EE was replacing cables on a bridge crane and was placing the cable across from the notch to the smooth section of the auxiliary hoist. EE directed the operator to come up on the hoist. EE lost balance and caught hand under cable

Mostrando los 34Employee was drilling holes in steel plate with a 1/2" shank drill. The bit got hung up and twisted the drill out of hand. The cord for the drill became wrapped around the trigger causing the drill to continue to twist and it caught right pinky finger.
Injured employee was flying a drone for end of month inventory activities. While landing the drone was blown off course and employee tried to catch it by one of the landing legs when employee made contact with one of the propellers causing cuts to two fingers on employee's right hand that required butterfly stitches to close.
Employee (was wearing safety glasses and face shield) was grinding metal when shard found its way into the employees right eye. Weather conditions was very windy and dusty in the afternoon that could have possibly aided to the accident.
EE was cutting a bearing from a shaft using a cutoff blade on a 4.5in grinder and the blade bucked and cut the EE's thumb. Requiring stitches.
Employee reported to work Monday morning with numbness in right hand, attributed to heavy welding and bolt torquing the previous week. He continued to work his normal job until 5/15/15, when he was transferred to a new position to accommodate healing. This created a restricted duty case and the reporting.
The front down rigger of a boom truck fell onto a table top while victim was holding it. This caused a crushing injury to the tip of his right pinkie finger. There is a small fracture at the tip of the finger and swelling. A splint was placed on the finger by a doctor and no further treatment is anticipated other than follow up visits.
Employee (mobile equipment mechanic) was loosening bolts on a loader wear plate. He used his left hand to push the bolt into the recessed sleeve, and his right hand on an impact wrench to loosen the bolt. When he activated the wrench, the bolt spun, cutting his thumb.
Employee was descaling material with an air lance. She bent down and a piece of coating came out of the port. The piece bounced off her jacket and went under her hood landing on her left eye lid.
While cutting with a hand held grinder the blade bound up and broke. This caused the grinder to flip backward cutting the employee above the right thumb.
Employee was utilizing a grinder on a piece of metal, employee felt a dust particle entered his eye and proceeded to remove his safety glasses and rub his eyes.
While cleaning the packing machine with an air lance, the employee brushed his right hand against the underside of the bag saddle causing a subdermal laceration to the dorsal surface of the digitus tertius intermediate section of the right phalange whereby requiring sutures. He returned to work with no restrictions.
Worker was cutting a 3"X3"X1/4" angle iron using a 4 1/2" angle grinder with a 1/16" cutting disc attached. During the cutting process, the disc became bound up and the worker lost his grip on the grinder. The grinder twisted and the cutting disc came in contact with his right thumb, resulting in a laceration.
Employee was preparing to sut weld with cutting wheel on grinder. When he turned grinder on wheel pulled glove & finger in thus cutting finger needing 9 stitches
Air impact gun spun around and hit EE in the arm; resulting in sutures.
After jack hammering, EE felt a soreness in his back. EE did not seek medical advise until 10/23/2009. On 12/03/09, the treating physician made an order for physical therapy.
Doing demo to a chute - cut a piece loose and it flipped the wrong way and hit him in the hard hat.
Employee was using a rivetbuster, when the tooth got caught in the material. As he tried to remove it, the rivetbuster lunged forward catching his right ring finger and pinky finger between the rivet buster and material causing a 1 cm laceration to his 5th (pinky) finger.
Miner was cutting on feed end of ball finish mill with a torch. A flamable material flashed from cutting slot. The material was unknown. Miner received first degree burn and 2 small 2nd degee burns of left side of face.
Employee was operating a roller during construction of a mine haul road. He drove up an incline to turn around ans as he was backing up lost control of the roller, drove over the edge of the road under construciton and down approximately 150 of steep hillside., coming to rest upright at the bottom of the slope.
EE was "sucked" by super sucker hose in crotch. Has suction bruise and mild back strain.
Water spray leaked into hot material. EE tried to open door. Hot material flushed into employee's overalls. Hot material burned his back(blisters). Employee had a poor position to open the door, contributing to injury.
EE energized a powered hacksaw with his finger against drive wheel. EE tested positive for marijuana; suspended; further investigation for level of drug; return TBA.
While air-lancing feed pipe, the feed shot backed under the employee's hood.
Cut left index finger. Whild holding a liner bolt head with left hand and using an air gun with my right, the bold head spun cutting my finger.
WHILE MANUALLY ACTIVATING AN AIR CANNON, A CARDOX PORT CAME OFF AND HOT KILN FEED CONTACTEDEE'S RIGHT HAND.

EE WAS TROUBLESHOOTING PLUGGED DUST COLLECTOR. HE THEN OPENED THE INSPECTION DOOR AND STUCK HIS HAND IN WHILE ROTARY AIRLOCK WAS STILL RUNNING. ROTARY AIRLOCK CONNECTED WITH THE TIP OF HIS RIG HT MIDDLE FINGER CAUSING AN AVULSION.
2 EES WERE AIR LANCING HOPPER AT C8 VIBRATING SCREW; BECAUSE THEY WERE USING SAME AIR OUTLET THE AIR PRESSURE WAS REDUCED AND INJURED EE PRYED AND DUG AT MATERIAL IN HOPPER. THIS CAUSED HIM TO SEVERELY STRAIN HIS LEFT SHOULDER.
EE WORKS AS PLANT OPERATOR, WHILE DRILLING A CARDOX PORT, HE WAS BENDING OVER TO PORTHOLE, WHICH IS LOW. THE BIT WAS TOO LONG MAKING IT HARD TO KEEP DRILL LEVEL AND STRAIGHT. USING HEAVY DRILL WHILE REMOVING BUILDUP IN TOWER. EE CLAIMS STRAIN OF LOWER BACK.
EE WAS WORKING INSIDE THE KILN-GRINDING THE METAL SURFACE TO PREPARE IT FOR REFRACTORS. HE WAS WEARING SAFETY GLASSES BUT NOT GOGGLES OR FACE SHIELD AS REQUIRED. WHILE GRINDING OVERHEAD HE FEL T MATERIAL IN HIS RT EYE. HE FINISHED HIS SHIFT & WENT HOME. BY NEXT MORNING EYE WAS VERY IRRITATED & HE WAS ESCORTED TO MD. DIAGNOSIS: TINY PIECE OF METAL EMBEDDED IN EYE & HAD TO BE REMOVED
EE WAS WELDING A GUSSETT ON THE DOOR TO THE WHOLE TIRE CHUTE. HE WAS WORKING OFF A TEMPORARY PLATFORM,WELDING OVERHEAD & A PIECE OF HOT SLAG DROPPED INTO HIS LEFT BOOT. THE BOOT WAS NOT LACED UP ALL THE WAY ALLOWING AN OPENING FOR MATERIAL TO DROP INTO. HE ALSO DID NOT REPORT THIS TIL 5 DAYS LATER. DIAGNOSED W/CELLULITIS LEFT LEG. BURN WAS @ ANKLE BONE.
EE WAS TRYING TO TIGHTEN UP LOOSE RAPS OF CABLE ON THE HOLDING LINE DRUM. AS THE CRANE OPERATOR WAS UNWRAPPING CABLE IT CAUGHT IN A NOTCH WORN IN THE BASE. HE KICKED IT TO FREE IT UP & INADVER TENTLY PLACED HIS HAND UNDER THE LOOSE CABLE. WHEN FREE IT SLAPPED DOWN ON HIS LEFT HAND. HE RECEIVED CUT AND SLIGHT FRACTURE TO SMALL AND MIDDLE FINGERS. LACERATION TO RING FINGER NEEDING SU
EMPLOYEE BURNED THE HEEL OF HIS RIGHT HAND WITH A TORCH, NO INITIAL TREATMENT, WENT TO THE DOCTOR ON 2-17-00 TO HAVE IT CHECKED. PRESCRIPTION FOR ANTIBIOTIC. CONTINUED HIS REGULAR WORK DUTIE S.
