Employee stated that they were operating a dozer working the highwall. As they were prying on rock, the track slid off rock hitting the wall causing dozer to jar the employee resulting in pain in the neck, back, and leg. Went to doctor on 10/20/17 to get checked resulting in the first lost work shift.

This caused pain to back, neck, and hip area. Continued working. To ER on 4/1 to check. Continued working till 4/11/16 when doctor took EE off work thus lost time starting 4/11.
Employee stated that he was operating excavator moving spoil material. A rock rolled out of the spoil and hit a foot long stick laying on the ground. The stick flipped into the air and through the windshield cab guard and busted the windshield. A foreign body got into his right eye. Would not flush out so he went to doctor to have removed. Given eye drops (used for 2 days).
Employee stated that he was prying on rock with the dozer blade. The blade slipped off of the rock causing the dozer to jar the employee resulting in low back pain. He decided to go to doctor on 3/2/15 due to persistent low back pain and missed the 3/3/15 work shift. He is currently on restricted duty.
Employee stated that he was climbing up ladder onto the drill. As he was pulling himself up the ladder, he felt a sharp pain in his left shoulder. Finished working this shift but missed the 11/11/14 work shift due to going to the doctor and having shoulder checked out.
Employee stated that he was back dragging the road from the parking lot to the pit area with a dozer. The road was muddy and rough. The dozer jarred the employee and he said that he felt pain from his waist down through his legs. He went to the ER doctor. Has a herniated disc and is off of work.
Employee stated that he was changing cutting edges on dozer. He was using a grinder, cutting torch, and air wrench. Sometime during this process, a piece of metal got into his left eye. Noticed it later that day but could not rinse it out. Went to doctor on 1/26/12 & had it removed from eye.
Employee stated that he was pushing out shot rock with his dozer. As he was backing his dozer, he ran over a rock causing the dozer to shift and become unbalanced. This motion jarred the employee resulting in low back pain. Went to doctor on 1/24/12 & became reportable.
Employee stated that he was operating a dozer scaling the highwall. He was prying on a rock with the dozer blade, when the blade slipped off and hit the highwall. This stopped the dozer and caused a whiplash effect. Went to doctor on 5/29/09 and on 6/9/09 missing two shifts work, thus reportable.
EMPLOYEE STATED THAT HE WAS BACK DRAGGING THE ROAD WITH THE DOZER. HE BACKED OVER A ROCK WITH THE DOZER TRACK. AS HE DID, THE DOZER JARRED THE EMPLOYEE WITH PAIN OCCURRING IN LOWER BACK AREA RADIATING INTO GROIN AREA. WENT TO DOCTOR IN ER THAT NIGHT AND MUSSED THE NEXT SCHEDULED WORK SHIFT.
EMPLOYEE STATED THAT HE WAS OPERATING A DOZER PUSHING SHOT ROCK TO THE LOADER. HE BACKED UP TO AVOID A ROCK WHICH WAS GOING TO FALL FROM THE MATERIAL BEING PUSHED. WHEN HE DID, HE BACKED THE D OZER UP AND INTO A LARGE BOULDER WHICH CAUSED THE DOZER TO JAR THE EMPLOYEE. WENT TO DOCTOR ON 1-15-04 AND INCURRED LOST TIME ON THAT DATE.
EE STATED THAT HE WAS OPERATING A DOZER PUSHING SHOT ROCK DIRT. HE SHIFTED THE DOZER TRANSMISSION INTO REVERSE AND TURNED HIS HEAD TO LOOK BACK HE STATED THAT WHEN HE TURNED HIS HEAD, HE FELT A "POP" IN HIS NECK AND HAD PAIN IN HIS NECK.
EE STATED THAT HE WAS SCALING THE HIGHWALL WITH A DOZER WHEN A LARGE ROCK BROKE LOOSE. THE ROCK FELL AND HIT THE CORNER OF THE DOZER BLADE AS HE WAS BACKING UP AND OUT OF THE WAY. THE CAUSED T HE DOZER TO SHAKE/JAR THE EE RESULTING IN A MILD CERVICAL STRAIN. REPORTABLE DUE TO LOST WORK SHIFT ON 3-9-01.
