Employee was walking through the plant to get to the parking lot at the end of EE's shift. EE walked past a flatbed truck with a large piece of expanded metal laying on it and scraped EE's arm along it, resulting in a three-inch laceration, requiring three stitches. No lost time.

Installing a hydraulic Pump on a boat, pump slipped on rigging and fell on thumb.

The employee was called to Stage 1 West and started air lancing to clear any material in the way. After the employee finished air lancing, EE noticed that EE's right was hurting and swollen.

The employee was transitioning/moving from the tugboat back onto the dredge after an impeller had been staged on the tugboat to be taken away. While walking past the impeller, the employee's right index finger grazed against a sharp edge on the impeller, resulting in a laceration. The employee went to have it examined and received stitches to the wound.

Holding on to upper rail of boomlift basket when coming down the basket made contact with the silo pinching their finger causing a fracture.

Employee was having continuous left shoulder pain from performing manual labor. Resulted in left shoulder sprain.

Employee operating Cat 775 haul truck was leaving River Bench Pit when they felt a sharp pain in their lower back.

Employee 1 and Employee 2 were manually moving a stacker base approximately five feet. During the movement, one adjusted their body position and footing. While twisting and repositioning, they experienced pain in left knee. The employees safely set the stacker base down and stopped work.

Elevator stopped between floors with two miners on board.

During an inlet sweep, the employee turned on the water, and the hot material blew back and got underneath the employee's gloves and burned EE.

Miner was on foot scouting a new drill pattern. EE bent to pick up EE's wheel chocks and felt a muscle pull in EE's back muscles.

Employee was shoveling material from under the conveyor belt and felt slight pain in lower back (failed to report), operating haul truck and went home. It was not until 6-3-2026 (following day) when employee had a hard time getting out of bed for work when it was reported. Resulted in acute lumbar myofascial strain

Employees noticed the round door for the dust collector was not sealed. Since they could not close the door properly, the control room was called to shut down the fan and close it properly. An EE was standing in front of the dust collector door when the fan was shut down. Immediately, positive air pressure built up and shot the door out, hitting the EE on the face.

while servicing a mixer truck the employee stated they twisted their knee and strained their back

The employee was assigned to inspect the clinker weigh feeder due to clinker chunks that could plug the system. They lowered themselves through the safety cage to reach the clinker weigh feeder, dropping their hard hat as they moved. They hit the pins with their head as they moved up, causing a laceration.

As employee was walking on catwalk to plant from production break room EE tripped while stepping up a set of two stairs. As the employee was falling, EE extended EE's left arm out in front of EE to break EE's fall onto the grating. The area was clear of debris with a light coat of dust. At this time, we don't know the result of injury but there is bruising to chest and bicep.

The employee had several knee replacement surgeries in the past that may have contributed to the weakening of their leg. Employee was walking backwards pulling a water hose when their leg gave out and their femur broke.

Mechanic was adjusting a wiper blade when their shirt or tool caught on conveyor belt pulling their arm into head pulley causing a dislocated elbow and fracture on ulna and missing finger nail.

While removing a blockage from silo 42 material flowed from a 3" access port causing cement to get into the miner's ear.

Employee's right hand was in the line of fire of a block and chain. The block and chain was under tension and shifted which struck employee's index finger. This resulted in a broken finger.

Employee was removing screens from shaker for cleaning when one of the screens caught the harness they were wearing causing them to fall and hurt a knee.

A contract truck driver was delivering Ione clay to the plant. After dumping almost all of the material from the end dump trailer. The driver climbed inside the trailer to remove some material that was stuck to the bottom of the trailer. The driver was found unconscious and unresponsive inside the trailer.

EE was using a cat telehandler to move urethane sheets from the shop to chem ad, load shifted and EE stopped and was attempting to readjust the load when they heard a small pop in their right shoulder.

Employee fell while walking through a doorway. They reached out to catch themselves, but their hand gave way and they struck their elbow onto the concrete, resulting in a hairline fracture of the elbow. Fall was from same elevation with no obstructions or material in their path.

Employee was using a cutting torch to cut scrap metal. The torch caused a small brush fire behind the employee which caught their pant leg on fire, burning through two layers of clothing. Employee sustained a partial thickness second degree burn on the back of left leg just above the ankle.

Non injury unplanned ignition occurred at the H7-6-CM1 Day Silo.

Employee was bending over to pick up a piece of pipe when they felt a strain in their back. Employee did not pick up the pipe only hurt their back bending over.

Employee was exiting a 633D scraper using 3pts of contact when they felt a pop in right shoulder. Employee did not think nothing of it until the following day when the shoulder felt much worse.

The employee was cleaning a cement coupling with a drill grinder. The drill bit became dislodged from the drill and lacerated the employee's left index finger.

PCS received a displaced rupture disk alarm in Forney indicating an explosion in the Coal Mill 1 system. The investigative process is continuing at this time to identify the root cause. All CM1 Recovery Phase work will be performed safely and methodically.

Employees were on the ground laying a spreader bar down to begin break and as it was laying down EE had hand on the underside as opposed to on top and EE's first instinct was to push it and it pinched EE's hand between the spreader bar and saw horse smashing three fingers.

Employee was attempting to hang hoses onto wall racks when the base of ladder slipped out from under EE. This resulted in employee falling from a height of 12' causing a broken femur and broken hand all on the left side of body. EMS was called to transport to hospital.

The employee was removing a rock from the grizzly head pulley at the 551BC101 conveyor belt when another rock fell and smashed employee's left index finger.

While installing a door liner in a tight space, a failed hook keeper caused the plate to swing, pinning the employee against a scaffold pole. The impact injured the employee's left knee. Witnesses operated comealongs from ladders. Employee had restricted duty. Inspect all rigging equipment, chains, and hooks. Enhance safety training and protocols to ensure a safer work environment.

On April 3, 2025, at 3 PM in Arizona, an employee cutting a liner plate got it stuck. After securing and cutting it, the plate twisted, pinching their hand. They iced it, continued work, and later saw a doctor who diagnosed a left hand metatarsal fracture. Supervisor was notified; swelling reduced.

Grinding hinge door bolts off with an 4.5 grinder when the grinding wheel came apart striking technician in the left side cheek area from lip to upper cheek, EE was wearing safety glass, no face shield. EE received medical treatment from Emergency room and was released same day and put on light duty next 5 days.

Contractor was painting an electrical box on 3rd floor of F Area. EE needed a paint brush, so EE entered the elevator going down. Contractor became lodged between floors, 911 was called and tech for elevator was called also. Contractor was retrieved from elevator without injury.

The employee was walking up the stairs on the front (west end) of the building and missed the top/last step. Employee fell face first and opened a 2" gash above right eye.

Unloading material from a skid. On the last pick EE found a nylon strap still attached. EE reached in with the cordless grinder to cut the strap. When the strap was cut the load shifted and EE's thumb was caught between the load and the diagonal brace on the skid.

Employee was entrapped in elevator for over 30 minutes. Elevator blew a main fuse.

A miner stepped downward and felt a pop followed by pain in the left thigh.

Employee notified manager that they were having discomfort in their lower back which was radiating to their neck and left shoulder. Employee was operating a CAT 775 haul truck and stated that it was due to bumps in the pit and haul road.

Miner slipped and fell onto their left shoulder while walking on a wet metal surface.

Cutting a conveyor belt to length the razor knife slipped and struck the employee in the right thigh just above the knee causing a laceration requiring 9 stitches.
