Worker was placing bag on fill spout and put their finger under the bag clamp. The clamp crushed the tip of their right ring finger causing an open fracture of the tuft of distal phalanx of the finger.

Operator was hand adding concentrate for the batching operation when air from fan blew some dust or dirt in left eye causing irritation.
Operator had just adjusted packer. Needed to adjust again, instead of turning off machine and completing the adjustment, operator tried to adjust machine with machine still on. Operator should have followed procedure. When adjusting machine from front side, operator's head hit switch that made cylinder move and caught operator's arm. Only bruise. X-Rays negative.
Operator was running palletizer for the bagging operation. Bag started to fall from the feed conveyor and operator reached to catch bag. In the process the operator cut right elbow.
EE said EE strained left shoulder on Friday while operating the main line palletizer. EE did not know what time it occurred. EE did not report the injury until Monday 6-18-18
Miner was running the main line palletizer and felt a pain in EE's left shoulder. The miner could not reach above EE's head without feeling pain in shoulder.
Maintenance employee failed to follow LOTO procedure while working on pinch tube mechanism. Pinch tube activated and crushed employee's left thumb causing loss of thumb tip at first joint.
Employee was using a chain saw on county road that leads into the mine site, to remove fallen tree limbs onto the road and fence. Employee stated later that afternoon that his back had been strained.
Tried to remove piece of plastic from bag sealer while still running. Lost tip of right index finger.
Finger got caught in pulley belt of packer.
While cleaning bag house air lock with air chisel, he slipped causing air lock paddle to rotate pinching left index and middle fingers between paddle and housing thus causing laceration to left index and left middle fingers.
Busted bag stuck between conveyors. Employee tried to free it - hand got caught. Took off tips of middle and rings fingers, skin across top of hand and two fingers was pulled off. Tips reattached and skin graft to repair hand.
