Welding on jaw of crusher, after shift realized scratching in eye from foreign object due to welding.

Operator set down a grinder while cutting disk was still spinning, disk contacted the conveyor belt and jumped to the side. The spinning disk contacted the back of the operators hand and caused a laceration to the hand. Laceration required stiches to close.

Flash burn to eyes while doing maintenance.

Using a grinder to remove a bracket and at some point got a piece of steel in their eye.

Using air nozzle to blow away debris, small piece got into left eye.

Operator was removing cutting edge from loader, holding bolt in place with left hand, bolt spun while using impact wrench, cutting operators left thumb.

Jarring motion of digging hard rock over the course of the day caused lower back pain.

EE alleges that while operating a backhoe scaling quarry highwall, his lower back began to hurt after being jarred when a large rock hit the bucket.

While replacing bracket using the air gun to spin off the bolts on top. Right was on the bottom holding the bolt up when the air gun jammed on, causing the bolt to spin resulting in a laceration on his finger.

EE WAS USING AN AIR HAMMER TO REMOVE MUD & CLAY OUT OF ROTO-LIFT ON THE ROLL PLANT WHEN HIS SHOULDER BECAME SORE-PAIN INCREASED OVERTIME, SO HE IS SEEKING TREATMENT. SOUGHT MEDICAL TREATMENT IN MAY.

THE WELDER PLACED THE HAND GRINDER THAT HE HAD BEEN USING ON THE FENDER OF THE TRAILER THAT HE WAS WORKING ON. APPARENTLY SOMETHING CAME OFF THE GRINDING WHEEL AND STRUCK HIS EYE FROM BEHIND T HE GLASSES.

EE WAS LOOKING DOWN IN BREAKER WHEN A ROCK FLEW OUT AND STRUCK HIS WRIST.

EMPLOYEE WAS SLOWLY MOVING DRILL INTO POSITION WHEN EITHER THE TRACK SLIPPED OFF ROCK OR TEETERED OFF ROCK. THE DRILL OVERTURNED AS A RESULT.

WHILE TORCHING HIS HAND CROSSED THE FLAME OF THE TORCH CAUSING BURN.

EE GOT DUST IN HIS EYES WHILE AIR ARCING.
