Redding Plant Metal/Non-Metal

Employee was attempting to hang hoses onto wall racks when the base of ladder slipped out from under EE. This resulted in employee falling from a height of 12' causing a broken femur and broken hand all on the left side of body. EMS was called to transport to hospital.

Contractor was painting an electrical box on 3rd floor of F Area. EE needed a paint brush, so EE entered the elevator going down. Contractor became lodged between floors, 911 was called and tech for elevator was called also. Contractor was retrieved from elevator without injury.

Employee was entrapped in elevator for over 30 minutes. Elevator blew a main fuse.

The incident occurred when a blown fuse on the tower elevator failed trapping a contractor between the 5th floor and 10th floor.

EE was cleaning the water trough on B Mill cooler by hand. It appears that a sliver of metal entered through glove into EE's right Pinkey finger.

Weather was good, working at heights. Employee slipped causing injury. All appropriate PPE was being utilized when incident occurred.

Employee was jackhammering around pan conveyor from EE's knees when EE rotated torso EE felt a strain in mid-back

Employee after shutting down compressor on lube truck was exiting off the back of truck when EE stepped onto the ground that appeared to have material on the ground causing EE to roll left ankle.

Employee was exiting off the locomotive steps EE planted right foot onto ground and felt a pop in right hip. Employee still had 3 points of contact including holding onto hand rail. Employee did not report injury / incident until 12/28/2023

Employee was walking in rock storage first floor. The employee slipped and fell onto EE's left knee. The first floor at the time was wet causing a slippery surface.

Transport elevator had a faulty relay switch. Tech replaced relay and retrieved elevator.

Incident occurred while employee was using rotary hammer from man-basket to remove build up on crane way wall and pipes. Employee was hammering build up on pipes when it became lodged in build up. Employee let off the trigger was a few seconds then engaged trigger to try and pull out hammer. While pulling on hammer employee felt pull/ strain in middle/ center of back.

EE reported was cleaning the diverter gate that allows the raw mill feed to go into B1 or B2. EE was chipping away the material in order to replace an air hose because EE heard a leak. EE had the air turned off to the valve and while EE was working on it, the valve actuated and smashed EE's finger.

Employee was shoveling material from under belt while Bobcat operator was moving in and out of building. The employee had positioned self by a wall and behind piece of equipment. As equipment was turning, a rock dislodge from under tire and struck employee below knee.

The production crew on shift was clearing a pre-heater tower plug in the feed pipe and stage 4. EE inserted Cardox tube into port and while trying to set the depth of the Cardox tube they felt pain in lower back on the left side. EE initially decided on self care however requested evaluation on 12/20/21 and was released to work with restrictions.

EE entered a passenger elevator, closed scissors gate, the exterior door did not fully close, EE reached through scissors gate to secure exterior door and smashed right thumb between door and frame.

On Sept 15, 2021 EE preforming electrician duties was pulling cable through conduit, tweaked EE's right shoulder while wiring a speed switch. EE refused medical attention, incident documented. On Oct 20, 2021, EE requesting a medical evaluation of the shoulder. Medical evaluation dictated restrictions, returned to work immediately.

The mine operator was made aware of a STS shift in a miner's hearing. The miner has been in the hearing conservation program and effectively uses hearing protection during performance of EE's duties at the mine.

Employee removed a Cardox port cap on a fluidized silo without authorization, material started flowing from the Cardox port. Employee tried to replace dislodged cap and received dust in the right eye in the process.

The injured employee was closing the door on a rail car, the door to the car was malfunctioning, in order to close the door the employee laid in the prone position on their back and extended their legs out to close the door, once secured the employee removed one leg to close the latch and felt discomfort in lower back. The employee requested medical attention July 9, 2020.

The production supervisor was assisting repairman free a transport valve stuck in the closed position. When the valve freed, the production supervisor lost balance causing them to fall awkwardly on right ankle. The fall caused a contusion injury to right lower leg and ankle.

Laborer employee was making a pour on a cooler tube, the employee was carrying two five gallon buckets, one in each hand of castable material that were half full from the mixer to the cooler tube. While the employee was carrying the material they adjusted the buckets when attempting to stage the material at the cooler tube causing a lower back strain.

Diabetic shock, was feeling bad, ate some candy then went into convulsions.

EE slipped on the walkway at the bottom of the stairs due to an intense rain that had just occurred at the plant. EE did not request medical treatment at the time of the incident. The EE was under first aid care from the time of the incident until physical therapy was prescribed due to lingering shoulder pain. The first physical therapy session was March 6, 2019

Employee was hooking up air to the trailer and hand slipped and finger bent backwards.

EE cutting a section of air hose with a utility knife and cut L index finger requiring 3 sutures

EEs were traveling in the elevator from the third floor to the ground floor of the Mill building when the cable failed. There were no EEs injuries

EE was walking up stairs in shop and heard a pop and felt a tear in EE's left knee

Stepped from a rail car and felt a twinge in R knee

EE was replacing cables on a bridge crane and was placing the cable across from the notch to the smooth section of the auxiliary hoist. EE directed the operator to come up on the hoist. EE lost balance and caught hand under cable

REMOVING BAR STOCK FROM STORAGE RACK AND FAR END OF BAR SLIPPED FROM THE RACK AND FELL TO THE GROUND CAUSING RAPID MOTION BEFORE EMPLOYEE COULD RELEASE THEIR HOLD

Employee was rolling scrap tires from one side of a cross-dock to the other. Employee reports feeling a sharp pain in left arm, shoulder, and ribs. Dr. reports contusion of left wall of thorax.

Employee has a strain of the flexor muscle, fascia and tendon at forearm level of right arm. EE's job entails loading and unloading scrap tires from trailers on a cross deck. EE is unsure whether the injury resulted from EE's job or non-work related actives (bucking hay off site), or combination of both. EE's duties have been modified temporarily according to doctors orders.

Employees stepped half on and half off of a 3 inch curb and twisted left foot.

Employee was trimming a piece of conveyor belting with a utility knife. While cutting the belting employee contacted left index finger.

Kiln feed popped during the cleaning process material splashed up under protective hood. A small amount of material contacted miner's eye

EE was cutting a bearing from a shaft using a cutoff blade on a 4.5in grinder and the blade bucked and cut the EE's thumb. Requiring stitches.

EE states that on an unknown day in January 2016, EE pulling on a tire that was "laced" with other tires. EE described a pop of crack sound coming from lower back, and subsequent back pain that extends down EE's left leg. A doctor has excused EE from work until 6/2/16. EE reported the injury on 4/26/16.

EE was inventorying idlers and stepped over one. His foot slipped and he hyperextended his knee.

EE was working in the shop, cleaning up scrap metal from the ground, next to a loader when the ee turned and felt a pop in his knee.

The ee in question was assisting a coworker in releasing a tire trailer from the fifth wheel coupling on a tractor. The employee managed to get the pin to release and instructed his coworker to move the tractor forward. The EE in question was not aware of his body positioning, had his foot in the front of the tractor's rear tires, resulting in his foot getting run over.

A fuel line at the kiln burner failed catastrophically spilling MDF on the ground. This fuel then caught fire due to the radiant heat in the area, and was not able to be extinguished with in 30 minutes. No injuries or illnesses to report.

Employee stepped on a rock walking down the East ramp of the rock dome causing him to slip and strain his left knee.

Employee was cleaning up in the basement of the pack house and slipped on a thin layer of hydrated cement that he was in the process of cleaning up resulting in a sprain to his left knee.

Employee was walking along the roadway on the East side of the switch yard and stepped on the edge of the concrete where it meets the dirt causing him to roll his ankle which resulted in a knee strain.

EE was heading to trailer for lunch when he saw co-workers hung up on beam while making pick w/ crane. He walked into deep pan drag chain room crossing danger tape explaining hole in floor. A 4' x 4' section of grating was taken out to get equip in. When walking in eyes were focused at opening in wall where crane was flying equip in & never saw opening in floor & fell 8' to ground

The ee was using a sledge hammer to clear a bin. On his backswing the employee lost his grip with his right hand, causing the weight of the hammer to shift, straining his left shoulder.

Employee was exiting the preheat blend dome and stepped on uneven ground, twisting his knee.

An employee with 1 year and 26 weeks of mining experience was working to clear chunks from a weigh feeder, using a bar. The employee's grip on the bar slipped, and the ring finger on the right hand was pinched between the bar and the guard for the weigh feeder, creating a small laceration on underside of the second metacarpal joint.

The employee in question was carrying tools down a staircase to stow them away, when he slipped and fell forward when he reached the bottom stair, striking his knee to ground, at which point his hand was struck by a jack hammer he was carrying, injuring his left hand.

The employee in question was working to weld a plate onto an overhead location. The employee had pre-positioned the plate against a ladder that was beside the ladder he was to ascend, the employee then reached down to grab the plate, and the plate began to slide, and was unable to catch the plate to stabilize it, causing the plate to strike the employee's left foot.

While the miner was pulling cable through becket, it slipped and hit him in the lip, requiring stitches.

Employee was off loading tires from a trailer in the cross dock area. He pulled a tire from its lacing. It swung around and struck him in the elbow.

The Employee had placed his hand on the end of the dolly while aligning it up to the hitch of the truck, and pinched his finger between the dolly tongue and truck hitch. The pinch resulted in an open wound to the small finger of the employee's left hand requiring sutures. Employee returned to full duty after treatment.

The elevator door did not close fully due to material build-up in the track; this prevented the door switch to make contact causing the doors to stay closed. The Cal. licensed elevator technician cycled the power allowing the doors to open so that the employee could walk out of the elevator; the technician then cleaned the track and switches. Contacted MSHA and they laughed.

During the process of loading tires from one trailer to another, employee was struck in the leg by a rolling tire, as he was crossing its path. Employee sustained a hyperextension sprain/contusion to his left knee.

Employee was using his leg to close the door on an empty coal car. He had pushed the door so that the latch was caught on the '1st' notch of the lock. As a second employee was closing a door on the other side, the vibration caused the first door to pop open and hit the employee's left shin causing a laceration.

Employee was lacing tires into a trailer as he turned to pick up another tire to lace into the trailer he was hit be a rolling tire on the side of his leg resulting in a sprain in his knee.

On 4/24/2013, the employee went to move a barrel; the barrel had material in it he did not know of. He felt a slight twinge in his shoulder, but did not think much of it. On 8/22, a doctor placed him on restricted duty due to found tissue scarring in his shoulder.

The miner is a garage mechanic that has noticed his hands cracking towards the end of the work week. He was sent to the clinic which diagnosed him with occupational dermatitis, and was given modified duty.

While the EE was setting up a ladder to assist a fellow employee who was working on a level above, he looked up and the other employee brushed some dust free that fell onto his safety glasses. When he moved his head, a piece of dust made it past the edge of the safety glasses and into his left eye. The clinic then issued the EE prescription eye drops as a precautionary measure.

While coming down the spiral staircase, a black cat ran in front of the EE startling him where he missed his footing on the third step, fell forward, and jammed his 5th metacarpal causing a spiral fracture.

On 11/23, the EE noticed pain in his right knee after working as a Brakeman all day. On the morning of 12/20, the EE noticed that his knee had swelled and had pain. He was sent to the clinic where the injury was diagnosed as a possible torn meniscus. The doctor took the EE off until a specialist could evaluate the situation.

While the EE was changing a feed line valve, he lost his footing causing him to slip and attempt to catch himself with his hand, causing his shoulder to jam resulting in an inflamed rotator cup.

Pulling tire from lace, tire fell, struck left wrist.

While walking to his job site, the employee detoured around the designated walkway and strained his knee on the uneven ground.

While cleaning the packing machine with an air lance, the employee brushed his right hand against the underside of the bag saddle causing a subdermal laceration to the dorsal surface of the digitus tertius intermediate section of the right phalange whereby requiring sutures. He returned to work with no restrictions.

While changing a blank in the cement conveyance line, the EE felt discomfort in his lower back. The EE seeked medical relief 2 days later and was placed in physical therapy.

EE stepped on a clinker ball while walking across a roadway, and twisted his left ankle.

While opening a gypsum car door, the EE squatted quickly causing a strain to his left knee.

Employee stated that on 02/17/2010, he was moving a 2.5 gallon tote of water and twisted wrong, causing a pain in his back. The employee did not seek medical relief until 03/03/2010 at which time the doctor placed him on restricted duty that cannot be accommodated.

The EE was wiring a terminal strip and contacted a hot surface causing a burn to his hand.

Employee was assisting in the the changing of crane cables. When employee stepped away from under the crane a pry bar was dislodged from an upper location, bounced its way through the framework of the crane, off a side wall, and struck the employee above the left eye causing a laceration.

While shoveling, the employee aggravated and enflamed a non work related pre-existing hernia.

After jack hammering, EE felt a soreness in his back. EE did not seek medical advise until 10/23/2009. On 12/03/09, the treating physician made an order for physical therapy.

Was repairing hearing protection on hard hat, when knife slipped and cut finger.

No conditions contributing

Injury was bent down raking under the A-Millclinker feeder belt when he felt a sharp pain in his lower back radiating down both legs.

Lab plunger slipped

Lifted a 5-gallon bucket of clinker. Note: Employee's injury occurred on 10/15/2007 as reported, but the employer was not informed of the injury as a reportable issue until 10/30/07.

Injured EE stepped on side of rock causing left knee to twist resulting in a sprain.

Piece of coating came off wall in the crossover duct and pinned worker.

While trying to lower motor amps to conveyor - placed cement bag on conveyor and hand was sucked into moving roll. Gloves were worn. Tore skin from left pinky and ring finger.

Employee was operating a roller during construction of a mine haul road. He drove up an incline to turn around ans as he was backing up lost control of the roller, drove over the edge of the road under construciton and down approximately 150 of steep hillside., coming to rest upright at the bottom of the slope.

Injured worker was digging out E8 elevator with co-workers. One EE's bar glanced off frame work striking injured worker along left shoulder and ear causing him to slip and fall.

Smashed index and middle fingers left hand while opening door on B mill separator door fell off latches on both did not hold smashing hand against toe board and floor.

Employee was barring buildup from calciner when a slab fell sideways, striking him on the right lower arm (closed fracture).

Hit right wrist.

While swinging hammer with right hand, hammer hit beam overhead and landed on left thumb.

Washing truck; slipped on moss on cement pad.

While air-lancing feed pipe, the feed shot backed under the employee's hood.

Foreign object in left eye while cleaning scale off center of pier. Wind blew object into eye.

Was pulling wire when pull wire broke. On the follow through right hand hit the edge of the pull box. Cut right hand on top of index finger.

Cut left index finger. Whild holding a liner bolt head with left hand and using an air gun with my right, the bold head spun cutting my finger.

Sprain/Strain of the stomach while lifting and pulling.

Was changing out a motor at ground level and spent time on knees on grating and when he got home noticed swelling in left knee.

Contusion of L pinkie

While moving 10" shaft in metal saw, pushing shaft he felt something later in afternoon. Suffered strain in groin area, later diagnosed as hernia when he went to company doctor on 11/16/04.

WORKING IN QUARRY AREA, TWISTED LEFT KNEE WHILE WALKING ON ROCK

WORKER HAD NOT REPORTED UNTIL 7/27/04. CLAIMS HE STRAINED LOWER BACK WHILE WORKING AS A PACKER AND TOLD SUPERVISOR OF MUSCLE CRAMPS IN MIDDLE BACK.

Employee indicates he had a stressful encounter with supervisor, leading to chest pain and shortness of breath.

EE WAS WORKING IN BURNER BUILDING AREA, REMOVING WELDING LEADS. EE STEPPED BACK TO TURN, STEPPING DOWN OFF RAMP. HE MISJUDGED THE EDGE AND FELL.

Installing center cone in roller mill, cone unexpectedly moved toward employee causing ee to jump out of the way. The lanyard snapped tight under ee's right arm pit, causing fractured scapula.

WHILE LIFTING AN H BEAM, EE FELT A TIGHTENING IN LOWER STOMACH.

WAS PLACING A VIBRATOR ON A GYP RAILCAR HOPPER & MISSED THE BRACKET. HELT ONTO THE VIBRATOR & FELT PAIN IN HIS BACK. WENT TO THE DR. ON 6/20/03 & WAS DIAGNOSED WITH A SRAIN/SPRAIN ON HIS BACK. HE WAS GIVEN RX FOR PAIN & MUSCLE RELAXER, BUT RETURNED TO WORK ON LIGHT DUTY. ON 6/24/03 HE RETURNED TO DR. WITH CONTINUOUS PAIN & WAS TOLD NOT TO WORK UNTIL NEXT VISIT.

AT C-MILL, WHILE CLEARING CHUNK OUT OF C-MILL CLINKER, RIGHT HAND WAS PINCHED CUTTING MIDDLE FINGER AND RING FINGER OF RIGHT HAND.

PICKING UP SHORT SECTION OF SCREW CONVEYOR AT THE COAL MILL AREA. EE LEANED TO ONE SIDE AND STRAINED RIGHT SHOULDER. EE DID NOT REPORT INJURY UNTIL 10/14/2003

EE CLAIMS LOSS OF HEARING BECAUSE OF LOUD NOISE EE HAS WORKED AT THIS PLATN SITE SINCE 12-20-61 IN VARIOUS JOB POSITIONS.

EMPLOYEE WAS TAKING A SAMPLE IN THE NO CRANEWAY AREA, HIS RIGHT LEG SLID DOWN, AND LEFT KNEE POPPED.

WHILE ADJUSTING SAFETY GLASSES, A SMALL AMOUNT OF REFRACTORY GOT IN TO RIGHT EYE OF EMPLOYEE WHILE WORKING NEAR ELBOW RACK AREA.

WHILE RELEASING STUCK CHAIN BRAKE ON RAIL CAR, STRAINED SOMETHING IN RIGHT HAND AND ARM.

EE WENT TO RE-SET THE FK PUMP AND A BAG FELL OFF THE SACK SCALE, STRIKING HIS LEFT KNEE, CAUSING SOME SORENESS IN HIS KNEE.

EE WAS REMOVING A DISCHARGE CARDOX TUBE FROM THE BASE SOCKET, WHEN HE WENT TO LEFT, THE CARDOX TUBE SAFETY DEVICE CHAINS HUNG UP IN THE GRATING.

AT 0900 ON 10-9 EE CONTACTED HIS SUPERVISOR AND SAID THAT HE WAS SICK AND WOULD NOT BE IN. ON 10-10- HE REPORTED TO HIS SUPERVISOR THE HE HAD BEEN TO THE DR. AND HE WAS DIAGNOSED WITH HIGH BLO OD PRESSURE AND TO STAY OFF WORK UNTIL HE SEES A SPECIALIST. HE TOLD THE DR. THAT HE WAS STRESSED BECAUSE OF HIS JOB AS A GARAGE MECH TRAINEE. HE FILED FOR WORKERS COMP. THIS IS BEING CHALLEN

EE HAS BEEN WORKING AT THIS PLANT SINCE 5/1/74 IN VARIOUS POSITIONS WITH NOISE EXPOSURE CLAIMED AS CAUSED FOR HEARING LOSS.

EE HAS BEEN WORKING FOR THIS COMNPANY IN VARIOUS POSITIONS SINCE HE WAS HIRED 10/01/80. APPROX. 5 YERS LABORER INTERMITTENT WITH 1 YR. AS PLANT OPERATOR, 3 HRS PACKER. 1.5 YR AS MECH WELDER, A ND HAS WORKED INT HE LAB AS WET ANALYST/LAB ANALYSTER OFF AND ON APPROX. 11 YEARS.

TAKEDOWN & WRAPUP OF 6-750MCM WIRES AT I.D. FAN; EMPLOYEE HELD WIRE TO PROTECT MAN ON LADDER FROM GETTING KNOCKED OVER; WHEN WIRE WAS CUT DOWN, EMPLOYEE BENT OVER TO HOLD WIRE FROM HITTING LAD DER. EMPLOYEE FOLLOWED IT DOWN TO THE GROUND, CAUSING HIM TO BEND OVER AT THE WAIST, STRAINING HIS BACK.

FIRE IN PREHEAT TOWER, 4TH STAGE PLUG, AND TWO MEN DOWN.

LEFT LEG CRAMPED UP, EMPLOYEE STTOD UP TO STRAIGHTEN LEG, AND OVEREXTENDED KNEE.

PIECE OF GRATING CAME LOOSE AT 3RD FLOOR PREHEATTOWER, EE STEPPED ON IT CAUSING JARRING, SPRAIN TO HIS LT. SHOULDER.

2 EES WERE AIR LANCING HOPPER AT C8 VIBRATING SCREW; BECAUSE THEY WERE USING SAME AIR OUTLET THE AIR PRESSURE WAS REDUCED AND INJURED EE PRYED AND DUG AT MATERIAL IN HOPPER. THIS CAUSED HIM TO SEVERELY STRAIN HIS LEFT SHOULDER.

FELLOW WORKER WAS CLEANING ROCKS OUT OF TRACK OF SHOVEL, INJURED EE WAS PARKED ON OPPOSITE OF SHOVEL UNREELING HOSE FROM LUBE TRUCK AND BACK INTO PATH OF THROWN ROCK, STRIKING HIM ON THE LEFT SIDE OF HEAD, CAUSING CONTUSION TO HEAD AND LACERATION TO EAR, WHICH REQUIRED STITCHES.

EE WORKS AS PLANT OPERATOR, WHILE DRILLING A CARDOX PORT, HE WAS BENDING OVER TO PORTHOLE, WHICH IS LOW. THE BIT WAS TOO LONG MAKING IT HARD TO KEEP DRILL LEVEL AND STRAIGHT. USING HEAVY DRILL WHILE REMOVING BUILDUP IN TOWER. EE CLAIMS STRAIN OF LOWER BACK.

EE, A MECHANIC WELDER, WAS BREAKING IN AS BACKUP FOR THE MACHINIST. HE WAS TURNING DOWN A PIECE OF STOCK IN THE 24.5" LATHE & WENT TO CLEAR OUT SHAVINGS. INSTEAD OF USING A TOOL HANDLE TO CLEA N SHAVINGS, AS REQUIRED, HE GRABBED A PIECE WITHHIS RIGHT INDEX FINGER. HE RECEIVED A LACERATION THAT REQUIRED 8 STITCHES.

EE WAS WORKING INSIDE THE KILN-GRINDING THE METAL SURFACE TO PREPARE IT FOR REFRACTORS. HE WAS WEARING SAFETY GLASSES BUT NOT GOGGLES OR FACE SHIELD AS REQUIRED. WHILE GRINDING OVERHEAD HE FEL T MATERIAL IN HIS RT EYE. HE FINISHED HIS SHIFT & WENT HOME. BY NEXT MORNING EYE WAS VERY IRRITATED & HE WAS ESCORTED TO MD. DIAGNOSIS: TINY PIECE OF METAL EMBEDDED IN EYE & HAD TO BE REMOVED

EE HAS BEEN WORKING FOR COMPANY SINCE 4/2/74. HE HAS SPENT THE MAJORITY OF THIS TIME AS A DRILLER-POWDERMAN (SINCE 6/20/78) 23+ YRS. HE WAS RUNNING AN AIR-TRACK FOR MUCH OF TIME, WHICH EXPOSED HIM TO HIGH NOISE LEVELS. THE AIR-TRACK IS NO LONGER UTILIZED.

EE WAS WELDING A GUSSETT ON THE DOOR TO THE WHOLE TIRE CHUTE. HE WAS WORKING OFF A TEMPORARY PLATFORM,WELDING OVERHEAD & A PIECE OF HOT SLAG DROPPED INTO HIS LEFT BOOT. THE BOOT WAS NOT LACED UP ALL THE WAY ALLOWING AN OPENING FOR MATERIAL TO DROP INTO. HE ALSO DID NOT REPORT THIS TIL 5 DAYS LATER. DIAGNOSED W/CELLULITIS LEFT LEG. BURN WAS @ ANKLE BONE.

EE WAS WORKING ON REPLACING AN AIRSLIDE @ TOP OF TRUCK LOADING SILOS LOCATED @ PACKHOUSE. HE WAS TIGHTENING BOLTS ALONG SEAM OF AIRSLIDE WHEN WRENCH SLIPPED OFF & HE STRUCK HIMSELF IN THE MOUT H W/WRENCH. THIS RESULTED IN CHIPPING HIS UPPER LEFT MOLAR. FILED A WORKERS COMP CLAIM 2/21/02 TO VISIT DENTIST.

WHILE REMOVING THE WEST HOURNAL END CAP ON THE ROLLERMILL, THE END CAP WAS SLIPPING OFF. IN THE EFFORT TO PUSH IT BACK ON, PINCHED THE LEFT LITTLE FINGER BETWEEN END CAP AND PIECE OF PIPE. REC EIVED A LACERATION TO THE FINGER AND THE FINGERNAIL WAS PULLED LOOSE. THE LACERATION REQUIRED 1 STITCH.

INJURED HAD JUST STARTED HIS SHIFT AND WAS ATTEMPTING TO REACH DOWN AND LIFT A 1" BY 3" BY 20' PIECE OF FLAT BAR. HE FELT A SHARP PAIN IN HIS LOWER BACK. HE WAS TAKEN TO DR AND DIAGNOSED WITH BACK STRAIN AND PUT ON RESTRICTED DUTY.

TWO MECHANICS WERE STRAIGHTING THE GLANGE ON A COOLER TUBE ELBOW. OUR MECHANIC WAS USING A ROSEDUB TO HEAT THE FLONGE AND THE OTHER MECHANIC WAS STRIKING IT WITH A SLEDGEHAMMER. WHEN STRIKING THE GLANGE THE HAMMER HAD DID NOT CATCH THE FLANGE FULLY AND CONTINUED THE SWING AROUND AND STRUCK THE OTHER MECHNAIC WHO WAS STANDING TOO CLOSE IN THE FOREHEAD. HE RECEIVED A LACERATION CONTU

EE WAS REMOVING THE GRINDING BOWL FROM THE "CHATTER BOX" (PULVERIZING EQUIPMENT) WHEN HE FELT A SHARP PAIN IN HIS HAND AND WRIST. HE WAS SENT TOMEDICAL CLINIC AND WAS DIAGNOSED WITH TENDONITIS OF THE RIGHT WRIST. THIS TASK HAS BEEN PERFORMEDTHOUSANDS OF TIMES OVER A 40 YEAR PERIOD WITH NOPREVIOUS INJURIES.

AROUND 10AM IT WAS REPORTED THAT THERE WAS "SMOKING" COAL AT THE BOTTOM OF COAL SILO. HOT ENOUGH TO BE MELTING FEEDER BELT. WE IMMEDIATELYSTARTED PUTTING WATER ON AT THE TOP OF THE SILO AND AT THE BOTTOM. WE OPENED THE EMERGENCY DUMP CHUTE AND EMPTIED APPROX. 400 TONS OF COAL ONTO THE GROUND. ONLY DAMAGE WAS TO FEEDER BELT. NO INJURIES.

EE REPORTED ON 10/15/01 THAT HE HAD INJURED HIMSELF ON 9/13 BY USING A ROPE AND PULLEY TO LIFT APIECE OF METAL TO THE TOP OF THE RAW FEED BINS. ACCORDING TO HIM, THE HAND OVER HAND ACTION CAUS ED PAIN IN HIS RIGHT SHOULDER. ON NOVEMBER 12 HEBROUGHT IN A SLIP FROM HIS TREATING PHYSICIAN WHICH STATED THAT HE HAD LIMITED OVERHEAD LIFT/PUSH/PULL OF 10 LB.

EMPLOYEE WAS PUSHING OUT TRANSITION SECTION BETWEEN CONE SECTION TO SILO AND PUMP, MATERIAL FELL FROM SILO. EMPLOYEE COULD NOT SEE BUT CONTINUED TO MOVE SECTION, FELT HAND GET SMASHED.

EE WAS HELPING TO MOVE A MOTOR AT C4 GRIZZLEY ON6/15/01. ON 6/18 HE REPORTED CRAMPING AND MUSCLESPASMS ON HIS LOWER LEFT SIDE. HE WENT TO CLINICON 6/19 AND WAS DIAGNOSED WITH ACUTE MUSCLE SPAS MS BUT RETURNED TO WORK WITH NO RESTRICTIONS. ON11/2 RECEIVED AN MRI. ON 11/6 DIAGNOSED WITH A SLIGHT BULGING DISC. STILL NO LIMITATIONS OR WORKDAYS MISSED.

EMPLOYEE WAS ATTEMPTING TO REMOVE METAL COUPLER FROM A 15' LENGTH OF 6" FLEX HOSE. HE WAS HOLDING HOSE WITH ONE HAND AND PULLING ON COUPLER WITHTHE OTHER. HIS HAND SLIPPED OFF OFF THE COUPLING AND THE HOSE SWUNG BACK AND HIT HIM ON THE BRIDGE OF THE NOSE. HE WAS DIAGNOSED WITH A BROKEN NOSE, WILL RETURN TO WORK THE NEXT SHIFT.

EMPLOYEE WAS HOLDING A KNOCKER WRENCH WITH HIS LEFT HAND AND HAMMER IN HIS RIGHT. AS HE SWUNG THE HAMMER TO HIT THE WRENCH, IT MADE A GLANCING BLOW STRIKING HIS LEFT HAND. DIAGNOSED WITH CONTU SIONS TO THE 4TH AND 5TH DIGIT OF HIS RIGHT HAND.

EMPLOYEE BECAME CONCERNED ABOUT A PAY ISSUE. HE DID NOT MAKE ANY ATTEMPTS TO COMMUNICATE HIS CONCERN AND BECAME ANGRY AND VERBALLY ATTACKED HIS DEPT MANAGER. THIS PERSON HAS SHOWN TENDENCIES TOWARDS ANGER IN THE PAST. AFTER HIS OUTBURST HE WAS TOO STRESSED TO WORK AND WE INSTRUCTED HIM TO SEEK A DR'S REPORT FOR HIS STRESS.

EE STOOD UP AND FELT A PAIN IN HIS LEFT ANKLE. EH DID NOT STRIKE OR BUMP ANYTHING. DIDN'T TWIST OR CATCH ANYTHING WITH HIS FOOT. HE SAYS THAT THERE WERE NO PROBLEMS PRIOR WITH HIS ANKLE AND SU PERVISOR IS NOT AWARE OF ANY PROBLEM. DIAGNOSED WITH ACHILLES TENDON AND PUT ON MODIFIED DUTY. IT IS NOT KNOWN WHAT CAUSED INJURY.

ELECTRICIAN HAD TREADED CONDUIT EARLIER IN THE DAY. APPEARS THAT SOME OF THE METAL SHAVINGS WERE CAUGHT IN HIS HAIR AND SOMETIME LATER DROPPED INTO HIS LEFT EYE. A PIECE BECAME EMBEDDED AND HA D TO BE REMOVED. A PRESCRIPTION WAS WRITTEN. THIS WAS DONE 2-23 - DID NOT RECEIVE DR. REPORT UNTIL 3-6-01(ATTACHED)

EE WAS SWINGING A 80 LB SLEDGE HAMMER TO ALIGN BOLT HOLES ON COOLER TUBE TO ELBOWS. HE FELT SLIGHT DISCOMFORT IN HIS LOWER BACK BUT DID NOT REPORT IT. HE REPORTED ON 2/14 THAT HIS BACK WAS HU RTING & NEEDED TO SEE A PHYSICIAN. DIAGNOSED AS LOWER BACK STRAIN & PUT ON RESTRICTED DUTY.

EMPLOYEE WAS ATTEMPTING TO REMOVE A 5 GALLON BUCKET FILLED WITH CEMENT. HE HAD GRABBED IT WITH HIS RIGHT HAND AND DRUG IT OFF THE TABLE. THE WEIGHT OF THE BUCKET YANKED HIS ARM DOWN SHARPLY . HE WAS DIAGNOSED WITH A POSSIBLE TEAR TO THE ROTATOR CUFF.

EE WAS GRABBING A TIRE AT THE DISCHARGE END OF THE TIRE HEAD BELT INTO KILN, WITH A GRAB HOOK THE TIRE FELL PULLING HIS RIGHT HAND FORWARD & STRIKING IT AGAINST THE HANDRAIL. HE WAS TAKEN TO RMC WHERE HE WAS TREATED FOR A LACERATION & SLIGHT FRACTURE TO LITTLE FINGER ON HIS RIGHTR HAND RELEASED TO WORK

EE WAS TRYING TO TIGHTEN UP LOOSE RAPS OF CABLE ON THE HOLDING LINE DRUM. AS THE CRANE OPERATOR WAS UNWRAPPING CABLE IT CAUGHT IN A NOTCH WORN IN THE BASE. HE KICKED IT TO FREE IT UP & INADVER TENTLY PLACED HIS HAND UNDER THE LOOSE CABLE. WHEN FREE IT SLAPPED DOWN ON HIS LEFT HAND. HE RECEIVED CUT AND SLIGHT FRACTURE TO SMALL AND MIDDLE FINGERS. LACERATION TO RING FINGER NEEDING SU

THE EE & 3 OTHERS WERE REPLACING WORN BUCKETS ON THE F355 KILN FEED BUCKET ELEVATOR. THE INJURED EE WAS GRABBING THE BUCKETS AS THEY WERE BEING UNBOLTED. HE WAS GRABBING THEM WITH HIS RIGHT HA ND. THE CONTINUAL WEIGHT (APPROX. 25165) DROPPING CAUSED A LIGAMENT STRAIN IN HIS RIGHT SHOULDE5R. HIS RIGHT ARM IS TO BE KEPT IMMOBILE FOR AT LEAST 2 WEEKS. EE REPORTED INJURY ON 9-5-00.

EE PUT HIS RIGHT HAND INTO HIS FRONT PANTS POCKET, AND RAN HIS FINGER ACROSS THE PARTIAL OPENED BLADE OF HIS POCKET KNIFE, CAUSING A LACERATION TO HIS RIGHT INDEX FINGER. THE LACERATION REQUIR ED 6 STITCHES.

A CUSTOMER DRIVER JUST HAD A LOAD OF SACK PUT ON HIS TRUCK. HE WAS INT HE PROCESS OF TYING DOWN THE SACKS. HE CLIMBED ONTO HIS TRALER. UNLOUD USE THE REQUIRED ALLEGESS LADDERS, TRIPPED AND FEL L BACKWARDS ONTO THE CONRETE. BROKE BOTH ARMS WRENCHED NECK AND BACK.

EMPLOYEE HAS WORKED FOR COMPANY SINCE 7-59. HAS HELS MANY JOBS THROUGHOUT THE PROCESS. HE IS GOING TO RETIRE AS OF 8-1-00 AND DECIDED HE SHOULD GET HEARING AIDS PRIOR TO RETIREMENT.

EE HAD PAIN IN RIGHT ELBOW PRIOR TO THIS TIME FRAME. HE THEN NOTICED PAIN IN HIS RIGHT ELBOW WHILE SWINGING A HAMMER TO STRIKE A KNOCKER WRENCH. HE REPORTED ON 7/26/00 THAT HE WAS EXPERIENCING CONTINUED PAIN. HE WENT FOR MEDICAL EXAM & WAS DIAGNOSED WITH STRAIN TO LOWER ARM, BUT ABLE TO PERFORM REG DUTIES. 10/4/00 DR PRESCRIBED PHYSICAL THERAPY & WORK RESTRICTIONS.

EMPLOYEE IS PARAPLEGIC FROM MOTORCYCLE ACCIDENT.HE HAD BEEN USING TOILET FACLITIES AND WAS TRANSFERING BACK TO HIS WHEEL CHAIR. HE HAD HOLD OF THE BAR ABOVE AND WAS LOWERING INTO CHAIR WHEN TH E CHAIR MOVED ON HIM. HE CONTINUED TO LOWER HIMSELF TO FLOOR AND HEARD SOMETHING POP. HE BROKE HIS LEFT FEMUR.

EE WAS WORKING ON SPLICING C11 CONVEYOR BELT. WHEN HE BENT OVER HE EXPERIENCED BACK SPASM IN LOWER BACK. ON 2-24-2000 HE SAW A DR. & WAS DIAGNOSED WITH LUMBAR MUSCLE SPASM. HE WAS PUT ON SOME RESTRICTIONS UNTIL 3/2, AT WHICH TIME HE WAS RETURNED W/O RESTRICTIONS.
