Visiting corporate engineer was climbing down the ladder to the woodchip hopper when they slipped and fell approx. 5 feet onto the concrete. They sustained a fractured left ulna and radius as well as a contusion on their back and shoulder.

Employee was installing seals on a metal door when a draft pulled it inward closing it on EE's left thumb

Employee was maneuvering a patch piece of material and dust and debris on the existing structure fell into EE's eye, behind the safety glasses.

Employee was standing on part of the structure, tied off, with working safety choker around anchorage point. EE repositioned to reach the work when doing so EE misjudged the step and fell onto the handrail striking the ribs on the right side.

Employee was removing the bearing cap from a belt conveyer pulley for inspection. EE forgot to remove the tension from the counterweight and when EE removed the bearing cover bolts it shifted smashing EE's hand between the bearing cap and support structure. The result was multiple lacerations and fractures of the left ring finger.

Employee cut left middle finger holding a bolt with hand while using an impact on the nut attached to it.

During demolition of a project, EE was holding a piece of steel and after the tube was cut, they pushed more out on the bottom piece then the top piece which caused the top to come down and hit their hard hat and glasses, causing the glasses to cut their eyebrow.

Employee was checking the shear pin on the K-4 coal mill. They reached for a wrench to leave the area and the coal mill clean out door nuts vibrated loose and the door pinched their right index finger between the door and the rotary feeder housing. This resulted in a minor fracture and a laceration of the right index finger.

Employee was positioning a sill for the D4 roller mill, weighing approx. 200 lbs, into position. While bringing it in the employees hand became caught between the sill and the bearing housing of the D4 mill resulting in a fracture of EE's right hand pinky finger.

Finished unloading belt trailer, Driver states that the tailgate was not closed completely and attempted to close by hand. Pinched two fingers between the tailgate and rear of trailer.

Employee was installing a bearing housing on a screw conveyor. Upon adjusting the bearing housing to line up the bolts the housing shifted catching thumb between the mating surfaces of the bearing and screw conveyor housings.

Employee was carrying an industrial sized fan down the stairway to the cooler basement. While switching EE's grip on the load EE felt a pop in right shoulder.

Employee had irritation on back when cement dust fell down the back of EE's shirt on Feb 18. EE decided to get it looked at on Feb 22 and was treated with first aid and released to full duty. On Mar 5 EE went to an Urgent Care clinic and was treated for an infection

On 11-4-15 EE was hit on right shoulder by a wire cutter dropped from above. On 12-7-15 EE stated that the shoulder was sore and was transported to the clinic. EE was diagnosed with impingement syndrome and placed on modified duty and therapy.

EE reports a spider bite, does not know when or where it happened. He says it probably happened when he opened a bottom door on a rail car. Informed his supervisor on 5-6-15, asked to see a physician on 5-7-15. The physician gave him a shot and a prescription. Returned to full duty, no restrictions

Passenger elevator stuck between floors. Elevator was overloaded. Elevator running at 8:50 PM. No one was hurt

Rainy morning, EE walking towards the front office. EE slipped on wet concrete falling and hitting her left knee on the concrete. EE went to the doctor on the 2-4-15 and diagnosed with a fracture patella-closed. Brace dispensed for use. No restrictions, regular duty

EE standing next to a hopper. EE was tilting the hopper attempting to empty/dislodge cement dust. EE stepped away and slipped on mud causing a fracture of his Right Fibula.

EE stepped off of vertical ladder, stepped back onto uneven ground and twisted his ankle. On 11-25-14 EE called and said his ankle was swollen. He was seen by a physician diagnosed with a sprain and placed on modified duty.

EE standing 5' away from another EE swinging a 4# hammer trying to break a bearing loose. A piece of the hammer head broke off and impaled in the right arm of the injured employee. Medical treatment same day (X-Ray), Specialist next day, probed and decided to leave metal in arm. Stitches to close wound, return to regular duty

EE standing rinsing out a water bottle, turned and fainted. Fell down hitting his head on the floor (?)causing a contusion/laceration on his left eyebrow requiring stitches

EE removed metal shavings from steel stock being machined on the lathe. The shavings cut through his glove causing a laceration to his finger. EE transported to emergency care & received six stitches, returned to regular duty. EE will have outpatient procedure on 7-25-14 (regular scheduled time off) & will be on modified duty on his next scheduled day.

While helping to install liners in a mill, he got small chips of metal on the sleeve of his shirt. When he was going to lunch he wiped his eye on the sleeve of his shirt and got a small piece of metal on his eye.

While cutting a rubber skirting with a utility knife it slipped and cut his knee.

Twisted his back while climbing out of duct work

While installing a dead grate, the grate shifted and pinched his hand against the wall.

EE standing, changing canvas on airslide. Airslide on a sawhorse, his left foot was behind the leg of the sawhorse. As he turned to walk away his foot caught on the sawhorse leg causing him to twist his left knee. EE requested to see physician on 5-6-09 and was placed on modified duty.

While cutting flexible tubing with a knife, the blade cut into right leg. This puncture wound required stitches. Returned to regular duty.

Employee states as he was using jackhammer, foreign object got into left eye. After washing out several times it became irritated requiring medical evaluation and removal of foreign object.

Employee alledges as he was walking he slipped and fell possibly straining his back.

Injured employee raised his hand to move cardboard held by another employee. The employee who was holding the cardboard also had the razor he used to cut the cardboard in his hand causing a laceration to the injured employee on his left ring & small finger requiring stitches. No strenuous use of left hand.

EE was climbing out of an overhead crane bucket. He cut his left small finger when he grabbed the edge of the bucket (sharp rolled edge). Saw a doctor on 6/27/08, laceration was sutured and on modified duty.

Installing a burner pipe in a kiln when the cable of the manual hand winch failed causing winch to move, pinching the finger of the worker between the handle and a steel I beam. Pinched top of middle finger on right hand, hairline fracture seen in X-Ray when worker taken in for medical treatment.

Foremen was opening a crate when the plywood came apart. Foremen strained left shoulder.

Received burns from working in a bag house. He received the burns from lime dust in the bags. According to the employee the burns turned into a fungal infection. Claim is in doubt. (reported 11/19/07)

EE caught his right ring finger between two steel tubes on 10/4/07. On 10/10/07, the EE saw an industrial doctor and was diagnosed with a fracture on the tip of his right ring finger. Placed on modified duty.

Employee standing on a step ladder, inside locking brace bent/broke, employee fell to the floor, causing contusion to ribs, contusion to right thigh and a laceration to right elbow. Placed on modified duty.

Strained back (upper) and shoulder while tightening bolts on a cooler.

On 6/5/07, EE claims that while he was working on the Bucket Elevator, torching off huck pins, his foot rolled over some of the pins on the floor and twisted his right knee. EE did not report incident until the next day. Refused medical treatment as well as filling out injury form. EE stated he wanted to go see his Dr and get a cortisone shot, continued to work rest of week.

hot coal or spark ignited pulverized coal in dust collector bin, explosion occured on 4th floor, area is off limits and locked at all times. Safety features worked as designed and intended, rupture discs blew out, dust collector bags and cages, and tube sheet were damaged.

Employee was walking between 2 railroad tracks & twisted his left ankle. Ground is level but he said the rocks were loose. He reported this injury on 5/5/07, taken to see a doctor, placed on modified duty, and diagnonsed with a sprain.

Walking down steps, tripped and fell hitting right shoulder and left elbow. Placed on modified duty.

Employee was shutting off electrical disconnect box when spring-released handle hit his hand causing a sprain to his right pinky finger.

While pushing a dust hopper, a wheel got caught on a crack causing him to push and jerk to move the hopper. Asked to see a doctor on 10-4-06, diagnosed with thoracic strain. Placed on modified duty. Amended: 6th rib fracture.

Employee was putting a cover over a shoot when dust got in his eye.

While repostioning hydraulic ram, the spacer being used tipped and fell pinching his right index finger resulting in a contusion/laceration requiring stitches. Modified use of hand, no strenuous or repetitive gripping.

Ee was hanging a mailbox on a pole, it had rained and the ground gave way causing him to overextend his left knee. Take to clinic, diagnosed with a strain. He placed on modified duty but able to perform all functions of his job.

Employee was cleaning airslide, turned to clean another airslide and felt pain in his right knee. His boot caught on the floor grating. Sent to see dr on 8-4-06, diagnosed with a strain, possible ligament tear. Placed on modified duty.

EE was removing a dust collector bag which had some material, felt pain in lower back. Asked to see doctor the next day and placed on modified duty. Diagnosed with back strain.

He was lowering a mag coupling and it dropped about a foot, and the rope caught his finger and jerked it.

Standing on electric cart bed while working on crane bucket in Crane Storage building. Possibly tripped and fell. Injured right ankle and hip and skinned knee. Transported to see Physician. Physician placed on light duty.

WHILE MOVING A PIECE OF WOOD TO COVER A HOLE, EE GOT HIS FINGER CAUGHT BETWEEN THE WOOD AND A DROP BOX (CUT ON RIGHT INDEX FINGER).

EMPLOYEE WAS OPENING THE SLIDE GATE ON A RAIL CAR. THE BAR HE WAS USING FELL ONTO A PILE OF CEMENT AND SPLASHED UNDER HIS SAFETY GLASSES. SENT TO SEE A DOCTOR AND WAS DIAGNOSED WITH CORNEAL ABRASION. SAW A SPECIALIST ON 10-27 AND PLACED ON NO DUTY. BACK TO WORK ON 10-31-05.

EMPLOYEE BENT DOWN TO LIFT A BAG OF CEMENT AND FELT PAIN IN LOWER BACK. HE WAS TAKEN TO MEDICAL CLINIC AND DIAGNOSED WITH LUMBAR STRAIN, PLACED ON MODIFIED DUTY.

Hot raw mix flushed by pan conveyer, employee extinguished a burning guzzler hose, stepped on hot raw mix, material splashed under his pant leg burning him above his sock. Taken to emergency and diagnosed with first and second degree burn. Placed on modified duty.

EE states that on 6-22-05 he was in basket of manlift troubleshooting, as he maneuvered manlift on roadway it stopped & he hit his left shoulder on the rail. On 9-19-05 he complained of his shoulder, sent to a dr & diagnosed w/shoulder strain, regular duty & attended 1 therapy session. He refused to see dr again & made appt w/another dr on 9/26. Placed on light duty.

EE was sitting on chair in cab of overhead crane. Adjustable back of chair gave way suddenly, jerking EE. Felt pain in upper back. Diagnosed with upper back strain. Returned to regular duty; given muscle relaxer medication.

While removing a door from a mill, cement dust blew into his eyes.

Moving a steel plate on a pallet that fell onto his finger.

Employee was using a bar to hit a plugged bine bottom and struck his left index finger on an angle iron support. the right side tip of his index finger was cutt off. x ray's were negative, no bone involved, sent back to full duty. on 7-15-04 he was seen by a dr and a graft was performed on his finger and sent back to full duty

EMPLOYEE WAS REMOVING A SHAFT MOUNT GEAR REDUCER, THE GEAR PULLER SLIPPED & HIT EE ON HIS LEFT KNEE, CAUSING A LACERATION REQUIRING STITCHES.

WALKING FROM ONE LOCATION TO ANOTHER. WHILE WALKING DOWN A RAMP, HE FELL ON MILL BALLS COVERED BY DUST. HIS BACK IS SORE AND DID NOT SEEK TREATMENT UNTIL MAY 14, 2004.

EMPLOYEE REMOVING BOLTS FROM COAL MILL FAN, STEPPING BACK AND TWISTED KNEE ON EDGE OF CONCRETE BASE. PLACED ON MODIFIED DUTY, DIAGNOSED WITH A STRAIN OF HIS LEFT KNEE.

EE WAS USING AN IMPACT WRENCH TO REMOVE BOLTS. THE SOCKET IS SECURED TO THE WRENCH WITH A PIN AND O-RING TO KEEP IT IN POSITION. THE O-RING SLIPPED OFF CAUSING THE PIN TO BE EXPOSED, CAUSING A LACERATION ON THE PALM OF HIS LEFT HAND. THE LACERATION REQUIRED STITCHES AND PLACED ON MODIFIED DUTY.

WHILE PLACING A CLIP ON A COOLING SYSTEM RADIATOR, THE RADIATOR SLIPPED OFF THE FRAME AND PINNEDEE AGAINST A RAILING.

EMPLOYEE WAS PREPARING A NEW HARD HAT TO WEAR. HE STEPPED BACK TO TOSS THE OLD HARD HAT IN THE GARBAGE AND STEPPED ON A FLOOR JACK CAUSING HIM TO FALL. EMPLOYEE REQUESTED TO SEE A DOCTOR ON 9- 19-03, DIAGNOSED WITH CONTUSION LEFT KNEE, LEFT SHOULDER. PLACED ON MODIFIED DUTY. THE JACK WAS NOT STORED PROPERLY.

NOISE INDUCED HEARING LOSS. EE HAS WORKED IN MINING AND RELATED INDUSTRY SINCE 1983.

WHILE TRYING TO REMOVE A PIN FROM ELEVATOR CHAIN, A CHIP FROM A WASHER SNAPPED AND PENETRATED HIS RIGHT ARM.

EE WAS OPERATING THE TIRE MACHINE, HE WAS BREAKING THE BEAD ON A TIRE FROM A RIM. WHEN THE BEAD BROKE FREE FROM THE RIM THE LEVER ONT HE BEAD BREAKER STRUCK HIS RIGHT HAND EE WAS TAKEN TO SEE A DR X RAYS WERE NEGATIVE, DIAGNOSED WITH A CONTUSION. PLACED ON MODIFIED USE OF RIGHT HAND.

WORKER HAD DUST FROM UNKNOWN SOURCE BLOW INTO RIGHT EYE. ATTEMPTED TO REMOVE, UNSUCCESSFUL. P.A. REMOVED. WORKER SUSTAINED CORNEAL ABRASION RETURN TO WORK 6/30, REGULAR DUTY.

EE WAS POSITIONING A HYDRAULIC RAM, THE RAM FLIPPED ON ITS SIDE & CAUGHT HIS LEFT SMALL FINGER RESULTING IN A LACERATION NEEDING STITCHES.

WORKER WALKING ALONG CATWALK IN PLANT AREA. WORKER TRIPPED OVER RAISED PIECE OF GRATING CAUSING HER TO FALL. STRUCK HEAD AND KNEE.

EE WAS REMOVING A SHEAR PIN ON A DRAG CHAIN. HE WAS USING A PUNCH TO DRIVE OUT THE PIN, MISSED THE PUNCH AND HIT HIS THUMB. HE WAS TAKEN TO THE E.R. AND DIAGNOSED WITH A THUMB TIP FRACTURE; PL ACED ON MODIFIED DUTY.

EE WAS DESCENDING A STEP LADDER & MISJUDGED THE LAST STEP CAUSING HIM TO FALL HITTING HIS TAIL BONE ON THE BACK OF AN ELECTRIC CART. DIAGNOSED WITH A CONTUSION, BRUISED TAIL BONE & PLACED ON M ODIFIED DUTY.

EE STATED WHILE USING THE POWER SPRAY CLEANER HEWAS SPLASHED WITH WATER AND DEBRIS IN HIS RT EYEHE REPORTED THE ACCIDENT ON 12-10-02. HE WAS TAKEN TO SEE A DOCTOR AND WAS DIAGNOSED WITH FORIEG N BODY IN HIS RIGHT EYE.

EE WAS USING A CHIPPING GUN TO CLEAN OUT A ROTARY FEEDER, WHILE APPLYING PRESSURE TO THE CHIPPING GUN HE FELT PAIN ON HIS RIGHT KNEE. HE WAS TAKEN TO EMERGENCY AND WAS DIAGNOSED WITH STRAIN RT BUTTOCK AND HAMSTRING. EE WAS PLACED ON MODIFIED WORK STATUS.

SUBCONTRACTOR DID NOT ROPE OFF AREA UNDER SCAFFOLD AND DID NOT HAVE ENOUGH PLANKS TO PREVENT A PIECE OF PIPE FROM FALLING APPROX 15 FT. EE WAS WORKING ON INSTALLING GRATES IN THE INCLINE. THE PIPE HIT HIS SHOULDER CAUSING A CONTUSION.

USING AIR CHIPPER TO CHIP ROCK, CHIP GOT INTO EYE.

SPRAINED BACK WHILE LIFTING BEAMS OFF A FORKLIFT.

WHILE REMOVING A GRATE A PLANK FELL ON HIS HAND.

EMPLOYEE WAS USING A PRY BAR TO ALIGN BOLT HOLESON AN AIR SLIDE. THE PRY BAR SLIPPED AND GRAZED HIS CHEEK CAUSING A LACERATION REQUIRING STITCH-ES.

EE WAS USING A 16 LB. SLEDGE HAMMER TO SLUG DOWNBASE BOLTS ON AC3 MILL. NEXT DAY HE TOLD HIS SUPERVISOR THAT HIS SHOULDER WAS STILL BOTHERING HIM AND REQUESTED TO SEE A DOCTOR. HE WAS DIAGNOSE D WITH A STRAIN AND PLACED ON MODIFIED DUTY.

REMOVING 142# PLATE CLAMP FROM PLATE. CLAMP ROTATED AND THERE WAS A POP IN LEFT SHOULDER.

NOISE INDUCED HEARING LOSS. EE HAS WORKED IN THEPLANT SINCE 1957.

EE WAS REMOVING A BELT GUARD TO REPAIR A CHUNK BREAKER. EE INJURED HIS HAND ON 7-10 BUT DID NOT SEEK TREATMENT UNTIL 8-7 WHEN HIS HAND DID NOT IMPROVE.

EMPLOYEE WAS USING PRY BAR TO REMOVE RADIATOR ON HAUL TRUCK #64, PRY BAR SLIPPED CAUSING EMPLOYEE TO ABRUPTLY JERK CAUSING PAIN IN LOWER BACK. SENT DOCTOR AND DIAGNOSED WITH LOWER BACK STRAIN, PLACED ON MODIFIED DUTY.

WIND BLEW SOMETHING IN EYE WHILE WORKING ON BUILDING A PLATFORM IN A MILL.

OWRKING ON CLINKER & DUST BLEW INTO EYES CAUSING INFECTION(CHANGING DUST COLLECTOR BAGS).

EE STATES THAT WHEN HE REACHED TO PULL HIMSELF ONTO THE ACCESS LADDER ON LOADER #136 HE FELT PAIN IN HIS ARM. ON 3-19-01 HE STILL FELT DISCOMFOT AND REQUESTED TO SEE A DR. DIAGNOSED WITH STRAI N/POSSIBLE TEAR RIGHT BICEPS. ON MODIFIED DUTY.

EMPLOYEE WAS WORKING ON TOP OF F2 SILO WHEN A GUST OF WIND BLEW SOMETHING IN HIS EYE. EE WAS TAKEN TO SEE A DOCTOR AND A FOREIGN OBJECT WAS REMOVED FROM HIS EYE, RETURNED TO REGULAR DUTY.

EMPLOYEE STATES THAT WHEN HE SQUATTED DOWN HE FELT A BURN IN HIS KNEE. EMPLOYEE WAS TAKEN TO SEEA DOCTOR AND WAS DIAGNOSED WITH A STRAIN, PLACED ON MODIFIED DUTY AND THERAPY.

EE WAS INSTALLING AN AIR SLIDE SECTION WHEN IT SHIFTED & CAUGHT HIS FINGER BETWEEN 2 SECTIONS. HE WAS TAKEN TO ER & DIAGNOSED WITH A HAIRLINE FRACTURE & LACERATION, GIVEN A SIMPLE SPLINT & RETURNED TO WORK. EE IS PERFORMING REGULAR WORK ACTIVITY.

WORKING ON COOLER - TWISTED BACK WHEN CRAWLING INTO COOLER.

EMPLOYEE STATES THAT ON 12/13/00 WHILE DRIVING UP A RAMP ON A CAT 990 LOADER THE BACK REST ON THE SEAT SHIFTED AND FELT PAIN IN THE NECK. ON 12/20/00 HE REQUESTED TO SEE A DOCTOR AND WAS DIAGN OSED WITH A NECK STRIAN. RELEASED TO FULL DUTY, NO RESTRICTIONS AND SCHEDULED FOR THERAPY.

WORKING ON CONVEYOR BELT.

ON 12-6-00 EE REPORTS HE HIT HIS ELBOW WHILE AT WORK ON 12-5-00. ON 12-7-00 HE WAS TAKEN TO SEE A DR. AND DIAGNOSED WITH A INSECT BITE. ON 12-8-00 HE WENT BACK FOR FOLOW UP AND WAS REFERED TO A HOSPITAL AND DIAGNOSED WITH A CONTUSION AND ANINFECTION, ADMITTED ADN TREATED DURING THE WEEKEND, RELEASED AND PLACED ON MODIFIED DUTY.

EE WAS AIR ARCING INSIDE A HAUL TRUCK BED. HE FELT SOMETHING IN HIS EYE. HE ASKED TO SEE A DR. ON THE 27TH. FOREIGN OBJECT REMOVED FROM HIS RIGHT EYE.

CLEANING COMPARTMENT IN BAG HOUSE.

REMOVING BAGS FROM CEMENT DUST COLLECTOR BAGHOUSE.

TWISTED BACK LIFTING A TOOL BOX. NOT REPORTED UNTIL 8-23.

EE WAS CLOSING THE DOOR TO THE ENGINE COMPARTMENT ON A SKID LOADER. THE LATCHING MECHANISM WAS NOT LINED UP, THE LATCH SPRUNG BACK STRIKING THE EE ON HIS RING FINGER, RIGHT HAND. SENT TO HOSPI TAL AND DIAGNOSED WITH A FRACTURE, SPLINTED AND SENT BACK TO WORK, CAR OF HAND IS TO KEEP DRY FOLLOWUP IN THREE WEEKS.

USING TORQUE WRENCH, LEFT HAND GOT CAUGHT BETWEEN THE WRENCH AND THE ROLL PRESS.
