The employee was climbing the stairs on the way to Tower #2 to change screens. When EE reached the platform, EE started to turn to the side and put EE's hand on the handrail when EE felt a "pop" in EE's lower back. EE was not carrying anything while climbing the stairs.

Employee was washing out around the scale when EE felt discomfort on right leg above the ankle near the top of where EE's pull on boots came too. The area looked red and a little festered so EE was sent to the doctor. The doctor diagnosed it as an abscess and prescribed an antibiotic. the employee returned to work at full duty.

We told a contractor that we would no longer need their services on 01/29/2015. We saw this contractor 2 times after this date and there was no mention of an injury. We asked him to return our companies equipment on 02/02/2015 and he then told us that he hurt his back and stomach. I do not have anymore detail on the injury because this is all the detail that the contractor gave us.

EE was standing outside of the mobile maintenance shop when wind caused dust/debris to enter his left eye. The eye was flushed on site, and examined after at an urgent care center. The dust/debris resulted in a corneal abrasion. The EE was wearing all of his required PPE at the time, including safety glasses.
