Mostrando los 16An over the road truck driver dropping off a load was struck by a golf cart after they had exited their vehicle. The driver indicated that they were not injured but later reported pain and went to a doctor. Medication was prescribed with two days out from work from the doctor.
Employee was adjacent to dock while equipment was being unloaded from a commercial truck with a forklift. The pallet that the equipment was on failed causing it to fall cutting the employee's hand and fracturing their wrist.
No contributing factors, no damage to equipment and no damage to property.
There was an incident where a driver was driving truck when they met IE driving IE's truck. EE swerved to EE's right causing EE's arm of EE's tailgate to strike IE's side mirror, which made the mirror break IE's side glass. It caused a small cut to IE's face. All involved were checked out at BASF nurse & had a drug test at the Urgent Care.
Employee Struck by Pallet with Product transported by Forklift. Employee's left ankle was struck causing sprain that required medical attention. Ankle was put in a boot and Employee was put on Restricted duty with medical follow up in 6-8 days.
Operator was walking in packaging area with the flow of forklift traffic. Forklift driver was carrying a pallet load of packaged product. Forklift driver struck operator on left side knocking operator to the ground. Operator suffered bruising, skin abrasions and pain in left shoulder.
Crude truck was dumping on the Dixie stockpile. As the dump fully extended the unit overturned onto its left side.
A BASF employee was releasing the hand brake on a railcar when the tension on the brake caused the handle to recoil. The recoil caused the employee's right pinkie finger to come in contact with the brake chain of the rail car. The employee was diagnosed as having a fractured finger and received two stitches at the tip of his finger.
Employee (Mechanic) got his middle finger (right hand) caught between the cable and load block pulley on a mobile crane. Emp. was attempting to push cable back into pulley groove when injury occurred. Emp. was not wearing gloves.
Employee arrived at nurse's clinic at 7:30 am to report complaints of increasing pain in both hands.
Employee was dropping railcars when it bumped into another railcar. The impact caused him to be thrown against the side of the railcar, which caused multiple bruises to his right side.
EE was operating the water truck on the haul road. He started up the ramp entrance to the pit area, as he got to the top of the hill, his truck started spinning on the wet road. The truck slip ped backwards, down the ramp and hit the berm, which caused the truck to turn over. There was cab damage to the truck.
HSI driver was traveling east on Gibraltar Mine haul rd. As the driver started negotiating to make a left turn curve he lost control of his vehicle.
Truck No. 1180 rolled over on a curve hauling a load of kaolin. The truck was running too fast for conditions and could not make the curve.
Left the pit loaded, came to a point in the haul road where setting sun hits the mirror. Other truck passed on his left in opposite direction, sun hit mirror & blinded him. He was close to the edge of the road next to the berm & drifted even closer right tire caught the berm & almost came to a stop. Berm gave way/truck rolled down slope 60 ft. climbed out window and climb up hill.
