Employee was removing a top roller frame from the Raw Coal Belt No. 3 when they lost grip and their fingers were caught between the structure and roller frame, resulting in a laceration.

Miner was lifting hanging rods and putting them in a basket to go into the shaft during a lining project. The miner bent down and was twisting while lifting which resulted in a minor strained muscle in EE's upper back. The miner worked through the shift went home and the next morning decided EE needed to be seen at the doctors office.

Employee was loading 16' straps onto the scoop, when the stack turned over and smashed EE's finger between the straps and the bucket of the scoop.

A belt examiner was found alone in the mine and unresponsive. Attempts to resuscitate the employee were unsuccessful and the employee was pronounced dead upon arrival on the surface. It's our position this death was not mine related and we reserve the right to retract this filing upon MSHA chargeable ruling.

While repairing a splice on the #2 belt, the belt clamp unexpectedly came loose and struck the employee in the left forearm, resulting in a laceration.

Employee was operating a rubber-tired personnel carrier when EE entered a crosscut and inadvertently drove over a belt scraper that was lying on the mine floor. As the vehicle passed over the scraper, it cantilevered upward and struck the employee on the left side of EE's face and shoulder, resulting in injury.

EE was operating the continuous miner on the 001 MMU when a coal rib rolled out striking EE on the right side. The rib measured 5'x4'x4" of loose unconsolidated material. EE was transported to LRMC where EE was fully released. EE followed up with an orthopedist, who wrote EE off work.

Employee was working in the No. 5 Right Entry when EE tripped over a lump of coal and fell, causing EE's right hand to flex during the fall. The employee continued working following the incident until EE was removed from duty by a physician on 6/10/26 for a fracture.

On 5-14-2026, the employee lost EE's footing and fell while working on a belt tail piece. The employee received a laceration to EE's head which required one suture to close.

A non-injury roof fall occurred in the #5 entry on the 2nd East Mains at spad #258. The fall was approximately 80 feet long, 20 feet wide and up to 50 inches thick.

The employee was lifting a rub rail into position on a continuous mining machine when EE experienced pain in EE's groin area. The employee continued working following the incident and remained at work until being removed from duty by a physician on 6/1/2026.

A non- injury unplanned roof fall occurred on the old worked out 1st North Submains return beginning at SS 3897 and extended for approx. 150 feet. The fall was approx. 20' wide and 2 to 5 feet thick. The fall was in the travelable return entry used by the weekly examiner.

Employee was standing in a crosscut, away from where a group was sheaving the belt take up back, when the sheaving device broke free. EE was struck with mud, rock, and debris causing corneal abrasions and a hairline fracture in orbital. EE was prescribed medicated eye drops and released.

EE was removing scrap metal from a muddy area when EE felt a pain in EE's back. EE visited the hospital on 2-24-26 and received a full release. EE worked until 2-26-26 when EE followed up with a PCP and was given another full release. EE went back to PCP on 3-5-26 when EE was taken out of full-duty work

Employee was positioning the draw head on the motor to line up with a flat car when EE felt a pop in EE's bicep. Employee finished EE's shift and was examined by a physician and fully released to return to work. The employee later had surgery on EE's bicep on 5/28/26 and written off work until further notice.

Employee drilled a test hole and was removing drill steel from the top when a piece became lodged. As employee was trying to retrieve the stuck drill steel, it fell out of the hole and struck them on the left pinky, causing a laceration.

Employee was removing rock from the on top of the continuous miner using a slate bar. The rock slid over and struck EE in the arm causing a laceration.

Employee was installing a roof bolt just inby spad no. 6978 on 1A section, when a rib rolled striking EE in the right lower leg, fracturing EE's Fibula.

EE was lifting a timber and felt a pain in EE's groin area

A loaded coal truck collided with an empty coal truck while traveling in opposite directions. The operator of loaded coal truck experienced soreness and was written off work.

The employee was flipping track rails over to begin installation, when a finger became caught between two rails, resulting in a laceration to right middle finger.

Employee was mining in the number 7 right crosscut located on the #2 section when a methane ignition occurred resulting in burns to the employee's face, hands and arms.

An Employee was assisting in replacing the cutter motor shield on the right side of the continuous miner when their left thumb was caught between the miner frame and the shield, causing a laceration that required four sutures.

During a section move, employee was conducting housekeeping and picking up hinge pins. When employee was on their way to place the pin on the power center, they accidentally tripped on a hinge pin and got their hands caught between the hinge pins and power center causing injury.

Employee was helping remove a broken torque shaft on the CM machine and as it was being pulled out by another employee the broke end struck EE's left hand resulting in a laceration.

The employee was setting standing support when EE turned EE's back on a standing unwedged timber to grab EE's hammer. The unwedged timber fell, striking EE in the head.

An unplanned roof fall at or above the anchorage zone occurred on the active working no. 3 section, 2 breaks inby spad no. 6687.

Employee was conducting maintenance when they stood up from a knelled position and strained right knee. Employee continued to work until 10/23/25 when they sought medical treatment and took out of work.

Employee was using a railroad jack to reposition a beam under a belt head when the bar slipped out of their hand and struck them just under the lip.

An 80 year old security guard was found deceased in the Guard Shack at the entrance to the mine property. The victim was found at 11:06 PM and based on written security logs, the victim died at some point after 8:20 PM when EE made EE's last written log entry.

EE was driving a rock truck out of the pit area when the load shifted while traveling uphill. The front wheels lifted off the ground and abruptly came back down once the load was dumped out of the bed, causing the employee pain in EE's back. The injury occurred on August 11, but the employee continued working until August 19, when EE visited the doctor and was written off work.

Section floor was muddy and wet

On 8/2/25 employee picked up a small air compressor and felt a pain in groin/stomach area. Employee continued to work and sought medical treatment on 8/8/25 and diagnosed with a potential hernia. EE continues to work and has fulfilled all their duties as a dispatcher. The employee is scheduled to see a specialist for complete diagnosis and applicable treatment.

Employee lifted two full oil cans to load onto a ride and felt a pain in their back.

On 7/19/25 the injured employee twisted EE's knee while rising to EE's feet from a kneeled position but finished EE's shift. Employee missed work on 7/21/25 to be examined by a physician and returned to work on 7/22/25 with no restrictions and continued to work until 7/25/25, when EE again missed work to be evaluated by a physician. The employee received a full release for 7/28/25.

Employee had drilled a hole to install a roof bolt when the drill steel became stuck. As they turned to get another piece of steel to retrieve the stuck steel, the stuck steel fell from the hole, catching their thumb between the steel and the drill pot.

Employee was installing a seat in the 773 truck when EE twisted and felt something pop in EE's shoulder.

Employee was hanging line curtain on the #2 section in the #4 entry when a piece of coal rib gave loose striking them in the lower back and ankle.

Employee was standing behind the continuous miner when EE was struck by a piece of rock that fell from between bolts. The rock measured 23"x15"x1".

Employee was stacking supplies on the roof bolting machine when a piece of draw rock fell from the top and struck them on the thigh. The employee was injured on 6/11/2025 and was treated, released, and returned to work until 6/21/2025, when they revisited the hospital and was written off work.

Employee was in the #4 entry when a coal rib gave loose striking them on the right ankle. Employee finished the shift and sought medical attention on 6/12/25 for a fracture ankle.

Employee was standing at the rear of a roof bolting machine when the bumper was lowered onto their boot.

Employee was rolling up line curtain and contacted a curtain hanger with EE's right middle finger causing a laceration.

Employee opened an airlock door, and it blew shut, catching EE's hand between the doors.

Employee was climbing down steps from the launch area of HWM No. 1, when EE contacted the ground rolling EE's right ankle resulting in a sprain.

On 5/5/25 the employee fell and hit their left thumb on a jersey barrier resulting in a strain/sprain. Employee continued to work and sought medical treatment on 5/8/25 when they were taken off work and returned back to work on 5/12/25.

Employee was kneeled down and when EE went to stand up felt a pop in EE's knee.
