A contract truck driver was delivering Ione clay to the plant. After dumping almost all of the material from the end dump trailer. The driver climbed inside the trailer to remove some material that was stuck to the bottom of the trailer. The driver was found unconscious and unresponsive inside the trailer.

Employee was bending over to pick up a piece of pipe when they felt a strain in their back. Employee did not pick up the pipe only hurt their back bending over.

EE twisted their knee while walking down the stairs in the D3 Mill building. EE states they planted their right foot to turn and felt a pop and pain in right knee. EE thought the pain would dissipate but it did not.

Employee found the loader operator unresponsive and called the plant foreman. The plant foreman entered the loader cab and placed the equipment in park and shut the motor off. EE was unable to get any vital signs from the operator and immediately dialed 911. The local fire department responded, and the loader operator was pronounced dead at approximately 12:35 pm.

Personal medical emergency - stroke

While EE was exiting an enclosed cab sweeper, the wind caught the door and smashed EE's right hand index finger in between the door jam and the door of the sweeper.

Employee claims to have strained an abdomen muscle while sitting down on a creeper.

A miner was operating a dozer when the miner felt nauseous and became ill

A miner had slurred speech and was not acting in EE's normal demeanor and was taken to urgent care for fear of a stroke, EE was diagnosed with a transient ischemic attack and given a full release to return to work

EE was cleaning Tower on floor 1A when dust blew toward them. Causing dust to get under face shield and safety glasses. EE had dust removed from under eyelid.

EE reported injury on 5/11/17 but did not want medical evaluation until 5/12/17. EE stated while walking during shift felt a sharp pain in lower back. EE cannot recall anything they did that caused the pain. EE taken for medical evaluation and placed on modified duty for 4 working days.

Miner was standing outside of the heavy equipment shop going through task training on a front end loader and was drinking a soda. A bee stung the employee on the lower lip. The miner had an adverse reaction to the bee sting and was transported to a local clinic where employee was treated for an allergic reaction to the sting.

The Employee was preparing to start up the lime AB system when some quicklime became dislodged and the wind blew the quicklime into the face of the EE. The EE used the eyewash system for a few minutes to wash out eyes and face and had not further issues until the EE tried to remove contact lenses which caused a corneal abrasion due to the quicklime

Employee working in load out control room watching sand conveyor. Opened window and sand blew into his left eye.

EE entered the cyclone area of the riser through the man way door on floor 1E. As EE was walking to his work area he said that the fan that was inside the work area blew dust into his eyes. The fan was inside to provide air movement along with an air horn to disperse airborne particulates to the outside atmosphere.

EE reports a spider bite, does not know when or where it happened. He says it probably happened when he opened a bottom door on a rail car. Informed his supervisor on 5-6-15, asked to see a physician on 5-7-15. The physician gave him a shot and a prescription. Returned to full duty, no restrictions

EE had right shoulder injury on 10/21/14. EE was taken for medical evaluation and returned same day with no restrictions or further treatment. On 3/11/15 EE reported pain in same shoulder, taken back for evaluation, Dr treated as first aid. EE went back for evaluation on 3/24/15 for evaluation and physical therapy to begin and pain meds prescribed.

Walking to work area.

Employee had a finger cut when it was pinched in the door frame of a front end loader when the door was blown shut by wind while he was in the process of cleaning the rear window of the loader. The cut required three sutures.

EE WAS RUNNING APPROX 200 FT TO SHUT OFF A PUMP WHEN HE FELT A PAIN IN HIS LEFT CALF MUSCLE. DIAGNOSIS: TORN L CALF MUSCLE.

Employee reported soreness in his back on 6/17/2010 after finishing his shift, having three days off on regular rotation and taking some vacation time.

The injured party was preparing to measure a silo and upon removal of a vent cover, material was exhausted by a gust of air through the vent into his face. PPE was in use at the time of the incident.

Employee complained of chest pains during lunch and asked for medical responders to be called. Employee was transported to local hospital for evaluation and passed away early the next morning. Employee has history of heart conditons.

Employee stated that he was working in an area that caused discomfort (ringing) in his ears. Employee supplied company with a letter from his doctor listing a requirement of restricted duty on 6-1-2009 indicating a restriction of work duties not to include high noise

Employee sitting on seat of haul truck, reached to turn knob on radio and felt pain in left lower back. Taken to er, off one day and placed on modified duty.

EE was troubleshooting a valve on a cement rail unloading bin when dust blew in his face and eyes.

Employee was standing by the side of his cart when the wind blew the door shut on his left thumb. Employee did not request medical treatment until 07/21/05. The doctor took x-rays that were negative and refered to orthopedics. Orthopedic doctor did in office surgery with sutures on 09/14/05 and returned employee to work 09/15/05 with no restrictions.

Employee was walking when a bee stung her on her back.

WIND BLEW METAL PARTICLE AROUND SAFETY GLASSES AND IMBEDDED IN EYE

EE HIRED IN 1994 WITH PRE-EXISTING BACK INJURY IS CLAIMING A CONTINUOUS TRAUMA TO HIS BACK FROM 9/94 TO 7/24/04. THIS EE HAS NOT REPORTED ANY INJURIES TO HIS BACK NOR HAS HE FILLED OUT ANY ACCIDENT REPORTS. HE HAS NOT LOST ANY TIME DUE TO HIS BACK. 9/24/04 COMPANY RECEIVED NOTE FROM HIS DR. WHO WAS TAKING HIM OFF WORK DUE TO HIS BACK. STILL NO REPORT

EMPLOYEE WAS KNEELING ON THE GROUND WORKING ON A PIECE OF EQUIPMENT. AS HE WENT TO STAND UP HE FELT PAIN IN HIS LEFT KNEE.

While removing a door from a mill, cement dust blew into his eyes.

Employee called into work and complained of back pain on 6/25/04 but not work related. He went to his doctor on 7/2/04 and was refered to a spine specalist who on 7/9/04 said that his injury was work related. Employee did not report anything until 6/29/04 and then went to his personal doctor who did not report it as work related.

EE WAS PUTTING ON HIS GLOVES WHEN HE FELT SOMETHING BITE HIM ON HIS FINGER.

DUST BLEW INTO WORKERS EYE R.

EMPLOYEE STATED THAT WHEN HE GOT READY FOR BED HE NOTICED THAT HIS RIGHT CALF WAS SWOLLEN & DISCOLORED. AFTER WORKING A WEEK HE WAS CONCERNED ABOUT THE DISCOLORATION & WENT TO HIS PERSONAL PHY SICIAN WHO THE PUT HIM ON MODIFIED DUTY.

IT IS BELIEVED THAT THE EE SUFFERED A HEART ATTACK.

EMPLOYEE STATES THAT WHILE DRIVING A FORKLIFT BETWEEN THE MILLS AND PACKHOUSE THE WIND BLEW SOMETHING IN HIS RIGHT EYE. HE DID NOT REPORT THIS UNTIL THE NEXT DAY. MAY 16, 2002. HE THEN WANTED TO GO TO THE DOCTOR. HE WAS TAKEN TO AN OPHTHALMOLIGIST WHI REMOVED A SMALL FOREIGN OBJECT. NO MEDS, RESTRICTIONS, RETURNED TO WORK.

WIND BLEW SOMETHING IN EYE WHILE WORKING ON BUILDING A PLATFORM IN A MILL.

OWRKING ON CLINKER & DUST BLEW INTO EYES CAUSING INFECTION(CHANGING DUST COLLECTOR BAGS).

EMPLOYEE WAS WORKING ON TOP OF F2 SILO WHEN A GUST OF WIND BLEW SOMETHING IN HIS EYE. EE WAS TAKEN TO SEE A DOCTOR AND A FOREIGN OBJECT WAS REMOVED FROM HIS EYE, RETURNED TO REGULAR DUTY.

EMPLOYEE STATES THAT WHEN HE SQUATTED DOWN HE FELT A BURN IN HIS KNEE. EMPLOYEE WAS TAKEN TO SEEA DOCTOR AND WAS DIAGNOSED WITH A STRAIN, PLACED ON MODIFIED DUTY AND THERAPY.

WORKING ON COOLER - TWISTED BACK WHEN CRAWLING INTO COOLER.

EE STATES THAT WHILE HELPING ANOTHER EE ADJUST THE LOAD CELLS ON THE KILN, THE WIND WAS BLOWING AND BLEW SOME FOREIGN MATERIAL INTO HIS LEFT EYE. HE WASHED OUT HIS EYES AND FELT IT WOULD BE AL RIGHT. HE GOT HOME AND IT WAS STILL BOTHERING HIM AND THE NEXT DAY HE REQUESTED TO GO TO THE DOCTOR. THE DOCTOR REMOVED TWO SMALL PIECES OF FOREIGN MATERIAL.

EE STATES THAT WHILE INSPECTING KILN END SEAL, A FOREIGN PARTICLE BLEW INTO LEFT EYE. EE WASHED OUT HIS EYES & AFTER GETTING HOME, IT STILL BOTHERED HIM & HE WENT TO HIS OPTHALMOLOGIST & HAD H IM REMOVE A PIECE OF DUST. THE DR GAVE HIM SOME DROPS TO PUT IN EYES AT HOME.

HORSEPLAY. PORTLAND CEMENT EE THREW A 1/2 BOLT WITH NBUT FROM THE TOP OF THE JIB TOWER STIRKING A IRWIN EE IN THE BACK.

EMPLOYEE WAS STANDING AND TALKING WITH ANOTHER EMPLOYEE WHEN WIND BLEW DUST INTO HIS EYES.

EE WAS OPENING THE SHOPE DOOR AT THE GHARAGE AND A GUST OF WIND BLEW DUST INTO HIS EYES. EE WASHED OUT EYES AND DID NOT ASK FOR MEDICAL ATTENTION UNTIL 5-4-98.

A TRUCK DRIVER DELIVERING PETROLEUM COKE WAS FOUND ON THE GROUND BY THE FRONT TIRE OF THE REAR TRAILER OF HIS RIG. PARAMEDICS WERE CALLED AND HE WAS TRANSPORTED TO THE HOSPITAL DUE TO AN APPAR ENT HEART ATTACK.

EE WAS SITTING AT THE SCREEN TOWER AND DUE TO PAIN IN THE ARM AND NECK THOUGHT HEMIGHT BE HAVINGA HEART ATTACK. MEDICAL EVALUATION INDICATED A MUSCLE STRAIN, BUT THERE WAS NO SPECIFIC OCCUR ANCE OR INCIDENT NOTED.
