End Dump operator exited cab to reset master switch. End dump started rolling downwards. Employee hurried back to the cab to stop the truck from rolling. 20 Minutes later employee stated lower back was hurting. 10/31/16 employee started physical therapy making this injury reportable to MSHA.

Employee was performing pre-op inspection on pickup truck at Ops building. Employee was standing inside the driver's door. Employee reached in, placed foot on brake and put the truck in reverse to check back-up lights. Foot slipped off brake pedal and onto accelerator causing truck to go in reverse. Employee was knocked to the ground and the front left tire rolled over legs.

Employee was operating a Case 620 scraper tractor with pans loaded with red dirt headed to the dump area. The employee drove through a rough spot in the road and jammed EE's left pinky finger in the steering wheel. Employee reported the incident to the Supervisor and was taken to the Emergency Room. X-rays confirmed there was a small fracture in the left pinky finger.

A articulating Volvo A40F water truck with a full load of water was traveling on a haul road. The truck was not meeting any traffic at the time he allowed the trucks right front tire to veer off the edge of the road. The operator tried to overcorrect to bring it back on the road. This caused the water truck to roll on it's side. The operator sustained a broken collarbone.

Employee was operating a hauler while descending the #3 Stockpile ramp in route to the crusher. The hauler traveled over uneven terrain causing the seat to bottom out straining the employees lower back.

Dirt contacted the front windshield of a 992 CAT loader causing small glass shards to contact employee face. The employee was transported to the doctor to have a small piece of glass removed. The doctor prescribed anti-bacterial ointment.

The injured employee was riding in the passenger seat of a tractor. The tractor operator, travelling at an unsuitable speed for the conditions, attempted to cut a corner, crossing uneven ground, causing both riders to be jolted around. At this point the injured employee's elbow struck some metal switches on the nearby armrest, breaking the switches and lacerating his right elbow.

Employee transporting coal to crusher along a haul road when the Hauler contacted a pot hole, causing a muscle strain to the employee's back.

Employee started up ramp with loaded haul truck when the truck shifted from 1 to 2 gear, 3-4 times. Employee didn't seek medical attention at the time of the incident but did ask to see a doctor on 6/09/14. The hard shifting resulted in the employee suffering from cervical strain/spasm. He was issued restrictions on 6/10/14 from a follow up visit, making this a recordable injury

Employee injured his neck while riding in a pump truck in route to a barge pump.

Operating tractor pulling tandem pans. Excavating and hauling soil. Operator fell asleep returning from dump area. Tractor veered off road and hit cable crossing pole. Damage to front of tractor, no damage to pole or cable. Area was well lighted. Operator claimed no injury. Drug test negative. EE called safety on 12/4/13 & claimed lower back ache. Taken for exam same day.

While pulling a shaft out of the deflection sheave, load shifted smashing employee finger between load and fork of loader.

Employee was operating 38077 Heavy Haul Truck when it skidded and jack knifed the trailer and striking the bump stop causing employee to bump his head in the cab.

Employee reports neck pain when he turned his head to the right while he was operating a hauler. The employee alleges that it ties to an old injury dating back 2 years.

Contributions to the accident were a failure to maintain situational awareness. When yielding to heavy haul equipment employee moved over too far to yield the ROW striking a barrier with the right front tire. * Upon post accident drug screen employee tested positive to be under the influence.

Employee received cervical strain when shift change vehicle hit bump in road. On 07/31/2012, employee did not work full shift and was prescribed physical therapy treatments causing injury to be reportable.

An employee received a back injury while operating a Cat 777 end dump. The employee sustained back spasms. The Cat 777 seat suspension ride was found defective and replaced. The employee did not initially receive restrictions but it was determined on a follow up visit on 07/20/2012 that he would need physical therapy.

Contributing conditions were rough terrain, hard to spot unlevel ground conditions. When traveling over the rough terrain, the employee encountered a "pot hole" in the path of travel. When the machine dipped into it the operator was jarred in his seat.

Employee was operating hauler # 62006, it was loaded and driving to the crusher from B-5. When the truck shifted on up to 5th gear, the bouncing of the seat caused employees back to jar resulting in lower back pain and spasms.

Employee was cleaning coal in the D-3 pit area. As he was backing up with the loader bucket in the air, the left back tire rolled off the ramp edge causing the machine to slowly roll onto its left side. The operator sustained bruising to his left shoulder, elbow, and shin. He was taken in for medical evaluation and placed on restricted duty.

Employee was returning to the pit with an empty hauler after dumping at the truck dump. The employee jack knifed the hauler in the haul road, and the impact to the bump blocks caused a strain in the neck and upper back. The employee was wearing his seatbelt.

A forklift was being used to install a pin in a dragline bucket's left drag hitch extension. The forklift operator was using the forklift tines to put side pressure on the pin to seat it. The tines slipped off the pin and contacted a scaffold that one employee was standing on. The jarring motion caused pain in the employee's lower back.

Employee was moving trail cable in with a CAT 930H. The equipment went over a deep dozer track. The back tires dropped into the dozer track. As a result the employee suffered a lower back strain.

Employee was riding in shift change bus when it hit two large holes in the roadway. The injured employee was sitting near the rear of the bus. The incident caused him to hit his head on the ceiling and be thrown to the floor causing pain in his lower back and an abrasion to his shin. Work restrictions were issued.

Employee operating front end loader reached up to turn the AM/FM radio knob and strained right shoulder. On 01/25/2011 the doctor ordered multiple physical therapy treatments.

Employee drove his haul truck off the roadway and bumped his head on the steering wheel, resulting in cuts that required stitches.

A mechanic was helping a group of employees install a roller and roller guide assembly onto a Hitachi Backhoe under carriage using a forklift. The assembly fell off the forklift and mashed the mechanic's left hand fracturing 4 of his fingers.

Employee was making adjustments to winch at the back of the Caterpillar log skidder. They were in the process of pulling a pump barge up an incline (ramp). While he was on the back of the machine on the deck, the machine began to roll down the incline and the employee jumped from the machine. The left hip and right heel were fractured.

Employee was operating end dump when he hit a slick area on the ramp. The employee lost control of the vehicle and the equipment turned over.

EE was replacing hauler parking brake bracket, was lining up the bolt holes and started to install the keeper bolts when the retainer bracket slipped and mashed EE's left thumb between retainer and tail housing shaft of transmission.

A Journeyman was pulling on a loader tire with a tire truck. The supervisor was beside the rear of the tire reaching over behind the tire operating a porta power. As the tire truck hand was pulling the tire the tire hand slipped and the tire retracted back and the tread of the tire struck the supervisor and knocked him against the frame of the loader fracturing his scapula.

A mechanic was assisting a group of employees change our a drag chain on a dragline bucket rigging. One employee was utilizing a loader with a hydraulic hammer attachment to remove hitch pin. During the pin removal a chip of metal flew off the hammer head and struck the other mechanic in the left forearm requiring surgery to remove the chip and repair a damaged artery in his arm.

While operating a 740 end dump, the employee slammed on the brakes to avoid an incident. The employee stated that their knees hit the radio and ashtray in the truck.

Employee was working with a group of mechanics installing a drag hitch keeper plate on a dragline bucket. A IT28 loader with a hydraulic hammer attachement was being used to drive in and hold the keeper plate in place. While installing the plate bolts, the IT28 unexpectedly moved mashing the employees left middle finger between the hammer head and the plate requiring 4 stitches.

Employee was turning to sit down in a shift change bus seat. As he was turning the bus took off causing him to twist in the direction he was already turning. He felt pain in his left hip. He has an existing condition of lower back problems causing his left sciatic nerve to be aggravated. The incident caused his existing condition to "flare up".

Employee was operating a scraper and was approaching a loaded coal hauler to push it up the ramp after it lost traction and stopped on the ramp. As the scraper operator was approaching the hauler, the hauler tried to pull the ramp again and began to roll back and rolled into the scraper jarring the scraper operator, causing soreness in his neck and back.

While employee was operating a Euclid End Dump he received a pinched nerve in his neck from the bed closing hard and jarring the truck after dumping his load of dirt.
