While the roof bolter operator was starting to drill a hole for a rib bolt, eight channel straps slid forward into the operator station striking right leg above the ankle causing a fracture.
As employee was installing a cable bolt, the strap that was between the bolt and top began spinning and struck the employee on right cheek causing a laceration that required sutures.
When employee lowered the boom on the roof bolter after drilling a hole, the steel came out of chuck and struck him on the left thumb. He later sought medical treatment and had surgery on the thumb to repair a torn ligament on 10/22/2015.
Employee was assisting another worker install a roof bolt. When the mast was lowered, it caught his right hand between the drill pot and mast base causing multiple fractures to the hand and the loss of the ends of his index and middle fingers which were removed during later surgery.
While installing a cable bolt, the employee reached up to align the plate when he hit the joystick on the roof causing the tip of his middle right finger to get caught between the plate and the rock deflector causing an avulsion.
While employee was bolting, the strap he was installing got caught on the mast of the roof bolter as it was being swung out. When the strap came free, it struck the employee on the forehead over the right eye causing a laceration requiring sutures.
While operating the continuous miner a piece of rock flew back from the cutting head and struck the employee in the left leg causing a laceration requiring sutures.
While the employee was lining up drill steel to start a bolt hole, he moved the drill head and caught his left thumb between the steel and the ATRS rock deflector. He suffered a laceration on his left thumb requiring sutures.
While the employee was repositioning a strap while roof bolting, he caught his wrist between the drill head and the rock deflector on the roof bolter. He suffered a contusion to the wrist which required medical treatment. He returned to work for his next scheduled work shift.
As employee was trying to lower the boom on the roof bolter, he hit the wrong lever and closed the steel guide catching his finger between the guide and steel. He suffered a fractured left middle finger. He began missing work due to the injury on 3/15/2010.
While employee was bolting a strap slid off the ATRS and stuck employee on the cheek causing a laceration requiring sutures.
While checking the suction on the drill head, the roof bolter steel came out of the drilled hole and struck the employee on the left middle finger causing a laceration requiring sutures.
Employees were cutting 6" off of a 20" piece of 4' steel pipe which was suspended by a crane. The injured employee was holding down the pipe near the sling. When the pipe was cut the suspended piece rose up and caught the employee's thumb between the pipe and a piece of metal gurting resulting in a laceration to his left thumb requiring sutures.
While roof bolting, the employee was adding the second piece of steel to the starter steel when the bolter head moved, catching his right thumb between the two pieces of steel. This resulted in a laceration and fracture to the thumb.
As employee dropped the head on the roof bolter a hose on the boom hit a piece of drill steel that was sticking out of the tray. The drill steel came up and hit the employee in the mouth chipping two teeth.
While bolting, the drill wrench broke. While the employee was retrieving the broken piece of the wrench out of the drill pot, the drill steel fell out of the hole striking his left index finger resulting in a fracture to the finger.
As EE was checking the suction on the head of roof bolter, a piece of drill steel fell out of hole being drilled & struck left ring finger.
Employee was bolting top when a bolt he was attempting to anchor dropped approx. 9 feet and struck his right thumb. He was treated and released at the local ER. He required stitches to close a laceration. His thumb was fractured as well.
Elder-Ridge Contracting Company (4 men) were changing bearings on a snub roller of the Huff Creek Mine slope belt located on the surface. The crane boom failed, causing boom to fall striking 3 contractor employees.Injured was struckin the upper back, neck and left forearm. He has an MRI scheduled for his back and neck.

As he was operating a roof bolting machine, he bumped hard into ATRS portion of the machine with his shoulder. He continued to work, but started missing work on August 14,2006 to have rotator cuff surgery.
He was removing drill steel from a hole in the roof. Two pieces separated and the deepest piece fell from the hole and pinched his finger against the bolt wrench.
HE WAS OPERATING THE RIGHT DRILL HEAD ON THE 001 SECTION DRILL. HE HAD DRILLED THE HOLE AND WAS INSTALLING A CABLE BOLT. HE HAD PUSHED THE BOLT INTO THE HOLE AND WAS ATTEMPTING TO PLACE THE BOLT HEAD INTO THE TIGHTENING WRENCH WHEN HE CLOSED THE STEEL GUIDE ON HIS THUMB.
A coal outburst occurred on the 005 section, 7 left panel in #3 entry inby #28 crosscut. The outburst was triggered by the continuous miner while mining the "B" run to the right.
He was installing roof bolt into previously drilled hole. As he pushed bolt into hole, it caught on lip of hole & bent the bolt. He struck bolt with tightening wrench, in hopes that bolt would jump into the hole, but instead, the bolt came loose & struck him on lt wrist. He was instructed to follow up w/his dr on next day & returned to work on his next scheduled shift.
HE WAS OPERATING THE RIGHT DRILL HEAD ON THE 001 SECTION DRILL. HE HAD DRILLED THE HOLE AND WAS INSTALLING THE RESING CARTRIDGE AND BOLT. THE BOLT HUNG IN THE LIP OF THE HOLE, CAUSING THE BOLT TO BEND, CATCHING HIS LEFT RING FINGER BETWEEN THE BOLT AND THE MAST.
He was in the process of drilling a roof bolt hole, when a piece of rock approximately 12" x 30" x 0" to 7" thick fell and struck him on the right forearm.
He was drilling a roof bolt hole when a piece of rock approx 12" by 15" by 1" thick fell from the roof and struck the drill head and broke into pieces. A small piece of rock bounced up from the drill head and struck him under his nose.
HE WAS OPERATING THE CONTINUOUS MINER AND MINING A PILLAR LIFT WHEN A PIECE OF ROCK APPROXIMATELY 12" X 20" X 1" THICK FELL FROM BETWEEN THE BOLTS AND STRUCK HIM ON THE LEFT HAND.
AFTER DRILLING A HOLE, HE WAS REMOVING THE DRILL STEEL FROM THE HOLE WHEN A PIECE OF ROCK APPROXIMATELY 2'X2'X7" THICK FELL, PUSHING HIS HAND AGAINST THE HEAD OF THE DRILL.
After bolting a cut, EE stepped onto roof drill of ATRS to hang reflectors in the roof bolt plate. The bolter operator dropped the ATRS to move the machine. He was caught in a pinch point.
HE STATED THAT AS HE WAS OPERATING THE CONTINUOUS MINER IN RETREAT MINING, A PIECE OF DRAW ROCK FELL OUT OF THE MINE ROOF FROM BETWEEN BOLTS AND STRUCK HIM ON THE RIGHT SHOULDER AND BACK. HE STARTED MISSING WORK ON 10-07-03 AND WAS DIAGNOSED AS HAVING A FRACTURE TO 1 RIB. THIS OCCURRED ON THE 002-0 SECTION, 2 SOUTH PANEL AT THE #24 CROSSCUT.
HE HAD JUST DRILLED A NINE FOOT HOLE IN THE ROOFUSING 3 SECTIONS OF DRILL STEEL. AS HE WAS RETREATING THE STEEL OUT OF THE HOLE, THE TOP SECTIONOF STEEL GOT HUNG IN THE HOLE. HE HAD HIS HAND ON THE BOTTOM SECTION OF STEEL TO GUIDE IT AS HEUSED THE FEED TO RECONNECT THE STEEL, AND THE TOP SECTION FELL FROM THE HOLE STRIKING HIS FINGER.
WHILE LOWERING THE HEAD OF THE ROOF BOLTING MACHINE, HE REACHED FOR A PIECE OF DRILL STEEL. THE DRILL HEAD HIT AGAINST HIS LEFT THIGH AND KNEE, BRUISING IT. HE STARTED MISSING WORK ON 12/6/02.
HE HAD DRILLED A HOLE IN THE ROOF FOR A BOLT AND THE DRILL STEEL HAD HUNG IN THE HOLE. HE PLACED HIS RIGHT HAND ON THE DRILL HEAD AND THE STEEL FELL FROM THE HOLE AND STRUCK HIS LEFT HAND.

HE WAS INSTALLING A 5' RESIN ROOF BOLT INTO A DRILLED HOLE AND THE BOLT BENT. AS HE WAS TRYING TO REMOVE THE BOLT FROM THE HOLE, HE USED THE DRILL ROTATION AND THE BOLT STRUCK HIS RIGHT WRIST.
HE WAS DRILLING A HOLE FOR A ROOF BOLT AND AS HE STARTED THE HOLE, A SMALL PIECE OF ROCK FELL FROM THE ROOF AND STRUCK HIS FOREARM.
HE & A CO-WORKER WERE ROOFBOLTING THE TURN CUT IN THE RIGHT CROSSCUT OF #7 ENTRY. WHILE WAITING ON HIS BUDDY HE DECIDED TO CLEAN HIS DUST BOX. HE WAS STOOPED OVER BESIDE THE DRILL WHEN A PIECE OF ROCK APPROX. 9' LONG X 21" WIDE X 15" THICK FELL FROM THE RIB NEAR THE TOP OF THE SEAM & STRUCK HIM IN THE BACK.
HE HAD INSTALLED A BOLT IN THE ROOF AND AS HE SWUNG THE DRILL HEAD OVER TO START ANOTHER HOLE,THE CANOPY BRUSHED AGAINST THE ROOF AND DISLODGED A PIECE OF ROCK, WHICH FELL AND STRUCK HIM ON THE LT. ARM. THE ROCK HAD A SHARP EDGE THAT CUT HIM.
HE WAS DRILLING A HOLE IN THE FLOOR WITH A HAND DRILL. THE DRILL STEEL BECAME STUCK AND HE AND A CO-WORKER PULLED ON THE DRILL IN ORDER TO FREE THE STEEL. THE DRILL SEPARATED FROM THE STEEL AN D THE DRILL HANDLE STRUCK HIM IN THE MOUTH.
HE WAS OPERATING A CONTINUOUS MINER AND THE RIB POPPED, DUE TO OVERBURDEN PRESSURE, AND A PIECE OF COAL STRUCK HIM ON THE ARM.
A PIECE OF DRILL STEEL HAD LODGED BETWEEN THE STEEL TRAY & THE FRAME OF THE DRILL. AS HE SWUNG THE DRILL HEAD OVER TO INSTALL THE NEXT BOLT, THESTEEL CAME OUT AND STRUCK THE BACK OF HIS HEAD.
HE WAS ROOF BOLTING WHEN A COAL AND ROCK RIB ROLLED OFF AND STRUCK HIM ON THE BACK AND LEGS.
HE WAS USING CUTTING TORCHES TO CUT BOLTS OUT OF A CAT PAD ON A D7 BULLDOZER, AS A BOLT WAS CUT, THE PAD JERKED, CATCHING HIS HAND AGAINST ANOTHER PAD.
HE WAS SPOTTING ROOF BOLTS OUTBY THE LAST OPEN XCUT AND A SMALL PIECE OF ROCK FELL FROM THE ROOFAND STRUCK HIS FINGER. HE WORKED ON THE 11TH ANDHAD OUTPATIENT SURGERY ON THE 12TH.
HE WAS PUSHING COAL TO A RAW COAL FEEDER WTH A DOZER. HE TURNED THE DOZER AND CHANGED DIRECTION AND THE DOZER DROPPED SUDDENLY OFF SOME LOOSE COAL. THE IMPACT STRAINED HIS NECK. HE BEGAN MISSI NG WORK ON 7-30-01.
