Employee was removing a rock from the hopper when the bar came loose and he fell onto belt which was moving and fell onto ground, hurt hand.

Employee was getting into truck when he fell backwards onto rocks, hitting head, although he had his hard hat on.

While descending the steps of the loader, Injury hit his head on the metal support bar for the left side mirror. The mirror had been removed for repairs leaving a bare metal support bar protruding out from the top of the loader.

EE CLIMBED ON TOP OF AN AREA HE SHOULD NOT HAVE. OTHERS NOT AWARE OF HIM ON BELT STARTED MACHINE. EE STILL ON LIGHT DUTY, PHYSICAL THERAPY FOR KNEE.

PERSON WORKING ALONE TO REMOVE BATTERIES FROM 200 KOMATSU (WEIGHING APPR. 100 LBS APIECE). REQUESTED ASSISTANCE AFTER LEANING DOWN TO REMOVE, BATTERY SLIPPED, HE GRABBED IT INJURING HIMSELF.

IMPROPER LIFTING BY EE & THEN CONTINUING TO CARRY ITEM ABOVE CONTRIBUTED TO MUSCLE SPRAIN - STRAIN IN BACK.
