Employee stated was weed eating around a groundwater well when it slung some type of insect onto EE's hand. It bit the back of EE's left hand. No symptoms appeared until the 15th. Did not go to doctor until the 17th due to swelling/redness in the left hand/arm. Given antibiotics and no lost time was incurred.

Employee stated was servicing a coal loader. Squatted down onto knees to change the engine air filter. When employee did, felt a pop in right knee resulting in pain in right knee. Went to doctor to get checked out. Was put on restricted work duty.

Employee stated that he was operating a dozer. He stopped the dozer so as to check the hard bar oil. When he opened the panel door to check the oil level, wind blew a foreign body into his right eye. He could not get it flushed out. Went to ER to get removed. Given eye medication and missed one shift, thus reportable.

Employee stated that he was working in the shop putting a seat in a coal loader. He squatted down to put in bolts and when he tried to get up, he felt severe pain in his left knee. Checked by doctor on 02/11/2015 due to persistent pain and only became reportable on 02/20/2015 when he missed his first shift of work.

Employee stated that he was removing radiator cap to check anti-freeze in a light plant. As he reached in to remove cap, a small non-venomous snake bit him on the arms. Went to ER to get checked. Given tetanus shot and antibiotic ointment to put on bite area. No lost time incurred and no work restrictions.

Employee was hauling coal from surface pit to tipple in a Mack DM886SX tandem coal truck. As he was descending a grade on haul road, the truck overturned onto the driver's side entrapping the driver. The driver had a medical condition which led to the accident. The heart attack led to the driver's death.

The individual (self-employed) was not an employee of Red River Coal. He was in the process of salvaging scrap metal from the mine site. He complained of heartburn earlier and became nauseous and sat down. He fell over with an apparent heart attack. Transported to hospital where he was pronounced dead.

Employee stated that he was operating a dozer. The muffler stack fell off. He stopped and got out of the dozer to pick it up, when something was blown into his left eye.

Employee stated that he was operating loader in coal pit. He noticed his right hand itching with red spot developing. Swelling became more pronounced at end of shift. To ER next day when right hand and arm was swollen. Probable spider bite but was not felt by employee.

EMPLOYEE STATED THAT THE LOADER WOULD NOT START . HE RAISED THE LID ON THE BATTERY BOX TO CHECK THE TERMINAL ENDS. AS HE SHUT THE LID ON THE BATTERY BOX, THE WIND BLEW A FOREIGN BODY INTO HIS RIGHT EYE. THE FOREIGN BODY WAS EMBEDDED IN HIS RIGHT EYE AND WENT TO DOCTOR ON 8-19-98 TO HAVE IT REMOVED. ONE LOST WORKDAY THUS INCURRED.

EE STATED THAT HE WAS STANDING STRADDLE OVER A DRILLHOLE. HE BENT OVER TO LOOK DOWN THE BLAST HOLE WITH A FLASHLIGHT, SO AS TO CHECK OUT THE BLAST HOLES. HE STOOD UPRIGHT SO AS TO PROCEED TO T HE NEXT DRILL BLASTHOLE, HE TWISTED HIS BODY AS HE JUMPED TO THE SIDE OF THE HOLE AND FELT IMMEDIATE PAIN IN HIS BACK AS HE JUMPED. TO DOCTOR. FIRST LOST DAY OCCURRED ON 3-17-97.

EE STATE THAT HE WAS STANDING ON THE SPOIL DUMP WHEN A GUST OF WIND BLEW DIRT INTO HIS EYES. HE FLUSHED OUT HIS EYES BUT COULD NOT GET THE FOREIGN MATERIAL OUT OF HIS LEFT EYE. HE WENT TO DR O N 11-5-96, AND HAD THE FOREIGN MATTER REMOVED . NO LOST TIME BUT REPORTABLD DUE TO MEDICATION ADDED TO HIS EYE.

EMPLOYEE STATED THAT THE DOOR ON THE MOTOR GRADER WAS OPEN WHEN A GUST OF WIND BLEW DIRT INTO THE EMPLOYEES LEFT EYE REMOVED FOREIGN MATTER FROM EYE BUT WENT TO DOC DUE TO CORNEAL ABRASION IN EYE DR APPLIED ANTIBIOTICS TO EYE THUS MAKING ITA REPORTABLE ACCIDENT NO LOST TIME INVOLVED

EE STATED THAT HE STEPPED OUT OF THE DOZER TO CHEC K ON A HYDRAULIC LEAK ON THE TILT HOSE. A GUST OF WIND BLEW DUST INTO BOTH OF HIS EYES. WENT DR. AND MISSED THE 10-07-94 WORK SHIFT.

EMPLOYEE STATED THAT HE OPENED THE DOOR ONT EHC AB OF THE DRILL TO CHECK ON THE MATERIAL THAT HE WAS DRILLING. WHEN HE OPENED THE DOOR, A GUST OF WIND BLEW DUST INTO THE DRILL CAB AND INTO EM PLOYEE'S LEFT EYE. FLUSHED EYE OUT. HOWEVER, HAD TO GO TO DR ON 7-22-94. RESULTIN GIN CORNEAL ABRASION TO LEFT EYE.

EMPLOYEE STATED THAT HE WAS WELDIGNON A TANK ABOVE THE HYDRAULIC UNIT AT THE COAL AUGER SAMPLER. A GUST OF WIND STIRRED UP DIST AT THE SAMPLETER CAUSING FOREIGN MATTER TO BE BLOWN INTO EMPLOY EE'S RIGHT EYE. HE WENT TO THE DR ON 4-13-94 AND HAD DUST/RUST REMOVED FROM THE RIGHT EYE. THIS IS REPORTABLE DUE TO MEDICATION (ANTIBIOTICS) FOR THE EYE.

EE WAS AT THE EXPLOSIVE MAGAZINE GETTING BLASTING SUPPLIES WHEN HE RECEIVED A BEE STING TO THE LEFT HAND AND TO THE RIGHT LEG CAUSING A LOCAL REACTION TO THE BEE STING EE WENT TO DR AND RECEIV ED AN INJECTION OF DECADRON AND RETURN TO WORK. NO LOST RESULTED.

WIND BLEW P C O FFOREIGN MATTER IN L EYE

EMPLOYEE WAS OUTSIDE OF HAULER CLEANING THE WINDOWS WHEN THE WIND BLEW FOREIGN MATTER (DIRT) INTO HIS RIGHT EYE. WENT TO DOCTOR ON 11-5-90 TO REMOVE FOREIGN MATTER FROM EYE. NO LOST TIME RESUL TED BUT REPORTABLE DUE TO USING PRESCRIPTION MEDICATION FOR EYE. (MILD CORNEAL ABRASION).

EMPLOYEE WAS GETTING ONTO A 777B HAULER SO AS TO START HIS WORK SHIFT WHEN THE WIND BLEW SOME FOREIGN MATTER INTO HIS RIGHT EYE.WENT TO DOCTOR AND WAS GIVEN PRESCRIPTION FOR EYE DROPS AND PAIN PILLS.NO LOST TIME WORK DAYS INVOLVED BUT PRESCRIPTION FOR EYE-THUS REPORTABLE.

EMPL ADJUSTING PACKING ON HEAVY MEDIM CYCLONE PUMP.AS HE WAS WORKING HIS WAY AROUND PUMP,TURNED & TWISTED AND FELT KNEE POP CAUSING EXTREME PAIN IN LEFT KNEE.WENT TO DR WHO DIAGNOSED CHIPPED K NEE CAP .

EMPLOYEE WAS STANDING AT THE COAL TRUCK DUMP AREA WHEN FOREIGN MATTER WAS BLOWN INTO EMPLOYEE'S LEFT EYE BY THE WIND WENT TO DOCTOR TO REMOVE MATTER FROM EYE DID SO AND WAS GIVEN A PRESCRIPTIO N TO TAKE TO PREVENT INFECTION NO LOST TIME
