Mostrando los 18Employee came to work on 3/16 with right eye irritation, went to urgent care and was diagnosed with acute conjunctivitis. Irritation worsened; employee sought additional care and had a foreign body removed from EE's cornea on 3/18/23. Employee was given prescriptions and returned to work. Employee believes grinding activities on 3/15 caused the foreign body to enter EE's eye.
Holding a bolt head while another guy was using an impact gun to loosen the nut. The bolt shifted down enough to come loose and spun in my hand cutting though my leather glove and cutting my hand.
Plant operator was using Allied breaker to break large rock at Jaw Crusher. EE did not notice a bolt head at the top of the hammer was in contact with a part of the shielding around the jaw opening. As EE pushed the hammer head down upon the rock, the bolt head broke the plate weld loose on one end and the steel sprung against operator's right arm.
While making a splice on Main belt, employee got finger lacerated while holding up on the Flexco bolt while nut was turned with an electric drill/driver. Employee was wearing heavy leather winter type gloves; removed glove and found finger lacerated.
Employee was adjusting the tightness setting on the Nordberg Portable crusher. He was reaching for a wrench with his left hand while resting his right on the hood. The hydraulic pressure released, pinching the tip of his right middle finger between lower adjusting nut and washer.
2 workers were welding, cutting and grinding on the screen deck to the wash tower and at lunch one worker's eyes started watering and burning.
EE was using a torch to help install gates on the pit feeder. He was holding the torch with left hand when the hoses moved and caused the torch head to swing around forcing the flame over the back of his right hand.
Employee was utilizing a angle grinder to remove built up slag from the screen deck. While grinding a piece of metal recocheted off the work area and got under the employee's safety glasses and face shield. Employee went to ER 3 days later and was given prescription medicine for pain.
Employee was cutting with a torch when hot slag got behind safety glasses and burned eye.
INJ WAS CUTTING STEEL W/TORCHES. EE WAS HOLDING THE TORCHES CLOSE TO HIP AREA, WHEN HE HEARD A POP. HE NOTICED TORCHES WENT OUT & FELT PAIN IN LEFT THIGH AREA. HE TOOK OFF HIS CARHART BIBS & NOTICED HE BURNED HIS LEFT THIGH AREA. BIBS HAD AN 1/8"HOLE IN THEM BUT NO SIGN OF BURN ON OUTSIDE. PANTS, LONGJOHNS, &LEG HAD BURN THE CIRCUMSTANCE OF SOFTBALL, LEG THIGH SUFFERED A 3RD DEGRE
EE WAS CUTTING A BOLT OFF A CHAIN WITH A TORCH. HE THEN GRABBED THE HOT CHAIN WITH HIS LEFT HANDHE WAS NOT USING GLOVES (WHICH ARE PROVIDED). HESUFFERED BURNS TO THE PALM & FINGERS OF HIS HAND
EE TOOK IT UPON HISSELF WITH NO TRAINING TO WELDON HAND RAIL; LIFTING UP WELDING HELMET TO SEE AND STARTED TO WELD WITH HELMUT UP. GETTING FLASHBURN TO EYES. EE ADMITTED TO HIS MISTAKE. RESOLV ED WITH EYE DROPS. OUTCOME GOOD NO PROBLEM
WHILE USING A CUTTING TORCH, EE GOT A HOT SPARK (HOT METAL) IN LEFT EAR. EE HAD PROPER FACE SHIELD & SHIRT, HOWEVER, TO GET TO A CERTAIN ANGLE EE LAID HIS HEAD OVER TO THE RIGHT.
WELDING FLASH TO EYES
GRINDING PIECE OF STEEL,IT FELL OFF BENCH AND CAUGHT CORD.PULLED GRINDER OUT OF HAND,WHEEL CAME ACROSS BACK OF HAND.
EMPLOYEE STATES HE WAS WELDING ON HAMMERS AT WELDING TABLE & HE TURNED AROUND & WHEN HE T URNED HE BUMPED THE TABLE CAUSING HAMMER TO FALL
MAN WAS ON ROOF OF DREDGE.WE HAD ATTACHED CABLES TO IMPELLER AND PUT TENSION ON CABLES.MAN KNELT DOWN AND LOOKED OVER EDGE OF ROOF.JUST AS HE DID SO THE IMPELLER CAME LOOSE FROM SHAFT.THE LOSS OF RESISTANCE CAUSED CABLES AND BALL WEIGHT TO STRIKE MAN ON THE SIDE OF THE HEAD.
