against the bolter.

IE was installing outside bolt in #4 Entry of 1 Butt, when IE went to straighten the bolt, the bolt spun around hitting his left hand on the end of T-5 Channel resulting in a laceration of his left index finger.

Roof Bolter Operator - While attempting to remove a damaged strap by loosening the nut on a super twist roof bolt, the strap rotated suddenly around lacerating the left side of his face requiring three sutures.

While bolting in the cross cut R1-R2. After inserting the bolt into the chuck. The operator accidentally bumped the rotation lever. This action caught the glove on his right hand to be caught in the rotation. This caused a fractured left 5th proximal phalanx.

IE assisting in drilling & installing rib bolts using a Post Hole Drill. As IE was pushing 4' Torque Tension roof bolt into hole that he had just drilled, jack on drill that tightens against mine roof/floor to hold drill stable slipped out of position allowing Post Hole Drill to kick out pinching his hand between coal rib & drill assembly causing crush injuries to his left hand.

Employee was adjusting head on bolter when he instructed the RB Operator to lower the head. The operator lowered the head and employee got his finger tip pinched between the long & short bars .

Employee was installing 6'Roof Bolt, after pushing the first half of the bolt into the roof and he pushed on bolt to take bend out of bottom of bolt, he lost grip on the bolt allowing it to spring back striking him on upper lip causing a laceration requiring sutures.

Employee went to swing canopy out to install a bolt and did not realize it was against roof and it let go and it hit him on the left wrist.

While cutting sandrock bottom outby in #5 Entry to cut a ditch for water to run to a sump, a piece of sandrock flew up hitting employee in the right eye. The sandrock broke safety glasses of employee and cut his right eye.

Employee was removing a 4-foot drill steel from the roof, when the starter steel fell out of the roof striking his right pointer finger causing a laceration requiring sutures.

Injured releasing grease take-up to replace CBM Foot Shaft when the grease gun fitting he was using broke allowing pressurized grease to strike him in the face. Employee has been working his regular job, but had surgery on 9/15/2011 to remove imbedded grease from under the skin on his face. He has not missed any work as a result of the surgery and continues to do his regular job.

After scaling loose roof rock in previously bolted area belt - R1, a roof bolt that was previously tight to roof was now protruding away from the roof. As Injured pushed exposed roof bolt tight to the mine roof, his right hand, that he had resting on the drill head, got pinched between the drill head and the safety post canopy causing a laceration requiring stitches.

Tramming Joy CM downgrade when the miner began free-wheeling down the grade. The employee, positioned outby the miner, got his feet caught by the outby strands (loops) of the miner cable causing him to fall and be dragged along the bottom for a short distance.

Employee was holding a belt splicing machine and the machine shifted, causing his left thumb to be caught in the machine, resulting in a laceration requiring 5 stitches.

Employee stated he was backing drill steels out of a finished hole when the steels came apart and one of them flew back and struck him under the left eye causing a laceration.

Employee was operating continuous miner when something flew off the rotating cutting drums and struck him on the upper lip causing a laceration.

Employee putting 1st bolt in row(outside bolt). He started hole, noticed loose rock,let it fall,drilled hole,created a ledge 10"x10" when he put up a roof channel with bolt in hole, bolter head out of reach,added bolt wrench,he pushed up bolt channel hit offset created by rock that fell out & wrench slipped off bolt striking him in safety glasses, nose & forehead area.

Employee while roof bolting was pushing a bolt into the roof when the bolt sprung out of the wrench and struck him on this left wrist and hand. The employee was originally diagnosed as a wrist sprain. The employee continued to work his regular job until 02/23/2011 when he had an MRI diagnosing tendon damage. Employee had surgery on 02/24/2011 and is now off work.

Employee was drilling a hole when the drill steel stuck in the hole. He raised the head to reengage the steel and caught his right hand between the drill head and the roof causing injury to his thumb and index finger

While installing roof bolt, employee twisted his right knee. He has worked full duty until 1/18/2011 when he re-aggravated the injury. Employee is working modified duty effective 1/25/2011 per doctor's orders.

Employee was advancing lifeline on the R1 Entry adjacent to a Fletcher Roof Bolter, when the bolter was started and trammed forward running over his right foot and toes, causing a simple fracture to the 4th metatarsal bone in his right foot. Employee is working his regular occupation.

Employee was bolting in 4 West section and had his left hand on drill head, steels fell out of roof striking employee on left hand.

While bolting in the 4 West Section employee stopped drilling and lowered head with steels in roof to check suction, steel fell out and struck employee on left middle finger.

While roof bolting employee put left hand on drill head, steel fell out striking employee on left ring finger.

Employee was installing a roof bolt (position channel), when he looked up, the bolt fell and struck him in the mouth.

Employee was installing roof bolt and guiding it into the roof bolter drill chuck when he bumped the rotation lever to seat the bolt his glove caught onto the bolt wrapping his hand around the bolt causing a "mallet finger". Employee continued working regular job until 04/28/09 when he had minor surgery on the finger.

Employee was tightening roof bolt and his left glove got caught between bolt and steel, twisting fingers around steel

Was using hydraulic drill off of scoop, drill spun, caught finger between drill steel and jack.

Continuous miner was cleaning up the left side of a 30 foot cut through x-cut 4 to 5. Spinning miner heads threw an unknown object toward the operator, who was located to the right side of the continuous miner, back along the first pig. The operator saw an object flying toward him, tried to get out of the way, but the object struck just below his right eye and his nose.

Employee was working with another man roof bolting on a twin boom bolter. The co worker moved the machine over without warning and caught the injured man's left leg between the bolter head and rib.

Employee claims he was tramming bolter and put his right hand on top edge of canopy on cab while trying to see where he was going. Machine was getting stuck on material and he trammed machine while right hand was on canopy. Machine went against rib, catching right thumb between canopy and rib.

Employee was drilling a 14' hole for a 14' cable bolt. As he lowered the head, the drill steels stuck momentarily in the roof and then fell out of the roof pinching his left thumb between the column of drill steels and the top of the bolter head causing a laceration on each side of the thumb. Employee was wearing Meta-Carpal gloves.

Employee was positioning the plate on a cable bolt when he lost his balance and inadvertently pushed on the up lever pinching his right thumb between the plate and the roof.

After inserting the drill steel into the drill chuck, he pulled forcefully on the drill steel to align the drill head. The drill steel suddenly pulled out of the drill chuck pinching his left thumb between the end of the drill steel and the safety post. The employee was operating the operator side of the Fletcher bolter. The employee was wearing metacarpal gloves.

Employee was dropping the head of bolter and had his leg in the wrong place, head of bolter struck him on his right thigh.

Employee was installing a roof channel. The channel was sitting on the canopies. As the employee was installing the bolt, the channel slipped and ran down the bolt, striking the employee's left hand, lacerating the tip of his left thumb. Employee was wearing company meta-carpal gloves at the time of incident.

Employee was withdrawing drill steels from roof when the steels suddenly came loose falling out of the hole and striking employee on his left hip causing a small cut and bruise. The accident was mentioned to his foreman on 10/2/06, however medical treatment was not obtained until 10/6/06.

Employee was roof bolting and while adding a drill steel, he hit the up lever and caught his right thumb between the steels.

Employee was using hand drill to purge a blocked line with water (overhead). Coal and water was running down on employee. Drill caught and slapped employee on the left check on his face causing laceration. Drill handle became wet and slipped.

Employee was using an air wrench when the 1" airhose disconnected from the wrench and struck the employee on his left eyebrow causing a laceration requiring 3 stitches.

Injured was drilling a 1" hole when the drill steels stuck in the roof. As he attempted to line up the drill steel and the drill chuck using his right hand, he caught his fingers between the chuck and the roof causing lacerations that required stitches.

Drill still stuck in the roof and employee swung drill head and caught left hand between steels and canopy (fractured left hand).

Injured was installing an 8 foot bolt into the hole when he accidently bumped the rotation lever causing the bolt to suddenly bend and rotate causing him to hyper-extend his left arm. Originally diagnosed as a strain. New diagonosis is rotator cuff tear requiring surgery.

Employee was installing a point anchor bolt. When he put the top and bottom together the coupler was near the roof; he had his left hand on coupler and hit the up pressure lever, jamming his left index finger in the hole.

Employee was installing a 6' double lock bolt, when he tried to push bolt into roof he felt pain in mid back.
