He turned to tram the roof bolter forward, twisted back. He felt something pop in the middle of his back. Also had pain in neck area.
Bolter Operator was starting drill steel, and it bent out and hit him on his left wrist.
Employee was operating a roof bolter, when he got his middle finger on his left hand caught between the boom and the frame, resulting in a laceration that required several stitches to close.
Employee was operating the left side of a dual boom bolter when he got his left hand entangled with the drill steel and a wrench. This resulted in an amputation of his left hand and arm near his elbow. He was treated on site and then taken by ambulance to a local hospital. He had surgery and his arm was reattached. He was released from the hospital on 4-4-08.
While putting up a rope bolt about 6" from top, bolt started to bend. He tried to spin it the rest of the way up when the head of the bolt broke off & plate flew off the end of the bolt striking him in the face leaving a 2" cut over right eye.
Mixer clogged with cement and EE reach his hand around the guard and into the hopper to free clogg and encountered the auger.
