Employee was standing at the rear of a roof bolting machine when the bumper was lowered onto their boot.

Employee was using a grinder with a cutoff wheel, the grinder them jumped making contact with employee's left little finger. This contact caused a laceration that required suturing. The employee was wearing gloves at time of incident.

While working to repair dust vacuum hose on the roof bolting machine the drill boom was inadvertently raised pinching the injured employee's fingers between the top of the drill pot and the mine roof resulting in lacerations requiring stitches to two fingers.

Employee was operating a Continuous Miner in the No. 8 Right, when a piece of rock 9'x3'x6" fell from between the outside roof bolt and mine rib striking their right leg.

Employee was operating a mobile surface drill parallel to the edge of the drill bench. The mobile drill's right track trammed onto some spoil along the edge of the bench causing the drill to drop off the bench and turn onto its side.

Employee was struck by A piece of rock came that from the CM ripper head hitting the employee above the ear.

Employee was struck on the left forearm by a piece of rock that fell from the mine roof resulting in a laceration.

Employee was operating a hand drill and pinched the area between EE's left ring and little finger between the drill and a beam causing a laceration.

Employee was swinging the drill station over to change a piece of drill steel out and mashed the web of EE's hand between the canopy and the drill steel hanging down out of the hole.

When loading the truck the employee was operating, a rock landed in the bed shaking the truck injuring the employee.

Employee was struck in the head by small rock while trouble shooting a fouled conveyor on the CM. The employee was examined by EMT and transported to the ER where they determined they may possibly have a concussion. The employee was written off until follow up with family physician who requested the employee remain off work until 2/12/24 for monitoring.

The employee informed management on 12/7/23 that EE was seen by a physician on 12/6/23 for back pains from an injury that EE suspects happened on the job on 12/1/23. EE did not notify EE's supervisor of any injury until 12/7/2023 and was released to return to work with no restrictions on 12/7/23 and continued to work until 12/13/23 when EE was written off.

Employee was struck on the left knee by a rock while roof bolting.

Employee was tramming roof bolter and a stack of roof bolt plates slid off the deck and came in contact with EE's hand causing injury.

Employee was dusting section when the duster fell out of the scoop bucket and made contact with employee's foot causing injury.

Employee was installing roof bolts in the number two entry when a rock fell from the mine roof striking the employee on the head.

Employee cut a tree approximately 5" in diameter. When tree fell it sprang back on top of employees foot. Employee kept working. Employee received medical treatment on 5-25-20 and a fracture was found.

Employee was cutting wood with an electric saw and a piece of wood went into employee's eye (left eye).

Employee stated that EE was bolting top. The bolt bent and EE went to realign bolt to drill pot, bumped rotation causing the bolt to strike employee in hand and arm.

Employee cut a tree. As the tree was falling it struck a broken tree causing it to flip back striking the employee in the leg.

Employee was in the process of roof bolting when applied up pressure and caused the drill steel to bend pinching left small finger between steel and guide.

Cable reel was engaged on roof bolter and when began taking up trailing cable slack the cable struck employee on the left leg causing muscle contusion.

Employee was tramming bolter across section and T-Channel (straps) slid into the operators compartment and cut hand in several locations.

Employee was pulling hose & wand turned & pressure from water cut employee's face.
