Employee was task focused, observing a choked feeder. EE stepped in front of an LHD that was in motion and was bumped by the LHD. The operator knew where the employee was before starting. EE put it in reverse for a short distance and back up alarm went on. When EE started forward and didn't see employee so EE stopped. Employee was already struck.

Mostrando los 13While running the scaler the back came down on the front across the boom to the left front catwalk causing the employee to be jarred in the cab. The employee sought medical attention and was returned to full duty. EE later sought further medical attention and was placed on restricted duty on 6/30/2020.
Employee operating Bobcat back dragging uneven ground. Hit hole and jarred neck. Employee failed to report injury in timely manner. Sought diagnosis from 3 different Health professionals until informing management of issue. Placed on restrictions as of 2/28/2020.
Employee was operating a CAT 236B Skid steer doing cleanups under belt lines. Employee stated ee struck something and was jolted forward. EE hand made contact with bumper plate and chest hit control joystick slamming bucket down smashing Right index finger. Finger was damaged extensively and required amputation.
Employee was operating an underground loader and hit a bump in the road. The employee felt pain in the back and pain down the leg. Therapy was prescribed. Employee is working with restrictions.
A tube winder operator was poking some stuck cans near a roller when the roller pulled the poke rod under the roller. The operator instinctively tried to grab the rod but missed. EE's hand was pulled into the roller and the machine was stopped and EE got EE's hand out. The hand was x-rayed and no fracture was found. No stitches were indicated.
Employee struck his head on the top of the canopy of the small LHD. Employee was cleaning up faces and the rear tires came off of the ground and slammed down. The employee stated that he lost his hard hat and struck his head on the canopy.
EE was operating fork truck in an easterly direction when he slowed down at an intersection in warehouse. Another fork truck operator was proceeding in a southerly direction and became distracted while looking for her scan gun. EE looked up and saw fork truck and attempted to stop to no avail and a collision resulted.
Employee incurred laceration to left thumb on steering wheel cap of Electrical Gator UTV. Employee struck lump in roadway of 12N 57 Block (North Panel) which caused steering wheel to forcefully jerk to the right causing laceration to left thumb. Employee received sutures and returned to work with no restrictions.
Two EEs worked from a work deck suspended beneath the production shaft west skip, to replace a skip pocket guide. They asked for the deck to be lowered slightly quicker. The deck held up at tight spot & was struck by the skip, moving the deck approx 10' until the cables were taut, rapidly stopping the deck. The decks stopping jammed EEs into the deck causing their injuries.

A forklift was used to pull chain. The forklift came forward and was strapped to the chain. EE did not notice fork got under a drag slat. When the forks were tilted back the fork came from under the slat, sprang up and hit EE on the left eyebrow cutting him. He got 5 stitches.
While lifting and lowering an outrigger extention block and pad on the condor 100 lift truck, employee states he mashed his right hand between the extension block and water cooler mounting bracket as he was lowering the extension block to ground level.
