EE was in the bottom compartment below the cooler while it was being stroked. EE was spraying blue paint on any metal that was rubbing on grates. EE lost balance, as EE regained balance EE threw EE's hands up in frustration. EE's hand and spray paint got caught in the grates above.

Electrician opened an electrical panel which contained a 4160v line. The employee tested the line with an instrument rated for 1000v. The employee received first degree burns to arms and face.

EE was using a self-retracting knife to cut zip-ties. The knife slipped and cut through the employees glove and sliced EE's right index finger requiring 6 stitches.

The fire was in our cooler system. Equipment failure caused part of the flow of material to plug. With hot material spilling there was a small fire at the bearing. It was behind guarding. The fire caused a lot of smoke and the fire department showed up. They were not needed.

Employee was shifting hanging spider line hoses in the pack house. EE was moving the hose from one connection point to the next, approximately 18" away. The employee lost grip while pulling the hose causing the hose to pinch left pinky finger. Employee received 6 stitches

EE was walking in the plant next to a conveyor. EE hit head on a metal beam approximately 6' off the ground, EE had hard hat on at the time. EE was taken to the clinic with a neck strain and placed on modified duty.

OE was attempting to move a roll of pond liner with a fork lift near railroad tracks. The forks became stuck on the tracks. When backing up, the forks popped off the ground and the liner rolled off of the forks down a slight slope about 3 feet. EE was standing in the path of the rolling liner and it rolled over ankle. EE suffered a small fracture on foot and ankle.

On 8/18/17 EE was doing normal job activities which included using a sledge hammer to unplug a chute. On 8/19/17 EE states ee felt pain in back but did not report any injury until 8/21/17. EE was taken to the clinic where ee was placed on restricted duty.

The employee had just taken off glove, the pin had grease on it and the end had a rough edge from being hit with a hammer.

Injured worker was performing clean-up at the tertiary duct. EE was pulling a bucket of loose material out of the duct with a rope, when ee slipped and fell against the handrail on the scaffold platform hitting lower back.

On Friday 04/07/2017 two persons were in the preheater tower service elevator when it became stuck on the 5th floor where its doors would not open. This condition lasted approximately 60 minutes until the elevator service company responded and manually released the elevator doors.

Injured employee was cleaning up loose material around the 5CD3 tail pulley. As employee was moving backwards with a wheel barrow, employee tripped on a grease drum pallet and fell on employee's left side causing pain to the left side of employee's back and left thigh.

Injured employee was flying a drone for end of month inventory activities. While landing the drone was blown off course and employee tried to catch it by one of the landing legs when employee made contact with one of the propellers causing cuts to two fingers on employee's right hand that required butterfly stitches to close.

The plant was removing the top section of a bucket elevator and they were unaware of another crew working on the bottom section. While removing the top section that jarred a 3"x5" compacted piece of debris within the bucket elevator. The debris landed on worker's left hand and splitting the webbing between index finger and middle finger

The ee in question complained on 01/23/2017 of having eye irritation. The employee was then given eye wash to flush the eye and then taken to the company clinic to have the eye examined. There were no foreign bodies embedded in the eye, however the ee was given prescribed medicated ointment to treat the eye.

On September 7, 2016 a employee reported that they had been bitten by a spider on the top of the right foot as EE dressed for shift. The employee refused medical treatment that day. On Sept. 8, 2016 the employee stated that the bite had become infected and wanted medical treatment. The employee was then placed on restricted duty due to cellulitis on the top of the right foot.

An employee was changing a hydraulic line and didn't release the pressure before taking off the clamp. EE got hit with pressured hydraulic oil in wrist and hand. There was not a cut or puncture wound to the employee. The employee was taken to the hospital to be checked out. EE was kept overnight for observation.

EE was working inside of 4GM1 (Horizontal Ball Mill) removing liners. A piece of metal fell from in between the liners striking the employee's right leg causing a laceration to the employee's inner thigh requiring 3 sutures to close.

Employee was shoveling in dusty area, specifically 5DDC2 of the cement plant - rash occurred on arms, legs from cement dust.

On 05/10/2016 at 1527hrs, a contract employee was traveling in an elevator from the 9th floor of the Pre Heat Tower towards the 1st floor when the car suddenly stopped between the 7th and 6th floors. The elevator service company arrived at 1710 hrs. At 1830 hrs the doors to the elevator were opened and the employee was able to exit the car.

EE was attempting to open a package containing a utility knife, when the knife unexpectedly fell out of the package making contact with the EE's left index finger causing a laceration, requiring 6 sutures to close.

Employee parked the haul truck he was operating for a restroom break. While in restroom, he was playing with his personal pocket knife when it slipped out of his hand and cut his upper thigh. The cut was 1" long and required medical attention. We bandaged him up and drove him to El Camino Urgent Care where he received a couple stitches. He was released an hour later.

The EE in question was dismounting his work truck, when he lost his balance and fell backwards landing on the ground in a seated position, which resulted in a lower back strain.

Employee was pushing a toolbox onto a trailer, and was applying upward pressure to the handle when it broke, the employee's arms then continued in a upward motion at which point the employee's right forearm was sliced by the shorn retainer screw for the handle.

Employee was pulling a horizontal cartridge filter from a dust collector, when he stated he felt a pop in his right bicep.

The person was attempting to remove a "stuck" bucket on a skid steer loader with two other people when it came free, dropping down to the ground and crushing his left foot. Three bones in his foot are broken and surgery is expected to be needed to repair according to the ER MD.

An employee was working to descale a sulphur ring at the feed end of the Kiln, he then discharged an air cannon that was missing an air line causing dust to enter the hood of his silver suit irritating his eye and obscuring his vision. The employee then proceeded to head to the service building and stepped into a pile of hot raw meal causing 2nd and 2rd degree burns to his legs.

The EE was working in a confined space to adjust the fan blades on a Booster Fan for a period of 6-8 hours on 2/21/2015. The EE stated that he experienced lower back pain the next day (Day Off) and was taken to the clinic on Monday 02/23/2015 and was diagnosed with a lower back strain.

On Friday 02/13/2015, 1 Lehigh Employee and 3 B&B Industrial Services employees were entrapped in the PreHeat Tower elevator, when it stopped between the 4th and 5th Floors. Employees were entrapped a total of 50 minutes.

The employee in question was stepping out of a work truck, with a razor knife in his left hand. The employee then put the knife in his right hand and attempted to put the knife in his right pocket when the knife opened slicing his right index finger requiring 4 sutures to close.

Employee was tearing out (demo) refract material when a piece fell and hit him in the right shin. He was taken to the clinic, had a contusion on his shin.

Arrived to work on 01/29/2015, informed his foreman that he had woken up and his right eye was swollen. On the night of 01/28 he had been welding/grinding/gouging as well as working in close proximity to other welders. He was taken to the doctors, particle removed from his right eye and he immediately returned to work with no restrictions.

Employee was tearing out(demo) refract material when a small piece of refract fell and hit his right wrist. Took him to the doctor, diagnosed as wrist sprain.

He was walking up a rope ladder and lost footing and rolled his ankle.

Tripped while working and back of left hand hit walkway handrail while trying to grab rail to regain balance. Employee mentioned incident to supervisor but declined any treatment at the time. One week later it still hurt, was sent to clinic and x-rays show a fracture. Hand placed in splint. Light duty for 10 days. No lost time.

The employee in question was working to close a damper manually, when it released, pinching the employee's left middle finger between the damper flap and lip, fracturing the employee's fingertip.

After stepping down from a 2'ladder, the ee turned and felt a pop in his right knee. EE was seen by the Dr. on 04/25/2014. A causation investigation was launched at company's request. The ee underwent surgery to right knee on 09/29/2014. On 12/29/2014,company received causation findings stating that an acute on chronic injury meeting the 1% WC causation requirements occurred.

Employee was stocking bolts in a bin, twisted to reach and grab some more bolts from a box behind her and sprained her lower back.

The front down rigger of a boom truck fell onto a table top while victim was holding it. This caused a crushing injury to the tip of his right pinkie finger. There is a small fracture at the tip of the finger and swelling. A splint was placed on the finger by a doctor and no further treatment is anticipated other than follow up visits.

A contractor was working on demolishing a crusher stak, that has been decommissioned by Lehigh SW Cement. The contract employees were cutting a conduit, when slag material fell into the apron of the jaw crusher, and caught residual grease, that was not visible from their working location on fire.

Employee was working to rebuild a Roll Press, when a plate slid from the top cab of the press striking the employee on the head, causing a 2" laceration that requiring 8 sutures.

An employee with 12 years experience was reclaiming clinker, when he came in contact with hot material, causing 2nd and 2nd and a half degree burns to 15% of his upper extremities.

Left hand pinkie finger was caught between the safety hand rail of a man lift and the equipment cross beam while cleaning up fugitive dust and buildup. Finger was lacerated, 19 stitches required. Possible partially torn tendon in finger. Employee referred to specialist for further treatment.

The employee was in an elevator with 2 other employees, traveling to the 3rd floor of the Main Service Building, when the elevator malfunctioned, and overtraveled past the 3rd floor landing, running into the top of the shaft. The elevator then dropped 3 to 4 ft. before stopping abruptly at the 3rd floor landing, causing a lower back strain.

Employee was moving empty barrels. While moving one barrel, another barrel behind it shifted and fell striking the employee on the left foot.

On Wednesday 02/06/2013 five people were in our service elevator when it became stuck at the 3rd floor. The condition lasted over 30 minutes until the elevator service company responded and activated the elevator.

Employee was installing a feeder belt and felt a pinch in his right shoulder.

While removing guard attached to a stand the weight shifted and guard pinched his right arm against a stationary stand resulting in bruises to his wrist and elbow.

Employee was inside service elevator when it became stuck between the 5th and 6th floors. This condition lasted over 30 minutes until the elevator service company responded and activated the elevator and corrected the stuck button.

Employee was descaling material with an air lance. She bent down and a piece of coating came out of the port. The piece bounced off her jacket and went under her hood landing on her left eye lid.

G-Cooler clean out during shutdown; While lowering plastic buckets of clinker down to the ground with a rope, the employee strained his groin. He did not report the injury until 1 week later (03/23/12)at which time he was taken to the clinic. He did not miss any work and will be on light duty next week. He is a 3 week temp worker, next week is his last week.

Employee was assembling equipment when his unk middle finger got smashed.

Walking from Spider Line sprained his knee.

EE pinched rt thumb during the removal of a portion of an air slide - resulting in sutures and fracture to rt thumb.

While cutting with a hand held grinder the blade bound up and broke. This caused the grinder to flip backward cutting the employee above the right thumb.

Employee was utilizing a grinder on a piece of metal, employee felt a dust particle entered his eye and proceeded to remove his safety glasses and rub his eyes.

Laborer was exposed to poison oak while clearing weeds. He did not realize until after he went home that is had happened. He waited until Monday to reort the exposure and was taken to the clinic to be checked out. He was given some ointment and medication and given time off to heal up.

Moving empty gas bottles and lifting one up 8 inches into the storage area employee felt a sharp pain in the left side of his groin. Examined at the medical clinic and possible hernia. Returned to work with restricted duty.

Employee started transferring carbon from the storage tank into the silo when an ignition occurred which blew the top safety hatch open. This was a rather large ignition that resulted in fire shooting out the top of the tank and light up the night sky. When the ignition occurred the employee was standing between the storage tank and silo.

Employee reported to his supervisor that his left knee hurt after rolling a vacuum hose that was loose into a full roll. Employee refused medical attention until he requested on 03/03/11 to see a company doctor.

A fork struck EE on the inside of his left ankle causing a contusion.

Lower back pain while shoveling walkway along belt way. Employee reported incident, but required no medical treatment. On 1/31/11 employee requested appointment at company clinic and 2/1/11 was placed on restricted duty. No lifting over 25 lbs and no work below knee level.

A unplanned inundation of gases into the working area which requied the removal of employees from the affedted location until conditions were stablized.

Heated material came in contact with acetylene and oxygen cylinders. The heated material caused the acetylene cylinder to over heat and the pressure relief valve to break and a 2" rupture in the acetylene cylinder, creating a flame exiting the cylinder.

Customer driver parked his truck on the one side of the Safe Racks to open the truck hatches. He proceeded to the top platform and activated the Safe Rack cage on the opposite side of his truck. He fell from the platform to the ground suffering a broken wrist and a head laceration. Also, knocking himself unconscious. He was transported to the hospital by ambulance.

Employee was shoveling at the tailpulley of 5BC01. Employee shoveled for about an hour and did not feel any pain. On September 31 employee reported that his back hurt.

Employee opened a port on a meal pipe to check for plug up and material flushed out burning the employee on the back of the neck, head, left ear and hand.

Employee was preparing to sut weld with cutting wheel on grinder. When he turned grinder on wheel pulled glove & finger in thus cutting finger needing 9 stitches

Air impact gun spun around and hit EE in the arm; resulting in sutures.

Employee was walking from parking area when a gust of wind blew dirt into her right eye. When she rubbed the eye, she scratched the cornea.

Employee stepped on a rock and twisted his ankle.

Employee sustained a small laceration to his stomach when a small piece of a shattered bearing, off of a shaft, struck the employee.

Associate B (Laborer) strained his upper thigh and hip when he slipped while shoveling mud.

Maintenance Mechanic strained right rib cage while trying to tighten a loose fitting as he was installing a pipe.

While stepping down from the truck employee miss judged the distance, lost his balance and fell backwards.

The employee was cleaning the build up on the sewage treatment tank when he slipped and fell on the left bank down the gravel slope.

While employee was cleaning up in the finish mill area, cement dust blew around his safety glasses and into his eye. Employee was given prescription eye drops.

When employee leaned forward while using a 15 lb. chipping hammer, he felt pain in his right foot. Employee was treated and placed on restricted duty. Restriction were accomodated, no lost time.

The employee was moving a 55 gallon drum while holding the barrel at an angle. He tried to push an object out of the way with his left foot. This caused a pull or tear in the groin area.

Employee was repositioning an oil-covered pump onto a pallet. The pump slipped from his hands and pinched his right middle finger. An x-ray of the finger showed a fracture of the distal phalanx of his right middle finger.

Employee carrying 5 gallon buckets when he slipped, fell and landed on his back.

A wedge the employee was installing dislodged from its mount striking the employee on both legs.

Employee was moving a pump with a bar in his hand. While trying to wedge the bar to move the pump, the bar slipped moving forward striking the employee on the left hand.

The employee was checking the triple gates on the coal mill. He climbed up a ladder, leaned over to look in to the mill then felt a sharp pain to his low back.

Employee was diagnosed with bi-lateral carpel tunnel syndrome.

Employee was working in the Pre-Heater tower to clean tipping valves when the left hand middle finger became caught between the hand rail and the tipping valve weight.

The employee was removing bliver mill bowls from the bliver machine. While lifting the bliver bowls the employee felt a strain in the wrist.

While moving boxes and furniture from second floor office to first floor office employee began feeling pain in neck, shoulder and back.

While trying to pry a screw out of a pump with a 5' bar, the bar slipped off the fulcrum point causing a strain to the right shoulder.

While overhauling Unit 7FKI a pipe slipped and fell on employee.

While attempting to bend a metal tab on a Clinker Cooler door using a four pound sledge hammer, the hammer hit the edge of the door, deflected and hit the worker's left thumb. The impact resulted in a laceration requiring sutures.

While placing a wrench on a cam nut the wrench slipped and fell to the deck where the employee's hand was located. The wrench struck the employee's right hand.

Installing head liners in the ball mill, employee lost footing while hooking liner to the hoist and strained his upper back. Employee was placed on restricted duty for 2 days.

Clearing belts with blower. Put hand on belt. Fan moved catchig finger between pulley and belt.

Employee pinched left hand middle finger palm side when he was placing a lid on the airslide. The finger got pinched between the two flanges. Employee was placed on restricted duty.

Employee placing gear guards on the mill gear when he felt pain in his lower back. Clinic diagnosed with strain to the lower back and placed on restricted duty for 2 days.

Employee was walking down the stairs when he slipped and fell to the bottom.

Installing liner in the mill when the liner slipped and pinched skin on palm side right hand pinky finger. The employee elected to voluntarily go home.

Employee was cutting rubber backing for shell liners with a razor knife when he slipped and cut his pinky finger of left hand. Employee received 9 stitches.

Employee was installing liner in the mill when his foot slipped causing strain to the lower back. Employee was released to modified duty but declined the modified duty position.

While prying a gearbox off of a shaft, the bar slipped pinching the employee's finger between the bar and weighfeeder.

Employee was installing liners in the mill when a foreign body got in the left eye. Foreign body was removed from the employee but he still was still complaining. He was taken to the clinic where a rust mark was found on the cornea. The rust was removed by an Ophthalmologist.

Miner adjusting a ball mill liner in the mill, when the liner swung to the left, pinched left index finger between the liner and the diaphram which caused a fracture. Miner was placed on light duty on 1/13/07 but elected to decline the modified duty postion.

Contractor suffered a broken left leg and right heel when he inadvertently stepped back off mill pier while installation of raw mill bearing housing was underway. Employee fell approximately 15'5". Employee had removed his fall protection to gather parts on the ground below and had failed to put it back on.

Miner removing liner in the ball mill, when it slid down and landed on his right foot behind the steel toe. The miner was informed on 1/3/07 that there was a fracture of the right 5th metatarsal. Miner was placed on restricted duty.

Miner initially reported on 12/26/06 that slag went through his left sleve on 12/23/2006 & his elbow was swollen. Miner stated that the alledged injury might have happened at a different operation. Miner was taken to clinic where he was diagnosed with cellulitus. He was placed on restricted duty from 12/26 thru 1/1/07.

A co-worker was pulling a drive chain back into position when the chain slipped out of his hands striking the injured worker in the right elbow.

Employee was using a jack hammer when the hammer slipped causing his finger to become caught between build up of the hammer and the hammer handle.

After the employee had changed a belt his hand became caught between the belt and the pulley.

Miner got struck by flying debris,(foreign body) on the left knuckle of index finger, as the operator of the jack hammer was chipping material. Miner received one stitch.

Miner was assisting two other mines in sliding a 2' X 6' X 1" steel plate three feet away from dry grass. Miners were repositioning plate so that cutting sparks would not make contact with, and ignite, grass. The distal tip of the small finger on the miner's right hand was severed when he reflexively jerked his hand back when it made contact with a rock underneath the plate.

Employee was removing liners in the mill with a sledge hammer when a piece of hammer head chipped off hitting employee in the forehead. Employee received stitches.

Employee was performing an electrial inspection/repair when he struck his right elbow on the edge of a hydraulic panel.

Employee was walking down the stairs when his foot slipped off the step. The employee held on to the stair rail but felt pain in his low back.

While cleaning under Palblizer; employee's foot slipped striking a metal cross beam. Injury became reportable after extensive medical treatment.

The employee was moving empty pallets when he felt pain in his low back.

The employee was using a jack hammer when he slipped and fell, striking his left shoulder. Injury was not reported to employer until 2/13/06.

While walking along a wet/muddy conveyor path, ee slipped and twisted his leg/knee.

DESCALING IN THE PRE-HEAT TOWER, EMPLOYEE WAS POKING, DUST AND POWDER FROM THE PLUG GOT TRAPPED BETWEEN POKING SUIT AND SKIN RESULTING IN A CHEMICAL BURN. INITIALLY REPORT ONLY, DIAGNOSISED 5/23/05 ON FIRST DOCTORS VISIT

While pulling hose from bottom part of oraface - high power water gun hit kick plate knocking cement material loose which fell underneath safety glasses causing corneal abrasion in left eye. Went to doctor on 5-19-05.

DAYS UNKNOWN AT THIS TIME, PRIOR TO THE FAILURE AN EMPLOYEE CLEANED THE ROADS COVERING THE DRAIN PIPE FOR THE LAKE. AS THE WATER CRESTED THE DAM GAVE WAY.

WHILE DESCALING 5RC BY WATERBLASTING FEED SHELF CHEMICAL DUST GOT UNDERSLEEVE. INITIALLY A REPORT ONLY, EMPLOYEE WENT TO DOCTORS ON 5/16/05 BECAME REPORTABLE WITH DIAGNOSIS

While using a chipping gun to knock off build-up on the rails to the 7 FK pump, the employee strained his left shoulder.

Employee was lifting transformers 7.5KVA from building to truck and truck to building. While pulling transformer out of back seat of truck, sprained muscles in elbow & shoulder. Initially first aid - now turned into lost time. Surgery on 3-3-06.

EE WAS CHIPPING SLAG OFF OF A WELD, WHILE ANOTHER EE WAS DOING THE SAME. A PIECE OF HOT SLAG WAS KNOCKED OFF BY THE OTHER EE AND HIT THE INJURED EE IN LOWER LEFT EYELID

EE was 2-3 feet away when an object used to pull a metal I beam to the steel: broke loose and the object hit the ee on the right hand.

EE WAD DOING HIS ASSIGNED TASK OF CLEAN UP. EE WAS ATTEMPTING TO CARRY OUT A PIECE OF I-BEAM. EE HAD SEEN ANOTHER EMPLOYEE CARRY THE I-BEAM ON HIS SHOULDER. EE THOUGHT THIS WAS AN EASIER WAY OF CARRYING OUT THE I-BEAM. WHEN EE PICKED UP THE I-BEAM TO HIS SHOULDER, HE LOST HIS BALANCE AND FELL, RESLUTING IN A RIGHT INDEX, MIDDLE AND RING FINGER FRACTURE.

While trying to open a waste bin container ee struck himself in the chin causing a small contusion/laceration requiring 2 stitches.

While unplugging the 1st stage cyclone, ee walked into hot material around where they were unplugging-resulting in 2nd degree burn to right ankle. ee diagnosed 12-6-04.

While washing down work area, hose twisted out of employee's hands - striking him on right side of face near eye, knocking off safety glasses.

WHILE CHECKING 8BC-1 TO 8BC-2 DISCHARGE; WHEN GOING DOWN THE STAIRWAY EMPLOYEE SLIPPED ON MAT 4, LANDING ON HIS TAILBONE. DUE TO MEDICATION ON DOCTOR'S VISIT OF 9/13/04, THIS HAS BECOME REPORTABLE.

Ee was installing bearing on drive. He hit the adapter to align it and a metal chip came off and lodged itself into his chin.

Ee was installing 12" driscoll line at rock plant 8PW50 - while bending, twisting, lifting he experienced pain in lower back. Ee was taken off work by doctors 7-23-04.

EMPLOYEE USING THE BALL HOPPER AND HOIST. THE BACKING PLATE USED TO HOLD THE HOIST GOT CAUGHT IN HIS RIGHT THUMB, PARTIALLY TAKING THE TIP OF RIGHT THUMB. PER CALL, VERY TIP OF THUMB WAS CUT O FF, THIS WAS NOT A TRUE AMPUTATION.

2 EE'S WERE MOVING A ROLLER IDLER FROM A CATWALK, ONE EE LOST HIS GRIP AND THE IDLER LANDED ON HIS LEFT FOOT.

EMPLOYEE WAS WALKING UP THE STAIRS, HE FELT PAIN IN RIGHT ANKLE.

AFTER USING A UTILITY KNIFE TO CUT HOSE, EE PLACE KNIFE IN HIS POCKET WITH THE BLADE NOT COMPLETELY SECURE THEREBY CUTTING HIS LEG.

EE WAS GETTINT COMPRESSION CYLINDERS OUT OF THE FOG ROOM. HE LIFTED THE CYLINDER AT AN AWKWARD ANGLE AND STRAINED BACK.

EE WAS BUSTING CEMENT CHUNKS, WHILE SWINGING THESLEDE HAMMER HIS FINGER WAS CAUGHT BETWEEN THE CHUNKS AND THE HAMMER.

EE WAS CLEANING STEEL SHAVING FROM UNDER LATHE HE CUT HIS FINGER FROM ONE OF THE SHAVINGS.

STAGED ROLL SLID DOWN CATWALK AND HIT EE ON RIGHT LEG, CUTTING LEG.

EE WAS DOING HIS REGULAR WORK DUTIES AND WAS OVER COME WITH GAS-FUMES O. EE WAS RUNNING ADDITIVE.

EMPLOYEE WAS WORKING ON A GEARBOX TIGHTENING NUTS WITH A 2 3/8 BOX ENDED WRENCH. HE SPRAINED RIGHT SHOULDER BY PULLING AND PUSHING WRENCH. HE WAS TREATED FOR FIRST AID AND CAME BACK TO WORK. H E WENT FOR SURGERY ON SAME SHOULDER ON 5/17/02 AND MISSED WORK UNTIL NOW.

EMPLOYEE WAS REMOVING LINERS IN THE BELL MINE. HE PLACED A WEDGE UNDER LINER TO LOOSEN FROM RUBBER POD. WHILE SWINGING A SLEDGE HAMMER HE MISSED WEDGE AND HIT HIS RIGHT FOOT ON THE INSIDE AT A PPROX THE KNUCKLE OF BIG TOE.

EMPLOYEE WAS CLEANING A GREASE GUN AND THE HOSE ON IT WHIPPED BACK AND HIT HIM IN THE MOUTH.

EE WAS TWEAKING LIMIT SWITCH. HE DUCKED TO MOVE BACK TO AVOID GETTING HIT. IN THE PROCESS HE TWISTED HIS ANKLE.

AFTER STEPPING DOWN FROM LOADER TRUCK, EMPLOYEE STEPPED IN A ROCKY AREA CAUSING HIM TO SPRAIN RIGHT FOOT AND ANKLE, HEAL OF FOOT ALSO.

EE STOOD ON TOP OF A PUMP TO GET LEVERAGE TO CLOSE A STICKING VALVE. THE VALVE HANDLE CAME OFF AND EE FELL BACKWARD.

EE WAS USING AN AIR LANCE TO CLEAN THE TRANSFER CHUTE FROM 1BC3 TO 1BC4. WHEN EXTENDING THE LANCE BELOW THE DISCHARGE BELT, A CLOUD OF DUST BLEWBACK AT HIM AND EXPLODED.

PULLING ON BOTTOM COVER OF DRAG CHAIN AND IT BROKE LOOSE AND FELL ON FOOT.

USING JACK HAMMER AND AIR IMPACTING ON THE OUTSIDE OF MILL. AT THE END OF SHIFT THE EMPLOYEE HAD PAIN IN HIS LOWER BACK. THE EMPLOYEE WAS GIVEN PAIN MEDICATION AND RELEASED TO FULL DUTY. SEVER ITY OF PAIN INCREASED AND DOCTOR TOOK HIM OFF WORK ON JAN 30, 01 FOR 3-5 DAYS.

EE WAS MOVING OIL DRUMS AND CAUGHT HIS LEFT MIDDLE FINGER BETWEEN TWO DRUMS.

EMPLOYEE USING 10 LB HAMMER DRIVING BOLTS INTO MILL HEAD WHEN THE HEAD OF HAMMER CAME OFF AND STRUCK HIM ON THE BRIDGE OF NOSE STRIKING SAFETY GLASSES AND CUTTING HIS NOSE.

EE WAS USING A AIR HOSE AND IT IT DISCONNECTED, HITTING HIM IN THE MOUTH AND FACE KNOCKING OUT FRON TOOTH (L) SIDE.

INJURY WAS REPORTED TO THE SUPERVISOR ON SEPT 9,2000. THE EE STATED HE DID NOT WANT TO SEE A DR. BECAUSE THE PAIN WAS NOT THAT SEVERE. DUE TO INCREASE IN PAIN THE EE WAS TAKEN TO THE DR. ON 10 -10-00. DR. DIAGNOSED SPINE SPRAIN AND INSTRUCTED EE TO TAKE TIME OFF WITH MEDICATION.

USING A SLEDGE HAMMER, HAMMERING A SCREW CONVEYOR SUIDE ON SHAFT, AND HE STRAINED HIS RIGHT SHOULDER.

WHILE CLOSING 5-CY-10 (CONE DOOR) WITH 5' BAR, EE FELT A SHARP PAIN IN LOWER ABDOMEN.

EMPLOYEE WAS REPLACING BAD BUCKETS ON ELEVATOR AND HIT THE TOP OF HIS HEAD.
